Purpose Melanoma is the most commonly fatal form of skin cancer, with nearly 50,000 annual deaths worldwide. We sought to assess long-term trends in the incidence and mortality of melanoma in a state with complete and consistent registration. Methods We used data from the Connecticut Tumor Registry, the original National Cancer Institute SEER site, to determine trends in invasive melanoma (1950-2007), in situ melanoma (1973-2007), tumor thickness (1993-2007), mortality (1950-2007), and mortality to incidence (1950-2007) among the 19,973 and 3,635 Connecticut residents diagnosed with invasive melanoma (1950-2007) and who died as a result of melanoma (1950-2007), respectively. Main outcome measures included trends in incidence and mortality by age, sex, and birth cohort. Results In the initial period (1950-1954), a diagnosis of invasive melanoma was rare, with 1.9 patient cases per 100,000 for men and 2.6 patient cases per 100,000 for women. Between 1950 and 2007, overall incidence rates rose more than 17-fold in men (1.9 to 33.5 per 100,000) and more than nine-fold in women (2.6 to 25.3 per 100,000). During these six decades, mortality rates more than tripled in men (1.6 to 4.9 per 100,000) and doubled in women (1.3 to 2.6 per 100,000). Mortality rates were generally stable or decreasing in men and women through age 54 years. Conclusion Unremitting increases in incidence and mortality of melanoma call for a nationally coordinated effort to encourage and promote innovative prevention and early-detection efforts.

Introduction Monitoring cancer trends can help evaluate progress in cancer control while reinforcing prevention activities. This analysis examines long-term trends for selected cancers in Canada using data from national databases. Methods Annual changes in trends for age-standardized incidence and mortality rates between 1970 and 2007 were examined by sex for 1) all cancers combined, 2) the four most common cancers (prostate, breast, lung, colorectal) and 3) cancers that demonstrate the most recent notable changes in trend. Five-year relative survival for 1992–2007 was also calculated. Results Incidence rates for all primary cancer cases combined increased 0.9% per year in males and 0.8% per year in females over the study period, with varying degrees of increase for melanoma, thyroid, liver, prostate, kidney, colorectal, lung, breast, and bladder cancers and decrease for larynx, oral, stomach and cervical cancers. Mortality rates were characterized by significant declines for all cancers combined and for most cancers examined except for melanoma and female lung cancer. The largest improvements in cancer survival were for prostate, liver, colorectal and kidney cancers. While the overall trends in mortality rates and survival point to notable successes in cancer control, the increasing trend in incidence rates for some cancers emphasize the need for continued efforts in prevention.

Abstract In the era of hyper-ageing, Creutzfeldt–Jakob disease (CJD) can become more prevalent as an important cause of dementia. This study aimed to evaluate the trends in crude and age-adjusted CJD-associated mortality and incidence rates in Japan using national vital statistics data on CJD-associated deaths among individuals aged over 50 years, as well as the government-funded nationwide CJD surveillance data (pertaining to the years 2005–2014) in Japan. The data were analysed using the Joinpoint Regression Program to estimate the long-term trends and average annual percentage changes (AAPCs). Overall, the AAPCs of age-adjusted CJD-associated mortality rates rose significantly over the study period (3.2%; 95% confidence interval [CI] 1.4–5.1%). The AAPC of the age-adjusted incidence rates also increased (overall 6.4%; 95% CI 4.7–8.1%). The CJD-associated increases in the mortality and incidence rates were especially prominent among adults over the age of 70 years. Given this trend in aging of population, the disease burden of CJD will continue to increase in severity. Our findings thus recommend that policymakers be aware of the importance of CJD and focus on preparing to address the increasing prevalence of dementia.

Background: Several countries have documented the relationship between long-term exposure to air pollutants and epidemiological indicators of the COVID-19 pandemic, such as incidence and mortality. This study aims to explore the association between air pollutants, such as PM2.5 and PM10, and the incidence and mortality rates of COVID-19 during 2020. Methods: The incidence and mortality rates were estimated using the COVID-19 cases and deaths from the Chilean Ministry of Science, and the population size was obtained from the Chilean Institute of Statistics. A chemistry transport model was used to estimate the annual mean surface concentration of PM2.5 and PM10 in a period before the current pandemic. Negative binomial regressions were used to associate the epidemiological information with pollutant concentrations while considering demographic and social confounders. Results: For each microgram per cubic meter, the incidence rate increased by 1.3% regarding PM2.5 and 0.9% regarding PM10. There was no statistically significant relationship between the COVID-19 mortality rate and PM2.5 or PM10. Conclusions: The adjusted regression models showed that the COVID-19 incidence rate was significantly associated with chronic exposure to PM2.5 and PM10, even after adjusting for other variables.

Background: The present study aims to assess the incidence and mortality rates of gynecological cancers and their changes from 1990 to 2016 at national and subnational levels in Iran. Methods: Annual estimates of incidence and mortality for gynecological cancers from 1990 to 2016 at national and subnational levels were generated as part of a larger project entitled National and Subnational Burden of Diseases, Injuries, and Risk Factors (NASBOD). After the precise processing of data extracted from the Iran Cancer Registry, annual age-standardized incidence and mortality rates were calculated for each cancer, province, year and age group during the period of the study. Results: In 2016, gynecological cancers constituted 8.0% of new cancer cases among women of all ages compared to 3.7% of new cases of cancer among women in 1990. The incidence rate of gynecological cancers has increased from 2.5 (0.9-5.6) per 100000 women in 1990 to 12.3 (9.3–15.7) per 100000 women in 2016, and the most common gynecological cancer has changed from cervical cancer in 1990 to corpus uteri cancer in 2016. Age-standardized incidence rates of ovarian, corpus uteri and vulvovaginal cancers increased from 1.3 (0.5–2.4), 1.7 (0.6–3.0), and 0.3 (0.0–0.7) in 1990 to 4.4 (3.6–5.2), 9.9 (6.8–13.4), and 0.6 (0.2–1.0) in 2016, respectively, showing a 3.3, 5.8 and 1.7-fold increase during this period. Age-standardized incidence rate of cervical cancer was 2.4 (1.7–3.3) cases per 100000 women in 2016 and did not differ significantly from the beginning of the study. An overall reduction was seen in national mortality to incidence ratios (MIR) from 2000 to 2015. Conclusion: The incidence rates of all gynecological cancers in different provinces have shown a converging trend that could indicate that attempts toward health equality have been effective. The declining trend of MIR could be interpreted as advancements in detection of cancer in its early stages and also improvements in treatments, in turn reflecting improvements in access to and quality of care.

Background Follicular lymphoma (FL) is the most common indolent lymphoma and the 2nd most common non- Hodgkin lymphoma, accounting for 10%–20% of all lymphomas in the Western world. Epidemiologic and geographic trends of FL in Canada have not been investigated. Our study’s objective was to analyze incidence and mortality rates and the geographic distribution of FL patients in Canada for 1992–2010.Methods Demographic and geographic patient data for FL cases were obtained using the Canadian Cancer Registry, the Registre quebecois du cancer, and the Canadian Vital Statistics database. Incidence and mortality rates and 95% confidence intervals were calculated per year and per geographic area. Rates were plotted using linear regression models to assess trends over time. Overall data were mapped using Microsoft Excel mapping software (Redmond, WA, U.S.A.) to identify case clusters across Canada.Results Approximately 22,625 patients were diagnosed with FL during 1992–2010. The age-standardized incidence rate of this malignancy in Canada was 38.3 cases per million individuals per year. Geographic analysis demonstrated that a number of Maritime provinces and Manitoba had the highest incidence rates, and that the provinces of Nova Scotia and Quebec had the highest mortality rates in the nation. Regional data demonstrated clustering of FL within cities or regions with high herbicide use, primary mining, and a strong manufacturing presence.Conclusions Our study provides a comprehensive overview of the FL burden and its geographic distribution in Canada. Regional clustering of this disease in concentrated industrial zones strongly suggests that multiple environmental factors might play a crucial role in the development of this lymphoma.

Focusing on West Africa, a region riddled with in situ data scarcity, we evaluate the summer monsoon monthly rainfall characteristics of five global reanalysis datasets: ERA5, ERA-Interim, JRA-55, MERRA2, and NCEP-R2. Their performance in reproducing the West African monsoon (WAM) climatology, interannual variability, and long-term trends for the main monsoon months are compared to gauge-only and satellite products. We further examine their ability to reproduce teleconnections between sea surface temperatures and monsoon rainfall. All reanalyses are able to represent the average rainfall patterns and seasonal cycle; however, regional biases can be marked. ERA5, ERA-Interim, and NCEP-R2 underestimate rainfall over areas of peak rainfall, with ERA5 showing the strongest underestimation, particularly over the Guinea Highlands. The meridional northward extent of the monsoon rainband is well captured by JRA-55 and MERRA2 but is too narrow in ERA-Interim, for which rainfall stays close to the Guinea Coast. Differences in rainband displacement become particularly evident when comparing strong El Niño Southern Oscillation (ENSO) years, where all reanalyses except ERA-Interim reproduce wetter Sahelian conditions for La Niña, while overestimating dry conditions at the coast except for NCEP-R2. Precipitation trends are not coherent across reanalyses and magnitudes are generally overestimated compared to observations, with only JRA-55 and NCEP-R2 displaying the expected positive trend in the Sahel. ERA5 generally outperforms ERA-Interim, highlighting clear improvements over its predecessor. Ultimately, we find the strengths of reanalyses to strongly vary across the region.

SUMMARYDiarrhoea is a leading cause of morbidity and mortality yet diarrhoea specific incidence and mortality rates for older children, adolescents, and adults have not been systematically calculated for many countries. We conducted a systematic literature review to generate regional incidence rates by age and to summarize diarrhoea specific mortality rates for regions of the world with inadequate vital registration data. Diarrhoea morbidity rates range from 29·9 episodes/100 person-years for adults in the South East Asian region to 88·4 episodes/100 person-years in older children in the Eastern Mediterranean region and have remained unchanged in the last 30 years. Diarrhoea mortality rates decline as the child ages and remain relatively constant during adulthood. These data are critical for improving estimates worldwide and further highlight the need for improved diarrhoea specific morbidity and mortality data in these age groups.

Abstract Background: The trends of incidence of solitary bone plasmacytoma (SBP) varied over time due to the changing definitions and the absence of clarity of the criteria. Prior studies have attempted to identify factors such as older age, gender, race as prognostic factors that influence survival of patients with SBP, but with changing paradigm of myeloma treatments, there is limited literature regarding the incidence, mortality and survival trends of SBP. Methods: We used the SEER registry from 1973-2009 to evaluate the incidence, mortality and survival trends in patients with SBP. The results were reported as crude incidence, mortality and survival rates. Two-sample t-tests, ANOVA as well regression analysis were used to examine correlation. Statistics were computed using the National Cancer Institute SEER*Stat software, version 8.2.0. and SAS software, version 9.4 (SAS Institute Inc, Cary, NC). Using the ICD-O-3 and morphologic codes of 9731/3 to identify cases, the final study cohort consisted of 2,734 cases. Trends were evaluated by the eras of diagnosis: 1973-1980, 1981-1985, 1986-1990, 1991-1995, 1996-2000, 2001-2005, and 2006-2009. Age-adjusted incidence rates (IR), standard mortality rates (SMR), survival rate (SR) were expressed as new cases per 100,000 person-years, and age-adjusted to the 2000 US standard population. Results: The median age of diagnosis of SBP among blacks is 61 years (range, 21-91) compared to others: 60 years (range, 28-88) and whites: 66 years (20-97). The age adjusted incidence rates for black males is: 0.3 (95%CI 0.2, 0.3) followed by black females 0.2 (95%CI 0.1, 0.2) white males 0.2 (95%CI 0.2, 0.2) white females 0.1 (95%CI 0.1, 0.1). The trends in incidence and mortality rates are illustrated in table 1 with highest IR noted for black males during the era 2006-2009. The 5-year survival rates for both males (figure 1) and females (figure 2) seem to be trending down over the eras examined. Regression analysis suggests males and other race have increased odds of survival (HR = 0.829, p=0.0078; HR = 0.54 and p=0.0038, respectively). Conclusions: Similar to myeloma, black patients tend to be diagnosed with SBP younger and have increased incidence. The incidence rates seem to be increasing, highest among blacks males, more likely from increased awareness and diagnosis. The mortality and survival patterns are comparable to whites. Interestingly, while the 5-year survival for myeloma among all racial groups is improving this analysis shows a decreasing trend for SBP. This observation is more likely from including myeloma patients under the diagnosis of SBP over the period of study. Recently, the International Myeloma Working Group (IMWG) clarified the definition of SBP which will help in accurate diagnosis and ultimately can help in accurate representation of the survival trends. The purpose of this article is to examine the convergence in CO2 emissions across 18 Asian countries over the period of 1972–2010, based on data obtained from World Development Indicators. The study used β and σ convergence for parametric tests and kernel density estimates for non-parametric tests of convergence hypothesis and found convergence in CO2 emission in all the tests. More recent trends have been considered for the tests rather than the long-term trends in CO2 convergence. During the initial period (i.e., 1972–1982 and 1982–1992), the values, although converging, are not significantly depending upon the previous period. In contrast, the results for recent decades show convergence in CO2 emissions significantly. Sigma (σ) convergence estimates also indicates that variation declined more in recent decades. Thus, the empirical findings of the study fulfil the major tests of convergence and provide evidence that many Asian countries’ relative per capita CO2 emissions are converging over time. The study suggests that these results should be looked at in multilateral negotiation agreements.

Background: Unintentional falls seriously threaten the life and health of people in China. This study aimed to assess the long-term trends of mortality from unintentional falls in China and to examine the age-, period-, and cohort-specific effects behind them.Methods: This population-based multiyear cross-sectional study of Chinese people aged 0–84 years was a secondary analysis of the mortality data of fall injuries from 1990 to 2019, derived from the Global Burden of Disease Study 2019. Age-standardized mortality rates of unintentional falls by year, sex, and age group were used as the main outcomes and were analyzed within the age-period-cohort framework.Results: Although the crude mortality rates of unintentional falls for men and women showed a significant upward trend, the age-standardized mortality rates for both sexes only increased slightly. The net drift of unintentional fall mortality was 0.13% (95% CI, −0.04 to 0.3%) per year for men and −0.71% (95% CI, −0.96 to −0.46%) per year for women. The local drift values for both sexes increased with age group. Significant age, cohort, and period effects were found behind the mortality trends of the unintentional falls for both sexes in China.Conclusions: Unintentional falls are still a major public health problem that disproportionately threatens the lives of men and women in China. Efforts should be put in place urgently to prevent the growing number of fall-related mortality for men over 40 years old and women over 70 years old. Gains observed in the recent period, relative risks (RRs), and cohort RRs may be related to improved healthcare and better education.
