Aim. To identify the factors of internal and nutrition-related diseases in females of reproductive age. Methods. A comprehensive general physical examination of 383 pregnant women aged 16 to 46 years old with the following internal diseases at pregnancy: gestational arterial hypertension (n=183), essential arterial hypertension (n=66), obesity (n=134) was conducted. Control group consisted of 153 pregnant women without any signs of internal diseases. Results. Combination of social and demographic, psychosomatic and inherited factors may define the risk of neurohormonal mechanisms deregulation, obesity, and type of arterial hypertension. Females with internal diseases had significantly lower level of physical activity compared to females of the control group. The largest number of long-term and heavy smokers was revealed in groups of female patients with gestational and essential arterial hypertension, patients with obesity and mature nulliparous women. Multivariate analysis showed that income level and physical activity of pregnant women have a significant impact on excessive fat body mass in pregnant women during gestation and before it. No statistically significant relationships between educational level and body mass index before pregnancy and at gestation were found. Conclusion. Factors influencing the risk for internal and nutrition-related diseases in females of reproductive age were: (1) the increased demand for nutrients and energy in developing placenta-fetus system, leading to irrational nourishment; (2) decreased body reserves, caused by repeated pregnancies and childbirth; (3) smoking; (4) social and economic status; (5) living area; (6) hereditary factors and constitutional features.

Abstrak   Latar belakang:  Status  gizi  perempuan  di  Indonesia  cenderung  memburuk.  Hasil  Riset  Kesehatan  Dasar (Riskesdas) menunjukkan bahwa kekurangan energi kronis (KEK) pada wanita usia reproduksi meningkat dari 13,6 persen pada 2007 menjadi 20,8 persen pada 2013. Ibu hamil yang kekurangan gizi akan berdampak buruk pada bayinya. Tujuan: Memperoleh determinan status gizi kurang yaitu KEK dan Indeks Massa Tubuh (IMT) rendah dari wanita hamil di Indonesia pada tahun 2013. Metode: Penelitian menggunakan data Riskesdas 2013. Analisis dilakukan dengan multivariat regresi logistik. Jumlah sampel adalah 7236 ibu hamil. Ibu hamil dikatakan KEK jika pertengahan lingkar lengan atas (LILA) < 23,5 cm dan IMT diukur dengan pendekatan metode Broca. Hasil: Determinan ibu hamil KEK dan IMT rendah adalah tinggal di perdesaan (AOR 1,20; 95% CI [1,11-1,13]), usia <20 tahun (AOR 1,62; 95% CI [1,60-1,65]), paritas 1 anak (AOR 2,04; 95% CI [2,02-2,06]), berpendidikan rendah (AOR 1,92; 95% CI [0,91-0,93]) dan status ekonomi rendah (AOR 3,36; 95% CI [3,31- 3,41]). Kesimpulan: Pendidikan harus ditingkatkan minimal hingga sekolah menengah atas sehingga pengetahuan gizi ibu meningkat dan kehamilan dini dapat dicegah. Ibu dengan status ekonomi rendah harus menjadi prioritas dalam pelayanan ibu dan upaya intervensi gizi.   Kata kunci: Sosial ekonomi, lingkar lengan atas, KEK, BMI, gizi ibu hamil.     Abstract   Background: The nutritional status of women in Indonesia tends to worsen. Basic Health Research (Riskesdas) showed that chronic energy deficiency (CED) among women of reproductive age increased from 13.6 percent in 2007 to 20.8 percent in 2013. Pregnant women who are lack of nutrition will have an impact on their babies. Objective: To determine the determinants of poor nutritional status which are CED and low body mass index (BMI) of pregnant women in Indonesia. Method: This study used 2013 Riskesdas data. The analysis was done using multivariate logistic regression. The total  sample  was 7236  pregnant  women.  Pregnant  women  with CED are those who  have mid-upper  arm circumference (MUAC) of less than 23.5 cm and BMI was measured by the Broca method approach. Results: The determinants of pregnant women with CED and low BMI are living in rural area (AOR 1.20; 95% CI [1.11-1.13]), age of <20 years (AOR 1.62; 95% CI [1.60-1.65]), having 1 child (AOR 2.04; 95% CI [2.02- 2.06]), having low education (AOR 1.92; 95% CI [0.91-0.93]) and low economic status (AOR 3.36; 95% CI [3.31- 3.41]). Conclusion: Education must be improved to at least high school degree, so that the mother's nutritional knowledge will increase, and early pregnancy will be prevented. Abstract Background Asymptomatic hyperuricemia (AHU) is defined as elevated serum uric acid (UA) concentration without symptoms. This study aimed to determine the effects of AHU treatment with allopurinol on selected hypertension mediated organ damage (HMOD) indices in patients with uncomplicated essential arterial hypertension (AH). MethodsPatients aged 30-70 years with AHU and essential hypertension grade 1-2 with adequate blood pressure (BP) control, without previous urate lowering therapy (ULT) were divided into two groups: a) receiving allopurinol (ULT group) and b) age-and sex matched patients without ULT (control group). Both groups received UA-lowering diet. BP (office, 24 hour and central), echocardiographic parameters, pulse-wave velocity, carotid intima-media thickness (IMT) and lab tests (high-sensitivity C-reactive protein (hs-CRP) were measured at baseline and at 6 months follow-up.ResultsOut of 100 participants 87 completed the study (44 ULT patients and 43 controls). At 6 months follow-up, there was a significantly greater reduction in serum UA concentration in the ULT group than in the control group (464±68.8 µmol/l vs 314±55.6 µmol/l, p<0.0001). Patients receiving allopurinol had significant reductions in office systolic (137±11.8 mmHg vs 134±9.3 mmHg; p=0.025) and diastolic BP (83±9.9 mmHg vs 79±8.7 mmHg, p=0.017), central systolic BP (56±8.9 mmHg vs 51±12.9 mmHg, p=0.046), pulse pressure (43±10.4 mmHg vs 39±11.2 mmHg, p=0.017), IMT (0.773±0.121 mm vs 0.752±0.13 mm, p=0.044), left atrium volume index (40±13.5 ml/m2 vs 38±12.3 ml/m2, p=0.044), and hs-CRP level (3.36±2.73 mg/l vs 2.74±1.91 mg/l, p=0.028) compared to controls. The decrease in UA concentration was significantly related to the reduction in IMT (R=0.37, p<0.001), central SBP (R=0.26, p=0.015) and hs-CRP concentration (R=0.30, p=0.004). Multivariate regression analysis revealed the independent relationship between reduction in IMT and UA lowering (R=0.3234, R2=0.0722, p<0.026).ConclusionsIn patients with AH and asymptomatic hyperuricemia treatment with allopurinol leads to further improvement in BP control and reduction in HMOD intensity, in particular IMT. The decrease in hs-CRP concentration associated with ULT may have a beneficial effect on patient long-term prognosis.

The aim – to study the features of structural and functional remodeling of the left ventricle in patients suffering from arterial hypertension with concomitant hypothyroidism and to determine clinical and past medical history and laboratory factors associated with their development. Materials and methods. 50 patients suffering from hypertension with concomitant hypothyroidism were enrolled into the study. Depending on the level of thyroid stimulating hormone in the serum the examined patients were distributed into two groups of 25 patients with the compensated and 25 with decompensated course of hypothyroidism. The control group consisted of 30 patients with hypertension in which the pathology of the thyroid gland was excluded. The comparison of the main parameters of the echocardiography study of the myocardium has been performed depending on hypothyroidism compensation. Assessment of parameters of transmitral diastolic blood flow has been performed. A regression analysis has been conducted to detect the relation of clinical and past medical history factors and echocardiographic parameters with the development of diastolic dysfunction by E/A ratio. The values of central hemodynamics have been studied for evaluation of the contractile function of the myocardium. Results and discussion. According to the results of the echocardiography of both examined groups it has been found that the final systolic and stroke volume exceeded the parameters of the control group. The mean value of the left ventricle ejection fraction was statistically significantly lower than in the control group (p=0.004). The left ventricle myocardial mass in patients with hypothyroidism was statistically significantly greater than in the control group. It was proved that in patients, who were diagnosed with decompensated hypothyroidism, the mean value of the left atrium size to growth by the degree of 2.7 was statistically significantly higher than in the control group (p=0.01), whereas the average value of the ratio of the size of the left atrium to the surface area of the body had no statistically significant differences between the groups. It has been found that in both groups the proportion of patients with diastolic dysfunction in which the E/A ratio was less than 1.0 was higher than in the control group (р=0.01 and p=0.03, respectively). The independent factors of diastolic dysfunction of the left ventricle in patients with hypertension in the presence of hypothyroidism have been found. Conclusions. In the presence of decompensated hypothyroidism in patients with arterial hypertension, both with obesity and with normal body mass index, left ventricular mass indexes were significantly higher in comparison with a control group. Regardless of the compensation of the thyroid state in patients with arterial hypertension, the ejection fraction was significantly lower. According to regression analysis, independent factors for the development of diastolic dysfunction in patients with arterial hypertension and hypothyroidism with a body mass index ≥ 30 kg/m2 is the index of mass of the left ventricular myocardium, determined by the degree of 2.7, the level of total cholesterol to statistical significance – the level of office systolic blood pressure and the duration of hormone replacement therapy for hypothyroidism; with body mass index < 30 kg/m2 – age and left atrial index, determined by body surface area. For patients with arterial hypertension and reduced thyroid gland function, violation of the left ventricular myocardial relaxation is typical as evidenced by a higher proportion of patients with a decrease in E/A to less than 0.8 in these patients.

Abtrak. Anemia merupakan masalah kesehatan masyarakat secara global baik dinegara berkembang maupun negara maju. Anemia terjadi pada semua tahap siklus kehidupan, umumnya terjadi pada masa anak-anak dan wanita hamil (WHO, 2008). Di puskesmas Pegandon Kabupaten Kendal pada bulan Agustus 2017 didapatkan ibu hamil dengan kadar HB 8,2 gr% mengalami perdarahan pada saat melahirkan. Penelitian ini bertujuan mengevaluasi pemberian tablet FEpada ibu nifas.Desain penelitian menggunakanmatching pretest-post test control group design. Penelitian ini menggunakan data kohort dari ibu hamil  di puskesmas Pegandon yang melahirkan pada bulan Desember 2017 – Januari 2018. Sampelnya ibu nifas pada bulan  Januari – Maret 2018yang datang di puskesmas Pegandon untuk melahirkan sebanyak 72 ibu nifas. Hasil penelitian menunjukkan sebagian besar  pada responden perlakuan mempunyai usia reproduksi 30 (83,3%),  sebagian berpendidikan (SD danSMP) 28 (77.8%) dan berpenghasilan sesuai UMK 22 (61.1%) sedangkan pada responden control untuk usia reproduksi25 (69%), berpendidikan (SD dan SMP) 31 (86%) dan berpenghasilan sesuai UMK 15 (41.7%).Pada analisis bivariate dengan uji kolmogorov-smirnov didapatkan p> 0.00 pada perlakuan dan kontrol. Disimpulkan bahwa diberikan dan tidak diberikan tablet FE pada ibu nifas didapatkan hasil sama berpengaruh dalam peningkatan kadar HB.Disarankansetiap ibu nifas meminum tablet FE supaya tidak terjadi anemia dan memberikan ASI pada bayinya dengan maksimal.Kata kunci : anemia, ibu nifas, tablet FE The Influence Of Iron Tablets In Pifandon Mother To Post Partum Anemia In Pegandon Community Health Center Area Abtract.Anemia is a global public health problem both in developing and developed countries. Anemia occurs at all stages of the life cycle, generally occurring during childhood and pregnant women (WHO, 2008). In Pegandon Puskesmas Kendal Regency in August 2017 it was found that pregnant women with HB levels 8.2 gr% had bleeding at the time of delivery. This study aims to evaluate the administration of FE tablets to postpartum mothers. The study design used a matching pretest-post test control group design. This study uses cohort data from pregnant women at the Pegandon puskesmas who gave birth in December 2017 - January 2018. The sample was postpartum mothers in January - March 2018 who came to the Pegandon puskesmas to deliver 72 postpartum mothers. The results showed that most of the treatment respondents had a reproductive age of 30 (83.3%), some were educated (SD and SMP) 28 (77.8%) and earning according to MSE was 22 (61.1%) while the control respondents were for reproductive age 25 (69% ), educated (elementary and junior high) 31 (86%) and earning according to UMK 15 (41.7%). In the bivariate analysis with the Kolmogorov-Smirnov test, p> 0.00 was obtained for treatment and control. It was concluded that given and not given FE tablets to the puerperal mothers found the same effect had an effect on increasing HB levels. It is recommended that every puerperal woman take FE tablets so that anemia does not occur and give milk to her baby to the maximum..Keywords: anemia, puerperal mothers, FE tablets

Introduction: Bone formation marker osteocalcin (OC) and bone resorption marker C-terminal telopeptide of type 1 collagen (CTX) can be used to detect or to monitor the early responses of the skeleton to physical activity. Literature suggests that it is likely that higher body mass index (BMI) has positive effect on bones and can postpone onset of osteoporosis.Aim of the Study: The aim of this study is to:1.Determine the effect of aerobic physical acitivity on OC and CTX in young women2.Investigate correlation of OC, CTX and BMI in young women engaged into structured aerobic excerciseMaterial and methods: Study included 64 healthy young women, aged 19 to 25 years, devided into two groups: intervention group (n=32) and control group (n=32). The study duration was six weeks with follow-up period of four weeks. The intervention group underwent structured aerobic physical activity program for six weeks, but the control group did not receive such program. Level of OC, CTX and BMI were measured at baseline, after 6-week aerobic program, and after 4-week follow up (only intervention group).Results: There was significant increase of OC level after 6-week aerobic program, while level of CTX did not changed. OC level was at its maximum immediately after finishing the program in the participants with normal BMI (p<0.001). There was no statistically significant interaction of BMI and CTX level.Conclusion: Aerobic physical activity increases level of osteocalcin, suggesting that it has positive influence on bone formation in young women, primarily in women with normal BMI.

Abstract BackgroundPhysical activity during pregnancy period is one of the issues with priority during pregnancy period. Researches show that women reduce their physical activity during this period and are unaware of the benefits on the health of the mother and embryology. Although researchers regarding physical activity during pregnancy have prepared many guidelines; it is not however clear why pregnant women do not perform physical activity and the effectiveness factors that facilitate the desired behavior. MethodsThe research population included all pregnant women aged 18 to 40 years with gestational age of 12-38 weeks referring to the healthcare centers in District 5 of Tehran Municipality, from 22 districts of the city who had eliminated their physical activity during pregnancy. Questionnaires of physical activity assessment questionnaire after educational intervention in pregnant women (PPAQ) and a questionnaire designed based on the results of the needs assessment and the dimensions of the PEN-3 model were used for assessment. This study is a Research Clinical Trial (RCT). Due to the nature, a quasi-experimental research design (pre-test, post-test) with the control group was used in this research. ResultsBased on the obtained results, the intervention based on E-learning has a significant effect of enabling factors to increase physical activity in pregnant women in the experimental group. In addition, the intervention based on E-learning has a significant effect on the training factors in order to increase physical activity in pregnant women in the experimental group. ConclusionBased on the research results, it can be said that there is a statistically significant difference between the experimental and control groups in the scores obtained from training factors in the pretest and the experimental group. Furthermore, it can be said that there is a statistically significant difference between the experimental and control groups in the scores obtained from the enabling factors in the pre-test and the experimental group.

The aim of the study was to assess the effects that a steps/day programme may have on body mass index (BMI) among primary education students (11.37±0.48 years). A six-week controlled trial with a follow-up was completed with an experimental group (N=66, pedometer + steps/day programme + reinforcement programme in their physical education (PE) marks + weekly follow-up in PE), and a control group (N=76). Omron HJ-152-E2 pedometers were used. Normoweight students complied with programme requirements to a greater extent than their overweight counterparts (72.7 vs 59.1%). The programmed minimum number of 12,000 steps/day for boys and 10,000 for girls was exceeded by 83% of boys and 60% of girls. The differences in the number of steps/day between boys (14,274) and girls (10,626) were significant across all the measured periods (p&lt;.05). The results show that the teenagers who complied with the programme requirements reduced their BMI significantly after the intervention (p&lt;.001) and this reduction persisted for six weeks after the programme (p&lt;.001). In conclusion, the six-week programme of 12,000 steps/day for boys and 10,000 for girls, jointly with a reinforcement programme in their PE marks and weekly follow-up by their PE teacher, reduces BMI significantly in 11-12-year-old schoolchildren. Monitored steps/day programmes in the PE curriculum increase out-of-school physical activity and reduce BMI in 11-12-year-old schoolchildren.

This study shows the feasibility of an eHealth solution for tackling eating habits and physical activity in the adolescent population. The participants were children from 11 to 15 years old. An intervention was carried out on 139 students in the intervention group and 91 students in the control group, in two schools during 14 weeks. The intervention group had access to the web through a user account and a password. They were able to create friendship relationships, post comments, give likes and interact with other users, as well as receive notifications and information about nutrition and physical activity on a daily basis and get (virtual) rewards for improving their habits. The control group did not have access to any of these features. The homogeneity of the samples in terms of gender, age, body mass index and initial health-related habits was demonstrated. Pre- and post-measurements were collected through self-reports on the application website. After applying multivariate analysis of variance, a significant alteration in the age-adjusted body mass index percentile was observed in the intervention group versus the control group, as well as in the PAQ-A score and the KIDMED score. It can be concluded that eHealth interventions can help to obtain healthy habits. More research is needed to examine the effectiveness in achieving adherence to these new habits.

Study was carried out in the department of physiology, Mymensingh Medical College, Mymensingh, Bangladesh during the period of July 2006 to June 2007 to investigate the effect of pregnancy on serum total cholesterol. The serum concentrations of total cholesterol was measured in 100 cases of 2nd and 3rd trimester of pregnancy and in a control group of 100 cases of non pregnant women which was matched on reproductive age. Data were analyzed by computer with SPSS program using unpaired student‘t’ test. The results showed that the pregnant women had significantly higher concentrations of serum total cholesterol. Higher concentration of total cholesterol was more common in pregnant than control and reaching maximum at 3rd trimester of pregnancy. This may be a purely physiological response to pregnancy or it may be indicative of pathology in some women. These results deserve a follow up study to investigative whether the hypercholesterolemia persists after parturition.
