Introduction: A portable fire extinguisher is one of the effective and active fire protection tool equipment used to extinguish fires at the beginning of a fire. The occurrence of a fire does not immediately become a big fire but it is starts from a small fire. This small fire is perfectly extinguished by using a portable fire extinguisher. Installation and maintenance of a portable fire extinguisher is one of the systems for fire prevention. However, it is important to know that a good and effective implementation must be balanced with proper installation and maintenance in accordance with existing regulations so that later it can function properly. This study aims to evaluate the application of the installation and maintenance of portable fire extinguishers at PT Y Indonesia. Methods: This research used descriptive research method with cross sectional approach. The data were obtained by observation using observation sheets and measurements directly in the field using a meter. Results: PT Y Indonesia has a policy regarding the installation and maintenance of APAR but its application has not fully met the requirements except for the color, pressure and distance between APAR. Conclusion: The implementation of APAR installation at PT Y Indonesia has not been in accordance with the relevant regulations except for APAR color, and the distance between APAR and implementation of maintenance is only done once a month without conducting detailed inspection with a period of 6 months and 12 months. Keywords: installation, maintenance, portable fire extinguisher

Introduction: Plastic sack industry is an industry that has medium (3) level of fire potential, caused by the material used in the production process is flammable and the industry itself uses electricity to generate heat. In this situation, a protection system is needed in the form of fire extinguisher. By seeing the fire potential in the plastic sack industry, this study aimed to evaluate the installation of fire extinguisher as an effort to prevent and control fire. Method: This study was an observational study through cross-sectional research design. The research samples were 45 fire extinguishers and the variables of the research were fire extinguisher installation process. Result: The classification of fire in the plastic sack industry was A, B, and C. The industry had 45 fire extinguishers spread through the whole production area and office area. It was found that there were three types of fire extinguisher with different capacity on every type, condition, label, height and the sign of installation that still did not comply to the regulation. Conclusion: Fire extinguisher installation in plastic sack industry does not follow the regulations such as procedures of installation and maintenance of fire extinguisher, team of fire prevention, color, the high of installation, condition and placement, label on tube, and marker of fire extinguisher, so it needs to take corrective actions to improve the performance of fire extinguisher when it is used. Keywords:  fire extinguisher, fire extinguisher installation, flammable material, plastic sack, protection system

Data collected by questionnaire, checklist, and observation. Result: The result showed that the active protective means for fire extinguisher in good category with a level of suitability of 92,9% and for hydrant in sufficient condition with a level of suitability of 73,5%. Conclusion:. There were some elements that were not suitable, such as some fire extinguisher covered by items, some fire extinguisher installations exceed 120 cm from the floor surface, some fire extinguisher were defective due to rusting, there were no hydrant use instructions, and no fire hydrant have been examined. PT. Dok dan Perkapalan Surabaya (Persero) is suggested to move items that cover fire extinguisher to be easily seen, the height of the fire extinguisher installation should not exceed 120 cm from Permenakertrans No. Per 04/MEN/1980, replacing inappropriate fire extinguisher, carrying out rountine fire check.

Years ago, a fire extinguisher was invented as an active fire protection device used to extinguish or control fires in emergencies. On board, fire extinguisher plays an important role to prevent and overcome any danger or threat resulted from a fire. Thus, crew’s capability in using the fire extinguishers is highly needed. In this case, crews are supposed to be trained in using the fire extinguisher by implementing a fire drill (at least once a month). However, in fact, in this research, it was found that most of MV. Vinca’s crews were not aware of fire-fighting procedure especially on how to use the portable fire extinguisher. This phenomenon was the reason why this research was conducted focusing on the crew’s low capability in using portable fire extinguishers during the fire drill implementation. The data were collected through observation, structured interviews, documentary studies, and library research. The data were then analyzed by using Fault Tree Analysis (FTA) and Urgency, Seriousness, Growth (USG). Based on the analysis, it was found that the crews had a low capability in using a portable fire extinguisher. There were some reasons: (1) They lacked the knowledge and understanding about portable fire extinguisher; (2) The fire drills were not seriously carried out; (3) The portable fire extinguishers were in bad condition due to the absence of periodic and regular maintenance. In order to overcome the problem, training and guidance for the crews about the various types of portable fire extinguisher and their uses are highly needed; fire drills need to be performed seriously. Moreover, regular maintenance to the fire-fighting equipment also needs to be carried out.
