Rifampicin is an anti-tuberculosis drug which has an efficient antimicrobial effect and the basis of a short-term treatment regimen for tuberculosis (TB) patients. Rifampicin plays an important role against the growth and slow metabolism of Bacilli M. tuberculosis. Resistance to rifampicin causes the duration of tuberculosis treatment to be longer. Interleukin-18 (IL-18) is a pro-inflammatory cytokine which plays a role in controlling the growth of M. tuberculosis through its ability to induce IFN-γ, while Interleukin-10 (IL-10) is an anti-inflammatory cytokine which plays a role in limiting tissue damage due to the inflammatory process and maintain tissue homeostasis. IL-18 and IL-10 has an important role in explaining the different degrees of inflammation in rifampicin resistant (RR) and rifampicin sensitive (RS) pulmonary tuberculosis patients. The purpose of this study is to determine different levels of IL-18 and IL-10 in new TB patients with RR and RS. This study was a retrospective cohort study with a cross-sectional design carried out from August-November 2018 in the TB-DOTS/MDR clinic at Dr. Soetomo Hospital, Surabaya. 50 research subjects were examined and grouped into two groups, namely pulmonary TB with RR (n = 25) and pulmonary TB with RS (n = 25) based on GeneXpert examination and anti-tuberculosis drug therapy ≤ 1 month. IL-18 and IL-10 were measured using the ELISA Method. Differences in IL-18 and IL-10 levels between groups were analyzed using the Mann-Whitney test. The mean level of IL-18 (pg/ml) in RR and RS pulmonary TB patients were 1273.53±749.86 and 787.96 ±589.28 respectively. The mean level of IL-10 (pg/ml) in RR and RS pulmonary TB patients were 125.25±118.32 and 128.81±135.77 repectively. The mean level of IL-18 in RR and RS pulmonary TB patients were found to have a significant difference, while the mean level of IL-10 did not have a significant difference. Keywords: Interleukin-18, Interleukin-10, Tuberculosis, Rifampicin Resistant, Rifampicin Sensitive

This study was carried out to study of some immunological aspects among the pulmonary Tuberculosis patients infected with causative agent, Mycobacterium tuberculosis. A Total of 200 sputum samples were collected from patients attending the consultant Clinic for Chest and Respiratory disease center, Diwaniya. Control group (No=15) also included. According to acid fast stain of sputum, the patients were classified as positive (No=91,45.5%) and negative (No=109,54.5, Lowenstein Jensen medium used for the cultivation of samples, on which 70% of sputum samples where positive culture for this microorganism. The grown microorganism were identified as M. tuberculosis, based on positive A.F.B, Niacin producers ,negative for catlase at 68c. The mean IgG level was l184.053±76.684 mg/100 ml in tuberculosis group compared with 1016.533 ± 44.882 mg/100ml in control group, rendering the statistical difference significant. For IgA and IgM levels, they were at mean of 315.880±38.552 mg/100 ml and 119.527±8.464 mg/100 ml in control group compared with 396.358±38.776 mg/100 ml and 134.207±11.696 mg/100 ml in patients group respectively with significant difference

Background Tuberculosis remains a global threat and a public health problem that has eluded attempts to eradicate it. Low vitamin D levels have been identified as a risk factor for tuberculosis infection and disease. The human cathelicidin LL-37 has both antimicrobial and immunomodulatory properties and is dependent on vitamin D status. This systematic review attempts to compare vitamin D andLL-37 levels among adult pulmonary tuberculosis patients to non-pulmonary TB individuals between 16–75 years globally and to determine the association between vitamin D and cathelicidin and any contributing factor among the two study groups. Methods/Design We performed a search, through PubMed, HINARI, Google Scholar, EBSCOhost, and databases. A narrative synthesis through evaluation of vitamin D and LL-37 levels, the association of vitamin D and LL-37, and other variables in individual primary studies were performed. A random-effect model was performed and weighted means were pooled at a 95% confidence interval. This protocol is registered under the International Prospective Register of Systematic Reviews (PROSPERO), registration number CRD42019127232. Results Of the 2507 articles selected12 studies were eligible for the systematic review and of these only nine were included in the meta-analysis for vitamin D levels and six for LL-37 levels. Eight studies were performed in Asia, three in Europe, and only one study in Africa. The mean age of the participants was 37.3±9.9 yrs. We found low vitamin D and high cathelicidin levels among the tuberculosis patients compared to non-tuberculosis individuals to non-tuberculosis. A significant difference was observed in both vitamin D and LL-37 levels among tuberculosis patients and non-tuberculosis individuals (p = < 0.001). Conclusion This study demonstrated that active pulmonary tuberculosis disease is associated with hypovitaminosis D and elevated circulatory cathelicidin levels with low local LL-37 expression. This confirms that vitamin D status has a protective role against tuberculosis disease.

Background: Tuberculosis is an infectious disease caused by Mycobacterium tuberculosis, which has posed a constant challenge to mankind in its treatment due to increasing resistance and longer duration of treatment. The newer approach is to look towards strengthening host immune system along with suppressing the organism. Aim of the study was to assess the existence of Vitamin D deficiency in TB patients and aid in the strategies and development of newer improvised approaches in the treatment of TB. Objectives of the study was to estimate vitamin D levels in Tuberculosis (pulmonary and extrapulmonary) patients, assess Correlation between vitamin D and   pulmonary tuberculosis and to assess correlation between Vitamin D and extrapulmonary Tuberculosis.Methods: This is a descriptive cross-sectional study. The study consisted of 80 tuberculosis patients both extrapulmonary and pulmonary. Blood samples was analysed for Vitamin D levels and results were compared with age and sex matched controls. Results was analysed using SPSS software.Results: The cases included patient in the age group of 18-60 year with the mean age being 42.34±14.65 year. Of the 80 tuberculosis patients 42 were diagnosed with pulmonary tuberculosis and 38 constituted extrapulmonary tuberculosis. The mean Vitamin D in cases was 24.82±12.33 and controls was 34.41±6.19. Among the cases 25 (31.3%) subjects had Vitamin D levels <20 pg/ml and none of the controls had levels <20 pg/ml. The mean Vitamin D level in pulmonary Tb patients was found to be 24.29±11.86 pg/ml and Extra-pulmonary Tb was 25.40±12.96 pg/ml. The unpaired t-test was statistically significant with p value of 0.005.Conclusions: This study has emphasized on the presence of nutritional deficiency in TB patients and necessity to correct them to achieve a better cure rate.

Pulmonary tuberculosis (TB) is a chronic disease that can bring about the sufferer's self-stigma and also affect his quality of life. A number of studies report that living with TB has a negative influence on the quality of life of sufferers even with or without self-stigma. The purpose of this study was to identify the quality of life of TB patients who experienced self-stigma. This research is a descriptive study, sample were 31 pulmonary TB patients. Data was collected using a questionnaire. Data analyzed by using frequency distribution and percentage. The researcher first screened TB patients who experienced self-stigma. The results showed that 25 people (80.64%) respondents experienced mild self-stigma. A total of 9 respondents (36%) had a quality of life score in the good category and as many as 16 respondents (64%) had enough category with an average quality of life score is 56.57. While respondents who had moderate self-stigma were 6 people (19.36%) with a good quality of life score was 1 person (16.67%) and enough category quality of life score were 5 people (83.33%) with an average quality of life score is 49.92.

Endometrioma on the ovaries is a benign gynecological disorder that is often found in women of reproductive age. The approach was made to the malignant transformation through the study of cyclin-D1 expression. This research aim to analyze differences in the level of Cyclin-D1 expression in ovarian endometriosis and ovarian carcinoma associated with the pathogenesis of endometrioma and ovarian carcinoma. Analytical observational study with cross sectional approach to cyclin-D1 expression between endometrioma and ovarian carcinoma with good and bad differentiation. The research subjects were 20 cases of endometrioma, each of the 20 cases of ovarian carcinoma were well and poorly differentiated. Statistical analysis using the ANOVA test on the level of cyclin-D1 expression between groups. The mean cyclin-D1 expression in endometrioma was 67.25. The mean of well-differentiated ovarian carcinoma was 132.41. The mean of poorly differentiated ovarian carcinoma was 128.83. Anova test resulted in a significant difference between the expression of cyclin-D1 endometrioma and ovarian carcinoma with good and bad differences (p = 0.00 0.05). There is a significant difference between endometrioma and ovarian carcinoma with good and bad differentiation. Endometrioma cyclin-D1 expression was lower than ovarian carcinoma with good and bad differentiation.

BACKGROUND: Immunological mechanisms of infertility are still poorly understood and controversial, both the cause and treatment. Inflammation, immunology, cell proliferation, cell differentiation, and cell survival are influenced by several proteins, including nuclear factor kappa-B (NFĸB), tumor necrosis factor-α (TNF-α), and interleukin-10 (IL-10). AIM: This study aimed to explore the potential of nano curcumin to prevent anti-sperm antibodies (ASA) formation due to the testes’ inflammatory process in Wistar rats. METHODS: This research is an experimental study with a pre-post-test approach with control group. The research subjects were rats (Rattus norvegicus) of the Wistar strain. The induced animals were grouped into three groups: Group 1 received nano curcumin 1 × 80 mg/kg BW orally, Group 2 received dexamethasone 1 × 0.3 mg/kg BW, and Group 3 received placebo aquadest 1 × 1 mL orally. TNF-α, NF-kB, and IL10 levels in serum were examined with enzyme-linked immunosorbent assay. RESULTS: The nano curcumin treatment showed the ability to reduce the pro-inflammatory cytokine protein TNF-α expression (47.3 ± 2.32) more optimally than dexamethasone treatment (54.4 ± 3.22). Nano curcumin has also shown the ability to reduce the pro-inflammatory cytokine transcription factor, NF-kB (32.5 ± 2.76) more optimally than treatment with dexamethasone (44.6 ± 2.43). CONCLUSION: Nano curcumin can prevent the formation of ASA in testicular trauma through inhibition of the inflammatory response.

ABSTRACTBackground: Erythrocytes Sedimentation Rate (ESR) is a test to determine the rate of erythrocytes settling blood containing anticoagulants in a vertical tube within a certain time. ESR is generally used to check and monitor for tissue damage, inflammation and indicate disease. ESR examination can be done by manual and automatic methods. LED levels in pulmonary TB patients generally have increased.Objectives: Knowing the difference in the measurement results of the erythrocytes sedimentation rate (ESR) between the Westergen manual and automatic methods in patients with pulmonary tuberculosis.Methods: Laboratory experiments with a post-test-only approach. The number of samples was 30 people and was taken by purposive sampling. The research data were obtained from the results of the ESR examination using the Westergren manual and automatic methods. Results: Examination of the sedimentation rate of healthy respondents used the Westergren method had an average of 9,40 mm/hour while in the automatic method 10,15 mm/hour. The examination of sedimentation rate of pulmonary tuberculosis respondents with the Westergren method had an average of 66,13mm/hour, while the Automatic method was 67,80 mm/hour. There was no significant difference in the mean value of ESR between the measurement method used manual Westergren and Automatic methods in patients with pulmonary tuberculosis ( p = 0,878).   Conclusion: There is no significant difference in the mean value of ESR between measurements using the manual Westergren method and automatic in patients with pulmonary tuberculosis. Keywords: Erythrocytes Sedimentation Rate, Method, Westergren, Automatic  PERBANDINGAN NILAI LAJU ENDAP DARAH ANTARA PENGUKURAN METODE MANUAL WESTERGREN DAN ALAT AUTOMATIK PADA SAMPEL DARAH SITRAT PENDERITA TB PARU DI RSUD. Dr. DRADJAT PRAWIRANEGARA SERANG TAHUN 2020 Latar Belakang: Laju Endap Darah (LED) adalah pemeriksaan untuk menentukan kecepatan eritrosit mengendap dalam darah yang berisi antikoagulan pada suatu tabung vertikal dalam waktu tertentu. LED pada umumnya digunakan untuk mendeteksi dan memantau adanya kerusakan jaringan, inflamasi dan menunjukan adanya penyakit. Pemeriksaan LED dapat dilakukan dengan metode manual dan automatik. Kadar LED pada penderita TB Paru umumnya mengalami peningkatan.Tujuan: Mengetahui perbedaan hasil pengukuran Laju Endap Darah (LED) antara metode manual Westergen dan Automatik pada Penderita TB Paru. Metodologi: Eksperimen laboratorik dengan pendekatan post test only. Jumlah sampel 30 orang dan diambil dengan purposive sampling. Data penelitian diperoleh dari hasil pemeriksaan LED menggunakan metode manual Westergren dan Automatik.  Hasil: Pemeriksaan Laju Endap Darah pada responden sehat metode Westergren memiliki rata-rata 9,40 mm/jam sedangkan pada metode Automatik 10,15 mm/jam. Pemeriksaan Laju Endap Darah pada responden TB Paru metode Westergren memiliki rata-rata 66,13 mm/jam sedangkan pada metode Automatik 67,80 mm/jam. Tidak terdapat perbedaan rata-rata nilai LED yang signifikan antara pengukuran menggunakan metode manual Westergren dan Automatik pada penderita TB Paru (p = 0,878). Kesimpulan: Tidak terdapat perbedaan rata-rata nilai LED yang signifikan antara pengukuran menggunakan metode manual Westergren dan Automatik pada penderita TB Paru.Kata kunci: Laju Endap Darah, Metode, Westergren, Automatik

Pulmonary tuberculosis treatment causes various side effects including nausea and vomiting, itching, vision problems, and anemia. Drug side effects in the early stages are one of the causes of non-adherence to complete treatment. The purpose of this study was to analyze the effect of the side effects Tuberculosis treatment in the early stages on treatment compliance for tuberculosis patients. This study used a cross sectional design. Samples were taken as many as 71 respondents, the instruments used were side effects of early-stage tuberculosis treatment and compliance with tuberculosis treatment in tuberculosis patients from the Morisky Medication Adherence Scale (MMAS). The results of the study found 97.7% adherent and 2.3% non-adherent, 39.5% mild side effects and 60.5% severe side effects. Chi-square test showed that no significant difference between the side effects of TB treatment in the early stages of tuberculosis treatment compliance in patients p = 0.669 at significant level of 95% (α = 0.05). There was no effect between the side effects of Tuberculosis treatment in the early stages of treatment compliance for Tuberculosis patients. It was recommended that health workers continue to monitor the side effects of tuberculosis treatment and provide motivation to carry out treatment completely. The condition of patients with pulmonary tuberculosis (TB) with poor nutritional status will slow down the healing period. Overcoming malnutrition in pulmonary tuberculosis patients is carried out by providing supplementary food (PMT), especially those based on local food ingredients. In addition to needing additional foods rich in macro nutrients, TB sufferers also need micronutrients that can trigger increased body immunity, such as vitamin C. This study aims to see the effect of giving cohi fish biscuits and vitamin C on the weight of patients with pulmonary tuberculosis in Percut Sei Tuan District in 2019. The design of this study was a quasi experimental pretest-posttest control group design which was carried out in Percut Sei Tuan District, Deli Serdang Regency, North Sumatra starting from May-July 2019. Subjects were 52 new cases of pulmonary tuberculosis patients with malnutrition who were divided into three groups: 16 people in the first group who received 100 g/day of cohi fish biscuits, 16 people in the second group who received 100 g/day of cohi fish biscuits and 500 mg/day of vitamin C, and 20 people in the control group. The treatment was carried out for 30 days. Giving 100 grams of cohi fish biscuits contributed 583.29 kcal of calories, 15.32 grams of carbohydrates, 25.54 grams of protein and 46.65 grams of fat. Statistical test using one-way ANOVA, the results obtained were no difference in pre-test weight of patients with pulmonary TB (p=0.616), there was no difference in post-test body weight of patients with pulmonary TB (p=0.674), there was a difference in changes in patient weight. Pulmonary TB (p=0.00). There was an increase in body weight in the second group of 0.7 kg, and even though there was no increase in body weight in the first group, giving cohi fish biscuits was able to withstand the weight loss compared to the control group. The conclusion of this study is that cohi fish biscuits and vitamin C can be an alternative to PMT to increase the weight of patients with TB. AbstrakTuberkulosis masih merupakan masalah kesehatan di Indonesia karena prevalensi yang masih tinggi,i terutama di negara berkembang. Karena penyebarannya yang tinggi, maka perlu diketahui bagaimana profil penderita penyakit ini agar penularannya bisa diminimalkan. Tujuan dari penelitian ini adalah untuk mengetahui profil penderita tuberculosis paru BTA positif yang berobat di Balai Pengobatan Penyakit Paru (BP4) Lubuk Alung periode 1 Januari 2012 – 31 Desember 2012. Penelitian ini bersifat deskriptif retrospektif. Instrumen yang digunakan adalah data dari rekam medik di Balai Pengobatan Penyakit Paru (BP4) Lubuk Alung sejak 1 Januari 2012 – 31 Desember 2012. Populasi yang ada seluruhnya dijadikan subjek penelitian. Kemudian dilakukan pencatatan dari beberapa variabel yang diteliti. Hasil penelitian ini menunjukkan bahwa dalam periode tersebut jumlah penderita yang berobat ke BP4 Lubuk Alung adalah 19.440 orang, sebanyak 3.224 orang diantaranya suspek. Penderita BTA (+) 1.109 orang. Jenis kelamin laki-laki (70,8%) lebih banyak dari perempuan. Usia terbanyak adalah 21-30 tahun (23,2%). Daerah asal terbanyak adalah Kab. Padang Pariaman (29,4%). Derajat kepositifan BTA sputum terbanyak berupa positif tiga (+3) adalah (44,2%). Tipe penderita terbanyak merupakan penderita kasus baru sebanyak (91,7%). Keluhan terbanyak yang dirasakan ketika berobat adalah batuk (99%). Sebanyak (13,4%) memiliki penyakit penyerta selain tuberkulosis. Riwayat penggunaan obat sebelumnya sebanyak (11,3%). Sebanyak (99%) dirujuk ke puskesmas dan unit pelayanan kesehatan terdekat. Berdasarkan pendataan profil penderita TB Paru BTA Positif bisa dilihat paling banyak adalah derajat (+3) dan dirujuk ke unit pelayanan terdekat.Kata kunci: profil, tuberkulosis paruAbstractTuberculosis is still a health problem in Indonesia because the prevalence is still high, especially in developing countries. The purpose of this study was to determine the profile of positive acid-fast-bacilli (BTA) in pulmonary tuberculosis patients who seek treatment at Medical Center for Pulmonary Diseases (BP4) Lubuk Alung during the period 1 January 2012-31 December 2012.This is a descriptive retrospective study by taking the data from medical records in BP4 Lubuk Alung. Using the enterety of the population. The results of this study indicate that in this period the number of people who went to BP4 Lubuk Alung were 19.440 people, 3.224 of them suspected tuberculosis. Patients with BTA (+) was 1.109 people. We found male 70.78%. Most are 21-30 years of age 23.2%. The area of origin mostly from Kab. Padang Pariaman 29.4%. The degree of sputum smear positivity mostly positive three (+3) was 44.2%. Type of most patients are people with new cases 91.7%. Most complaints was cough 99%. A total of 13.44% had concomitant diseases other than tuberculosis. History of previous anti tuberculosis drugs (OAT) we found in 11.3%. And 99% are referred to hospitals and health care units nearby. Based on the data collection, profile of positif pulmonary TB patients is (+3) and mostly referred to the nearest health center and service unit.Keywords:profile, pulmonary tuberculosis
