Abstract Epidemiological studies have demonstrated that various kinds of urinary element concentrations were different between healthy, prediabetes, and diabetes patients. Meanwhile, many studies have explored the relationship between element concentration and fasting blood glucose (FBG), but the association between joint exposure to co-existing elements and FBG level has not been well understood. The study explored the associations of joint exposure to co-existing urinary elements with FBG level in a cross-sectional design. 275 retired elderly people were recruited from Beijing, China. The questionnaire survey was conducted, and biological samples were collected. The generalized linear model (GLM) and two-phase Bayesian kernel machine regression (BKMR) model were used to perform in-depth association analysis between urinary elements and FBG. The GLM analysis showed that Zn, Sr, and Cd were significantly correlated with the FBG level, under control potential confounding factors. The BKMR analysis demonstrated 8 elements (Zn, Se, Fe, Cr, Ni, Cd, Mn, and Al) had a higher influence on FBG (Posterior inclusion probabilities >0.1). Further intensive analyses result of the BKMR model indicated that the overall estimated exposure of 8 elements was positively correlated with the FBG level and was statistically significant when all element concentrations were at their 65th percentile. Meanwhile, the BKMR analysis showed that Cd and Zn had a statistically significant association with FBG levels when other co-existing elements were controlled at different levels (25th, 50th or 75th percentile), respectively. The results of the GLM and BKMR model were inconsistent. The BKMR model could flexibly calculate the joint exposure to co-existing elements, evaluate the possible interaction effects and nonlinear correlations. The meaningful conclusions were found that it was difficult to get by traditional methods. This study will provide methodological reference and experimental evidence for the association between joint exposure to co-existing elements and FBG in elderly people.

Diabetes Mellitus (DM) is a chronic hyperglycemic condition which may affect all aspects of life of the sufferer, and it can be life threatening. This problem can be minimized by doing self-care. Self-care has an influence on glycemic control, prevention of complications due to uncontrolled blood glucose and improving the quality of life for people with diabetes mellitus. This study aimed to determine the effect of self-care on fasting blood glucose of type 2 diabetics. This type of research was observational with cross-sectional design. The inclusion criteria for the study were patients with a diagnosis of diabetes mellitus aged > 18-year-old, treatment at a health center >8 weeks, had records of the last blood glucose fasting ≤1 month before the interview, while the exclusion criteria were patients who refused to participate and patients who could not communicate. Self-care measurement uses the Diabetes Self-Care Management Question (DSMQ) instrument. The number of respondents from this study was 115 people where 79,1% of respondents had a good level of self-care and 47% of respondents with fasting glucose levels that had not been achieved. The data were analyzed using Chi-square test. From this study, it was found that self-care had an effect on fasting blood glucose (OR=3,349, P<0,05). The lowest value of self-care in this study is the domain of physical activity and health-care, therefore patient motivation needs to be improved by exercise and routine check-up because self-care is one of the important factors that could control blood glucose levels.

Background: Epidemiological studies generally aim to make simple but unbiased measurements of individuals. For this reason measurements of metabolic variables (including blood lipids, glucose and insulin) are usually carried out after a period of fasting. Few studies have examined the extent to which the use of a defined protocol for fasting in epidemiological studies abolishes the influence of fasting duration and time of day on biochemical measurements. Methods: Cross-sectional survey of British Regional Heart Study participants (4252 men aged 60-79 years), in which men without diabetes were asked to provide a blood sample after fasting for at least 6 h. Serum total, HDL and LDL cholesterol, triglyceride and insulin, and plasma glucose concentrations were measured between 08:00 h and 18:00 h. Results: Non-fasting men had lower mean LDL cholesterol and higher glucose, insulin and triglyceride levels than fasting men; these differences were more marked among diabetics. Among fasting men without diabetes, insulin and glucose levels were strongly related to time of day, falling gradually throughout the morning and remaining stable in the afternoon. Because of these relationships and the dependence of fasting duration on time of day, insulin and glucose displayed a periodic relation with fasting duration above 6 h. These associations were largely abolished by adjustment for time of day; associations with time of day were unaffected by adjustment for fasting duration. Triglyceride concentrations fell with increasing fasting duration. This relationship was also mediated through a gradual increase in triglyceride levels throughout the day. Adjustments to compensate for these variations are described. Discussion: Even after fasting, biochemical measurements may still differ in relation to fasting duration and time of day. In epidemiological studies, it is important to standardize both the period of fasting and the time of day as much as possible, and make adjustments where necessary.

BACKGROUND: Obesity is commonly associated with a systemic low grade inflammation and insulin resistance state. Although it is still being debated, increased lipolysis is known as one of the risk factors for inflammation and insulin resistance. Two factors already known to affect lipolysis are Angptl3, known as prolipolytic factor, and FGF21, known as antilipolytic factor. The aim of this study was to observe the role of Angptl3 and FGF21 to lipolysis, inflammation and insulin resistance in non diabetic obese male.METHODS: This was an observational study with cross sectional design. One hundred and thirty male subjects aged 30-60 years with non diabetic abdominal obesity characterized by waist circumference 97.32±5.63 cm and fasting blood glucose 90.19±8.78 mg/dL.RESULTS: The results of this study showed a correlation between Angptl3-FFA (r=0.203; p=0.021; R square=0.041; p=0.021), Angptl3-FABP4 (r=0.330; p=0.000; R square=0.109; p=0.000) and Angptl3-TNFα (r=0.288; p=0.001; R square=0.049; p=0.011). There was a correlation between FGF21-FABP4 (r=0.218, p=0.013; R square=0.047, p=0.013) and FGF21 HOMA-IR (r=0.308, p=0.000; R square=0.046, p=0.014).CONCLUSIONS: We conclude that Angptl3 may affect lipolysis and inflammation while FGF21 may affect lipolysis and insulin resistance. The increased FGF21 concentration might occur as a compensation (negative feedback mechanism) to reduce lipolysis and increase insulin sensitivity in non diabetic obese males. Further studies might be needed to observe Angptl3 and FGF21 profile in more severe obese population in Indonesia.KEYWORDS: obesity, lipolysis, inflammation, insulin resistance

Low magnesium intake has been shown to be associated with an increased risk of type 2 diabetes mellitus (T2DM) in several studies conducted in high-income countries. However, very few studies have been performed in Africa, where many countries have a growing rate of T2DM. We conducted a pilot cross-sectional study among 63 women in Ghana to investigate the association between magnesium intake and glycemic markers. We assessed dietary magnesium using a food frequency questionnaire and glycemic markers using fasting blood glucose and glycated hemoglobin A1c (HbA1c). Our findings showed that the mean magnesium intake was 200 ± 116 mg/day. The prevalence of T2DM was 5% by measuring fasting blood glucose and 8% by measuring HbA1c. Unadjusted linear regression models revealed that higher magnesium intake significantly predicted higher fasting blood glucose levels (β = 0.31; 95% CI: 0.07, 0.55; p = 0.01) and HbA1c levels (β = 0.26; 95% CI: 0.01, 0.51; p = 0.04). In adjusted analyses, magnesium intake was no longer significantly associated with either fasting blood glucose levels (β = 0.22; 95% CI: −0.03, 0.46; p = 0.08) or HbA1c levels (β = 0.15; 95% CI: −0.08, 0.39; p = 0.20). In conclusion, our study did not show a significant association between magnesium intake and glycemic markers in women of reproductive age in Ghana. The results of this study need to be further substantiated because this was the first study to examine magnesium intake and glycemic markers in this population in Africa.
