Background: Dengue hemorrhagic fever (DHF) is still periodically around developing countries including Indonesia. Morbidity and mortality of DHF can be reduced if early diagnosis and appropriate management. Objective: Our study evaluate risk factors of death in pediatric DHF patients hospitalized in Wahidin Sudirohusodo Hospital Makassar. Methods:  we review the medical records of patients aged ≤ 18 years from January 2016 to December 2018 with confirmed DHF  based on WHO criteria and serologically positive anti-dengue Ig M or positive anti-dengue IgM and Ig G. Results: During the study period, 70 patients aged 1-17 years with the complete medical records enrolled in this study.  The DHF severity consisted of 37 cases (52,9%) with shock(DSS) and 33 cases (41,7%) without shock and  mostly of them was  admitted to the hospital on > 3 days of fever (63 cases /90,0%). Boys were predominantly (39/55,7%) found than girls (31/44,3%) and the majority of cases above 5 years (50/71,4%) with well-nourished patients in 46 cases (65,7%).  The hematocrit level ≥ 40 mg/dl, platelets ≤ 40.000/mm3, and leukocyte ≤ 4000 mm3/l were observed in 41 cases (58,6%), 36 cases (51,4%), and  48 cases (68,6%); respectively. Death was observed in four girls (5.7%) (p 0,034/OR 1,148/ 95% CI 1,003 - 1,315) with DSS because of  severe condition on admission. Conclusions: Girl was an  independent risk factor of death among children with DHF.

Purpose:We examined risk factors of dengue hemorrhagic fever death in Sardjito hospital. Method: We conducted case control study from patient medical records and interview with parents. Results: We found 29 deaths and 58 survived. The probability of death in obesity children was 6.29 times higher than non obesity children and the probability of death in children with prolonged shock was 12.14 times higher than children without prolonged shock. Other variables were family occupation, family income, residential zones, transportation, treatment financing, accuracy of diagnosis in previous health facilities, and fluid resuscitation before being referred has no significant relationship with dengue mortality. Conclusion: Obesity and prolonged shock were risk factors of dengue hemorrhagic fever death in children. Improve education to parents about high risk of shock syndrome among patients, especially for obesity children. Further studies related to social determinants in dengue hemorrhagic fever death are also necessary.

AbstractWe aimed to retrospectively analyze the risk factors of a continuous dienogest (DNG) therapy for serious unpredictable bleeding in patients with symptomatic adenomyosis. This is a retrospective study based on data extracted from medical records of 84 women treated with 2 mg of DNG orally each day between 2008 and 2017. 47 subjects were excluded from the original analyses due to an inadequate subcategorization into subtype I and subtype II and a lack of hemoglobin levels. The influence of various independent variables on serious unpredictable bleeding was assessed. Of the 37 eligible patients who received the continuous DNG therapy, 14 patients experienced serious unpredictable bleeding. Univariate analysis revealed that the serious bleeding group had subtype I adenomyosis (P = 0.027). There was no correlation between age, parity, minimum hemoglobin level before treatment, previous endometrial curettage, and duration of DNG administration, or uterine or adenomyosis size and the serious bleeding. A DNG-related serious unpredictable bleeding is associated with the structural type of adenomyosis (subtype I) in patients with symptomatic adenomyosis.

Abstract Objective To investigate the clinical characteristics and long-term prognosis of coronary intervention in patients with premature coronary artery disease (PCAD) between different genders. Methods From January 2013 to December 2013, 4 744 patients diagnosed as PCAD with percutaneous coronary intervention (PCI) in our hospital were enrolled. The general clinical data, laboratory results and interventional treatment data of all patients were collected, and the occurrence of major adverse cardio-cerebrovascular events (MACCE) within 2 years after PCI was followed up. Results Of the 4 744 patients undergoing PCI, 3 390 (71.5%) were males and 1 354 (28.5%) were females. The 2-year follow-up results showed that the incidence of BARC grade 1 hemorrhage in female patients was significantly higher than that in male patients (6.9% vs. 3.7%; P<0.001); however, there was no significant difference in the incidence of major adverse cardiovascular and cerebrovascular events (MACCE), all-cause death, cardiac death, recurrent myocardial infarction, revascularization (target vessel revascularization and target lesion revascularization), stent thrombosis, stroke and BARC grade 2–5 hemorrhage between the two groups (P>0.05). Multivariate COX regression analysis showed that gender was an independent risk factor for BARC grade 1 bleeding events in PCAD patients (HR=2.180, 95% CI: 1.392–3.416, P<0.001), but it was not an independent risk factor for MACCE and BARC grade 2–5 bleeding. Hyperlipidemia, preoperative SYNTAX score, multivessel lesions and NSTE-ACS were the independent risk factors for MACCE in PCAD patients with PCI (HR=1.289, 95% CI: 1.052–1.580, P=0.014; HR=1.030, 95% CI: 1.019–1.042, P<0.001; HR=1.758, 95% CI: 1.365–2.264, P<0.001; HR=1.264, 95% CI: 1.040–1.537, P=0.019); gender, hyperlipidemia, anticoagulant drugs like low molecular weight heparin or sulfonate were the independent risk factors for bleeding events (HR=1.579,95% CI 1.085–2. 297, P=0.017; HR=1.305, 95% CI 1.005–1.695, P=0.046; HR=1.321, 95% CI 1.002–1.741, P=0.048; HR=1.659, 95% CI 1.198–2.298, P=0.002). Conclusion The incidence of minor bleeding in women with PCAD is significantly higher than that in men; After adjusting for various risk factors, gender is an independent risk factor for minor bleeding events, but not an independent risk factor for MACCE in patients with PCAD. Introduction Aortic valve calcification (AVC) accelerates development of aortic valve stenosis and cardiovascular complications. Hyperphosphatemia is one of the key risk factors for aortic valve calcification. Aim The aim of this study was to evaluate the prevalence of AVC in patients on regular hemodialysis and to assess the impact of different factors on its appearance. Method: The study investigated a total of 115 patients treated in the Hemodialysis Department of the Urology and Nephrology Clinic at the Kragujevac Clinical Center in Serbia. The variables investigated were: serum albumin, C-reactive protein (CRP), homocysteine, total cholesterol, LDL-cholesterol (LDL-C), HDL-cholesterol (HDL-C), triglycerides (TG), Apolipoprotein A-I (Apo A-I), Apolipoprotein B (Apo B) and lipoprotein (a), calcium, phosphate and parathormone, and calcium-phosphorus product (Ca × P). Patients were evaluated by echocardiography for AVC. Statistical analysis included univariate and multivariate logistic regression analysis. Results Univariate regression analysis showed that serum phosphate levels and Ca × P are the most important risk factors for AVC (p<0.001). Multivariate logistic regression analysis revealed that hyperphosphatemia is an independent risk factor for AVC (p<0.001). Conclusion Hyperphosphatemia is an independent risk factor for aortic valve calcification.

Objectives: Prehypertension frequently progresses to hypertension and is associated with cardiovascular diseases, stroke, excess morbidity and mortality. However, the identical risk factors for developing hypertension from prehypertension are not clarified. This study is conducted to clarify the risks. Methods: We conducted a retrospective 5-year cohort study using the data from 3,584 prehypertensive Japanese adults (52.1±11.0 years, 2,081 men) in 2004 and reevaluated it 5 years later. We calculated the cumulative incidences of hypertension over 5 years, then, we detected the risk factors and calculated odds ratios (ORs) for developing hypertension by crude analysis and after adjustments for age, sex, body mass index, smoking and drinking habits, baseline systolic and diastolic blood pressure, pulse rate, diabetes mellitus, dyslipidemia, chronic kidney disease, and serum uric acid. We also evaluated whether serum uric acid (hyperuricemia) provided an independent risk for developing hypertension. Results: The cumulative incidence of hypertension from prehypertension over 5 years was 25.3%, but there were no significant differences between women and men (24.4% vs 26.0%, p=0.28). The cumulative incidence of hypertension in subjects with hyperuricemia (n=726) was significantly higher than those without hyperuricemia (n=2,858) (30.7% vs 24.0%, p<0.001). After multivariable adjustments, the risk factors for developing hypertension from prehypertension were age (OR per 1 year increased: 1.023; 95% CI, 1.015-1.032), women (OR versus men: 1.595; 95% CI, 1.269-2.005), higher body mass index (OR per 1 kg/m 2 increased: 1.051; 95% CI 1.021-1.081), higher baseline systolic blood pressure (OR per 1 mmHg increased: 1.072; 95% CI, 1.055-1.089) and diastolic blood pressure (OR per 1 mmHg increased: 1.085; 95% CI, 1.065-1.106), and higher serum uric acid (OR pre 1 mg/dL increased: 1.149; 95% CI, 1.066-1.238), but not smoking and drinking habits, diabetes mellitus, dyslipidemia, and chronic kidney diseases. Conclusions: Increased serum uric acid is an independent risk factor for developing hypertension from prehypertension. Intervention studies are needed to clarify whether the treatments for hyperuricemia in prehypertensive subjects are useful.

Objective.Vancomycin-resistant enterococci (VRE) are a major cause of nosocomial infection. We sought to compare vancomycin-resistant (VR)Enterococcus faecalisbacteremia and VREnterococcus faeciumbacteremia in cancer patients with respect to risk factors, clinical presentation, microbiological characteristics, antimicrobial therapy, and outcomes.Methods.We identified 210 cancer patients with VRE bacteremia who had been treated between January 1996 and December 2004; 16 of these 210 had VRE. faecalisbacteremia and were matched with 32 patients with VRE. faeciumbacteremia and 32 control patients. A retrospective review of medical records was conducted.Results.Logistic regression analysis showed that, compared with VRE. faecalisbacteremia, VRE. faeciumbacteremia was associated with a worse clinical response to therapy (odds ratio [OR], 0.3 [95% confidence interval (CI), 0.07-0.98];P= .046) and a higher overall mortality rate (OR, 8.3 [95% CI, 1.9-35.3];P= .004), but the VRE-related mortality rate did not show a statistically significant difference (OR, 6.8 [95% CI, 0.7-61.8];P= .09). Compared with control patients, patients with VRE. faecalisbacteremia were more likely to have received an aminoglycoside in the 30 days before the onset of bacteremia (OR, 5.8 [95% CI, 1.2-27.6];P= .03), whereas patients with VRE. faeciumbacteremia were more likely to have received a carbapenem in the 30 days before the onset of bacteremia (OR, 11.7 [95% CI, 3.6-38.6];P<.001). In a multivariate model that compared patients with VRE. faeciumbacteremia and control patients, predictors of mortality included acute renal failure on presentation (OR, 15.1 [95% CI, 2.3-99.2];P= .004) and VRE. faeciumbacteremia (OR, 11 [95% CI, 2.7-45.1];P<.001). No difference in outcomes was found between patients with VRE. faecalisbacteremia and control patients.Conclusions.VRE. faeciumbacteremia in cancer patients was associated with a poorer outcome than was VRE. faecalisbacteremia. Recent receipt of carbapenem therapy was an independent risk factor for VRE. faeciumbacteremia, and recent receipt of aminoglycoside therapy was independent risk factor forE. faecalisbacteremia.

Background Shock in dengue hemorrhagic fever (DHF) still con-stitutes an important problem in children. Predicting DHF patientswho will develop shock is difficult.Objective The aim of this study was to find out factors associatedwith shock in DHF.Methods This was a prospective observational study. Subjectswere children hospitalized from April to July 2000 who met theWHO criteria for DHF and had positive serological confirmation.Association between independent variables (age, gender, dura-tion of fever, abdominal pain, vomiting, hepatomegaly, plateletcount, hematocrit level, and nutritional status) and the dependentvariable (shock) was analyzed by logistic regression model..Results There were 85 children who met the eligibility criteriaconsisting of 50 (59%) boys and 35 (41%) girls with an averageage of 7.1 years (SD 2.88). Shock occurred in 42 (49%) children.Of the 42 children with shock, the age group of 5-9 years madeup the biggest group (57%) consisting of 23 (55%) boys and 19(45%) girls. Hepatomegaly was found in 32 (76%) children, ab-dominal pain and vomiting in 30 (71%) and 36 (86%) children,respectively, and good nutrition in 22 (52%) children. Shock oc-curred mainly on the fourth and fifth days (76%). Almost half ofthe patients (45% and 52%) had hematocrit level of 46-50% andplatelet count of 20,000-50,000/ml, respectively. By logistic re-gression analysis, it was found that duration of fever, abdominalpain, hematocrit level, and platelet count constituted indepen-dent factors correlating with shock in DHF.Conclusion Abdominal pain, fever lasting four to five days, hema-tocrit level of >46%, and platelet count of <50,000/μl were associ-ated with shock in DHF

The paper discusses the materials of investigations dealing with falls as an independent risk factor for fractures in patients with rheumatoid arthritis (RA). It gives data on the incidence and possible risk factors of falls in this category of patients. According to the data obtained, the prevalence of falls in different countries varies from 10 to 50%, which may be related to differences in the methods of collecting information, and the relationship of the investigated factors with the risk of falls in patients with RA is uniquely unproven and calls for further investigations.

Objectives: to determine which of the risk factors above associated with the occurrence of severe preeclampsia at dr. Soetomo Hospital, Surabaya during 2015.Materials and Methods: The type and design of the study were analytic and retrospective. This study was held in the dr. Soetomo Hospital from April until November 2016. The instrument of the study was the medical records then being coded and analysed. The samples were 134 pregnant women, consisting of 67 pregnant women with severe preeclampsia as cases and 67 pregnant women as controls.Results: Maternal obesity (OR= 5,786; 95% CI: 2,300–14,555), history of hypertension (OR= 6,693; 95% CI: 1,848–24,237) and secondary elderly primi (OR= 6,384; 95% CI: 1,357–30,031) are associated with the development of severe preeclampsia.Conclusion: In conclusion, the significant risk factors of severe preeclampsia in dr. Soetomo Hospital Surabaya during 2015 are obesity, history of hypertension and secondary elderly primi variables.

A rise in tigecycline resistance inKlebsiella pneumoniaehas been reported recently worldwide. We aim to identify risk factors, outcomes, and mechanisms for adult patients with tigecycline non-susceptibleK. pneumoniaebacteremia in Taiwan. We conducted a matched case-control study (ratio of 1:1) in a medical center in Taiwan from January 2011 through June 2015. The cases were patients with tigecycline non-susceptibleK. pneumoniaebacteremia, and the controls were patients with tigecycline susceptibleK. pneumoniaebacteremia. Logistic regression was performed to evaluate the potential risk factors for tigecycline non-susceptibleK. pneumoniaebacteremia. Quantitative RT-PCR was performed to analyzeacrA,oqxA,ramA,rarA,andkpgAexpression among these isolates. A total of 43 cases were matched with 43 controls. The 14-day mortality of patients with tigecycline non-susceptibleK. pneumoniaebacteremia was 30.2%, and the 28-day mortality was 41.9%. The attributable mortality of tigecycline non-susceptibleK. pneumoniaeat 14 days and 28 days was 9.3% and 18.6%, respectively. Fluoroquinolone use within 30 days prior to bacteremia was the only independent risk factor for tigecycline non-susceptibleK. pneumoniaebacteremia. Tigecycline non-susceptibleK. pneumoniaewere mostly caused by overexpression of AcrAB and/or OqxAB efflux pumps, together with the upregulation of RamA and/or RarA respectively. One isolate has isolated overexpression ofkpgA. In conclusion, tigecycline non-susceptibleK. pneumoniaebacteremia was associated with high mortality and prior fluoroquinolone use was the independent risk factor for acquisition of tigecycline non-susceptibleK. pneumoniae. The overexpression of AcrAB and/or OqxAB contributes to tigecycline non-susceptibility inK. pneumoniae.
