Metabolic syndrome and other co-morbidities in the structure of COVID-19 mortality in the Republic of Moldova (March-December 2020) The mortality of the population of the Republic of Moldova through COVID-19 in March-December 2020 made up 2846 (100%) people confirmed with SARS CoV-2 (women –1362/2846 (47,8%), men — 1484/2846 (52,2%). In the urban sector it is 1649/2846 (58,0%), including Chisinau — 1034/2846 (36,3%), Balti — 158/2846 (5,6%), in the rural sector — 1197/2846 (42%). Mortality in the age groups constituted for the age group of 30-70 years (men — 862/2846 (30,3%), women — 805/2846 (28,2%). The share of comorbidity was 1905/2846 (66,9%), the structure of the main diseases being: diseases of the cardiovascular system – 966/1905 (33,9%); diabetes mellitus – 523/1905 (27,5%); malignant tumors – 91/1905 (4,8%). In the age groups — 30-70 years: diseases of the circulatory system – 485/1905 (25.4%), diabetes – 335/1905 (17.6%), cancer – 62/1905 (3.3%).

Relevance: According to Globocan 2018, ovarian cancer (OC) ranks 18th among all other cancers affecting women around the world. More than 295,414 new cases of OC were reported only in 2018. The incidence is low in Western Europe and is high in Latvia, Poland, Lithuania, Estonia, Russia, and Kazakhstan. In 2018, ovarian malignancies ranked 8th (3.1%) in Kazakhstan among all malignant tumors. At that, there are no clearly recognized preventative measures to ensure the early detection of OC. 70% of ovarian malignancies are detected at stage III-IV. The purpose of this study was to conduct a comparative analysis and assessment of the dynamics of the prevalence of OC in the Republic of Kazakhstan in 2013-2018. Results: The analysis of age-related OC incidence in Kazakhstan showed the presence of ovarian malignancies in all age groups, with a marked increase by the age of 65-69 years. The majority of patients were women of the working age. Differences were found in the incidence rates by regions of the country. In the study period, the incidence was high in Qostanai, Pavlodar, and North Kazakhstan regions (northern part of the country) and Almaty. The incidence was below the national average in Atyrau, Jambyl, and South Kazakhstan regions (southern and western parts of the country). Recent years have witnessed a sharp increase in OC incidence in East Kazakhstan and Karaganda regions. Conclusion: The analysis of OC prevalence in the Republic of Kazakhstan and by regions showed an increase in OC incidence. There is a marked increase in OC detection in the northern areas of the country and a decrease in the number of cases in the south.

Background: All these metabolic derangements exist for many years in the asymptomatic phase of type 2 diabetes and they predispose to development of complications even before clinical diagnosis. Metabolic syndrome is considered to be a precursor of type 2 diabetes. Present study is primarily aimed to study the prevalence of micro vascular complications and metabolic syndrome in newly diagnosed type 2 diabetes mellitus patients of low socio-economic group.Methods: This is a cross sectional prospective study conducted in the dept. of general medicine Konaseema institute of medical science Amalapuram, Andhra Pradesh, India from November 2016 to October 2018. Based on exclusion and inclusion criteria 103 patients were enrolled for this study. Various parameters like age, sex, body mass index, waist circumference, lipid profile, systolic and diastolic blood pressure, neuropathy, nephropathy and retinopathy was measured.Results: The mean age of the patient was 48±10.0 years. Fasting and 2 hours OGTT was 174.6±46.8 mg/dl and 255.6±75.6 mg/dl. The mean of total cholesterol was 204.7±41.9 (mg/dl), Triglycerides was 218±83.4 (mg/dl) and HDL was 44±5.3 (mg/dl). Symptomatic neuropathy constituted 35.6% in 51-60 age group and 27.1% in 31-40 and 41-50 age groups. Objective neuropathy constituted highest (36.6%) in 51-60 age group. Retinopathy constituted highest (60%) in 51-60 age group and nephropathy constituted 26.3 % in 21-30 and 51-60 age groups.Conclusions: Prevalence of microvascular complications in newly diagnosed diabetics of low socioeconomic group were as follows: symptomatic neuropathy-57.3%, objective neuropathy-39.8%, retinopathy-4.9%, nephropathy-18.4%. These were similar to published studies from general population from the same geographical area.

Objective: To see the frequency of diabetes mellitus and impaired fasting glucose levels in hypertensive patients. Design:Descriptive study. Place of Study: OPD / Filter clinic. Pakistan Institute of Medical Sciences. Islamabad. Patients and Methods: Thisstudy was carried out on 116 hypertensive patients during the period from August to September '2008. Out of 116 patients, 64 werefemales and 52 were males with age group between 20 to 70 years. Secondary causes are ruled out on the basis of history and physicalexamination. Blood samples were sent to PIMS laboratory for fasting glucose. Data and results were analyzed in SPSS. Results:According to this study, out of 116 patients, there were18 patients who had diabetes mellitus while 33 had hypertension with impairedfasting glucose tolerance which is statistically significant. The females as compared to males were increased in number who had bothdiabetes mellitus and impaired fasting glucose. According to age diabetes mellitus increased in age groups between 41-50 and 61-70years while impaired fasting glucose were increased in age group between 31-60 yrs. The results indicate that impaired fasting glucose ispresent in significant number of hypertensive patients. Conclusions: Hypertension is associated with diabetes mellitus and impairedfasting glucose.

Background The burden of diabetes is considerable not only globally but also nationally within Korea. The Global Burden of Disease study derived the disability-adjusted life years (DALYs) of diabetes depending on its complications as individual severity using prevalence-based approach from 2017. Conversely, the Korean National Burden of Disease study based on an incidence-based approach does not incorporate the severity of diseases. This study aimed to simulate incidence-based DALYs of type 2 diabetes mellitus (T2DM), given diabetic complications as disease severity using a Markov model. Methods We developed a model with six Markov states, including incident and existing prevalent cases of diabetes and its complications and death. We assumed that diabetes and its complications would not be cured. The cycle length was one year, and the endpoint of the simulation was 100 years. A 5% discount rate was adopted in the analysis. Transition cases were counted by 5-year age groups above 30 years of age. Age- and sex-specific transition probabilities were calculated based on the incident rate. Results The total DALY estimates of T2DM were 5,417 and 3,934 per 100,000 population in men and women, respectively. The years of life lost in men were relatively higher than those in women in most age groups except the 80–84 age group. The distribution of years lived with disability by gender and age group showed a bell shape, peaking in the 55–59 age group in men and 65–69 age group in women. Conclusions The burden of T2DM considering its complications was larger compared to the outcomes from previous studies, with more precise morbid duration using the Markov model.

The percentage for ischaemic heart disease of all deaths from diseases of the circulatory system for Whites in the age group 25 — 34 is 63% for males and 21% for females. In the age group 35 — 44 the percentages are 77% for males and 34% for females (22, p.1025).

Several studies emphasize that temperature-related mortality can be expected to have differential effects on different subpopulations, particularly in the context of climate change. This study aims to evaluate and quantify the future temperature-attributable mortality due to circulatory system diseases by age groups (under 65 and 65+ years), in Lisbon metropolitan area (LMA) and Porto metropolitan area (PMA), over the 2051–2065 and 2085–2099 time horizons, considering the greenhouse gas emissions scenario RCP8.5, in relation to a historical period (1991–2005). We found a decrease in extreme cold-related deaths of 0.55% and 0.45% in LMA, for 2051–2065 and 2085–2099, respectively. In PMA, there was a decrease in cold-related deaths of 0.31% and 0.49% for 2051–2065 and 2085–2099, respectively, compared to 1991–2005. In LMA, the burden of extreme heat-related mortality in age group 65+ years is slightly higher than in age group <65 years, at 2.22% vs. 1.38%, for 2085–2099. In PMA, only people aged 65+ years showed significant temperature-related burden of deaths that can be attributable to hot temperatures. The heat-related excess deaths increased from 0.23% for 2051–2065 to 1.37% for 2085–2099, compared to the historical period.

According to the WHO Strategic Plan, measles should be eradicated in 2020 in the five WHO Regions including European Region. However, large measles outbreaks are being periodically registered in diverse European countries. In the Republic of Serbia (SRB), 5,076 measles cases were detected in 2018, among which 15 cases were fatal.Aim of the study was to examine herd immunity to measles and rubella viruses in the population of the Republic of Serbia.Materials and methods. Blood serum samples obtained in 2018 and 2019 from conditionally healthy residents of the Republic of Serbia were tested for the presence of IgG antibodies to measles and rubella viruses in five age groups: I — children from 2 to 6 years old, II — children from 8 to 14 years old, III — 15 to 24 years old, IV — 25 to 49 years old and V — over 50 years old. A total of 1000 samples were obtained, 200 sera in each group. Enzygnost® Anti-Measles virus/IgG and Enzygnost® Anti-Rubella virus/IgG ELISA test systems (Siemens Healthcare Diagnostics Products GmbH, Germany) were used according to the manufacturer's instructions.Results. Overall, around 23.0% and 33.7% of the surveyed persons had no or low level of anti-measles IgG antibody (≥ 275.0 — ≤ 1000.0 IU/1). In age group I, 60% children contained no or “low” anti-measles antibodies titer (29.5% and 30.5%, respectively). In addition, low antibody titer level was mainly detected in individuals from age group II and III (p < 0.05). A third of children under 8—14 contained high IgG-antibodies titer against measles (> 3000.0 IU/l) that might serve as an evidence that such subjects recently recovered after measles. Similar results were obtained for IgG antibodies to rubella in the same age groups.Discussion. The study results evidence about altered routine immunization against measles and rubella in children aged 12—15 months (first vaccination) and those at age of 6—7 years (revaccination) with MMR vaccine. The data obtained correlate with official data on coverage with measles and rubella vaccines in the Republic of Serbia.

The object of the study was to assess the frequency of visceral obesity, cardiometabolic risk factors, metabolic syndrome and diabetes mellitus in women of different ages. Materials and methods: 562 women from 20 to 65 years old were included and divided into 3 groups: 273 women < 45 years old (group A), 160 women of 45 - 55 years old (group B) and 129 - > 55 years old (group C). The examination included antropometric parameters, measurement of blood pressure, blood samples collection in order to investigate concentrations of glucose and lipids. Results: the frequency of visceral obesity was 52,0%, and metabolic syndrome - 18,1%. These frequencies increased in parallel with age. Higher insidence of arterial hypertension, hypercholesterolemia, hypertrigliceridemia and impaired fasting plasma gucose level were also observed with age. Diabetes mellitus was revealed in 1,2% of women. It`s frequency was 0,6% in group B and 4,7% in group C; in women younger than 45 years old no cases of diabetes mellitus were observed. Conclusion: significant increase of frequencies of visceral obesity, cardiometabolic risk factors, metabolic syndrome and diabetes mellitus in women older than 45, and especially - older than 55 years old stresses the necessity of an intense medical care for women of these age groups, and implementation of measures for prevention of cardiovascular diseases and diabetes mellitus.

Background and aims. The Republic of Moldova ranks among the European Region countries with the highest global incidence of tuberculosis. Almost 5% of the cases with pulmonary TB had been diagnosed with DM in 2017. The aim of this study was to assess the impact of diabetes mellitus on the anti-tuberculosis treatment effectiveness for the improvement of the disease outcome. Methods. A retrospective, longitudinal and case-control study, which included a total number of 252 patients diagnosed with pulmonary tuberculosis and distributed into a study group of 93 patients diagnosed with diabetes mellitus and a control group of 159 patients without glycemic disorders was performed. The statistical analysis was performed using EpiInfo software. The statistical analysis of the differences between normally distributed continuous variables was tested with the Student T-test. Results. Based on the collected data we established that the majority of the patients with glycemic disorders were diagnosed with the type 2 diabetes, associated with hyperglycemia and in half of them complications of diabetes were revealed. The age older 55 years and the low economical state were common characteristics of the diabetic patients. Tuberculosis was detected in every second diabetic patient through the radiological screening compared with the passive detection of most of the non-diabetic patients. The anti-tuberculosis treatment outcome was endangered by a higher rate of the adverse drug events in patients with diabetes, which contributed to death in 15%, lost to follow-up 7%, and therapeutic failure in 6%. Conclusions. The anti-tuberculous treatment outcome in patients diagnosed with tuberculosis and diabetes mellitus was significantly diminished by glycemic disorders, history of the previous anti-tuberculous treatment and adverse drug reactions. Individualized therapeutic approach to tuberculosis could improve the treatment effectiveness.

Background: Ovarian malignancy is the second most common cancer of the female reproductive system and the leading cause of death from gynecologic malignancy. With increase in longevity, the incidence of epithelial ovarian cancer is increasing and its etiopathology remains unknown. A female’s risk at birth of having ovarian tumour sometime in her life is 6-7%. Two third of ovarian tumours occur in women of reproductive age group. The aim was to study the distribution of morphological pattern of benign, malignant, and nonneoplastic lesions of the ovary in different age groups and to determine the likelihood of bilateral involvement in different morphologic subtypes.Methods: A retrospective study from January 2018 to December 2020 was undertaken. A total of 210 surgical specimens were obtained. Detailed clinical information and radiological findings were recorded from the case sheets. Grossing of the surgical specimens was done in the pathology department, followed by histological examination.Results: Of 210 cases, benign cases were 140 (66.7%), malignant cases were 70 (33.3%). Surface epithelial tumors were most common (116/55.2%) followed by germ cell tumors (76, 36.1%) followed by others. Serous cystadenoma was commonest benign tumor (58, 41.4%). Serous adenocarinoma was commonest malignant tumor (19, 27.1%). Benign tumours were more common in the younger age group i.e. <40 years of age whereas malignant tumours were supervenes with increase in age. Most benign ovarian tumors (54, 38.6%) were seen between 31-40 years whereas most malignant tumors (24, 34.3%) were seen above 40 years. In 1st two decades, germ cell tumors were more common than other tumours.Conclusions: The prognosis and varying therapeutic strategies of ovarian tumours necessitate an accurate pathological evaluation. Histopathological study is still the gold standard in diagnosing most of these tumours.
