Abstract BackgroundTo observe the effect of periodic ketogenic diet intervention on newly diagnosed overweight or obese patients with type 2 diabetes.Methods60 overweight or obese T2DM patients were randomly divided into ketogenic diet group and diabetes diet control group, 30 patients in each group. The changes and significance of blood glucose, blood lipid, body weight, insulin, uric acid and other indicators before and after the intervention were observed.ResultsThe Weight, BMI, Waist, TG, TC, LDL, HDL, FBG, FINS, HbA1c of the two groups decreased significantly compared with the previous period (P < 0.05). However, UA showed an increasing trend in the ketogenic diet group and no significant change in UA in the diabetic diet control group (P > 0.05). Long-term adherence to the ketogenic diet was not as strong as that of the diabetic diet.ConclusionThe phased-ketogenic diet can control not only weight but also blood glucose and blood lipid in patients with overweight or obese T2DM. But long-term persistence is difficult.

This study is to investigate the effect of atorvastatin combined with losartan on inflammatory factors, vascular endothelial function, and cardiovascular events in patients with diabetic nephropathy. A total of 128 patients with diabetic nephropathy treated in our hospital from January 2014 to December 2015 were selected as the study subjects, and 64 cases were randomly divided into observation group and 64 cases in the control group. The control group was treated with losartan on the basis of routine treatment, and the observation group was treated with atorvastatin on the basis of the control group. The blood lipid, inflammatory factors, changes in vascular endothelial function and cardiovascular events were compared between the two groups. The levels of total cholesterol (TC), triglyceride (TG), low-density lipoprotein cholesterol (LDL-C), and high-density lipoprotein cholesterol (HDL-C) were not significantly different between the two groups before treatment ( P > 0.05); after treatment, the levels of TC, TG, and LDL-C in the observation group were significantly lower than those of the control group, and the level of HDL-C was significantly higher than that of the control group ( P < 0.05). The levels of high-sensitivity C-reactive protein (hs-CRP), tumor necrosis factor alpha (TNF-α), and interleukin 6 (IL-6) were not statistically different between the two groups before treatment ( P > 0.05); after treatment, the levels of hs-CRP, TNF-α, and IL-6 in the observation group were significantly lower than those of the control group ( P < 0.05), the level of HDL-C was significantly higher than that of the control group ( P < 0.05). There were no significant differences in the levels of endothelin-1 (ET-1) and nitric oxide (NO) between the two groups before treatment ( P > 0.05). After treatment, the level of ET-1 in the observation group was significantly lower than that of the control group ( P < 0.05), and the level of NO was significantly higher than that of the control group ( P < 0.05). After treatment, all patients were followed up for 2 years, and the incidence of secondary cardiovascular events in the observation group was 12.50% (8/64), which was significantly lower than 29.69% (19/64) of the control group ( P < 0.05). Combination of atorvastatin and losartan can significantly improve the levels of blood lipid, inflammatory factors, and vascular endothelial function in patients with diabetic nephropathy and can effectively reduce the incidence of cardiovascular events.

69 young men with obesity manifesting at puberty have been examined. The average age was 19,22±0,26. 17 healthy young men, whose average age was 22 ± 0,72 years old, constituted a control group. The examined were divided according to their blood pressure (BP): the first subgroup included 36 young men with normal blood pressure, the other subgroup included 33 young men with arterial hypertension. Levels of blood lipid spectrum, levels of leptin, resistin, adiponectin, insulin in serum, urinary metanephrine excretion were measured. We obtained the following results: young men with obesity identified atherogenic changes in lipid metabolism, insulin resistance and compensatory hyperinsulinemia. Regardless of the level of blood pressure they showed a significant increase in leptin levels. In the subgroup of patients with hypertension we found increased urinary excretion of metanephrine, indicating increased activity of the sympathoadrenal system and its involvement in the formation of hypertension. The level of adiponectin in the surveyed tended to decrease, more pronounced in the combination of obesity and hypertension.

Metabolic syndrome (MetS) is a combination of physiologically dysregulated parameters that can include elevated fasting blood glucose, high blood pressure, central obesity, increased triglyceride levels, insulin resistance, diabetes, elevated low density lipoprotein levels, and reduced high density lipoprotein levels in the blood. Effective clinical management of MetS is critical as it is strongly associated with long lasting and fatal complications in patients. Alongside standard care of lifestyle changes and medication, dietary supplements derived from herbal resources could be an alternative therapeutic strategy that is safe, efficient, culturally acceptable, and has few side effects. Of the dietary supplements, spicy foods have always been considered a great source of functional bioactive compounds. Herbal therapy is broadly used in many countries as a treatment or as a preventive measure in the management of MetS risk factors, including blood glucose, blood pressure, and blood lipid levels. Herein, an attempt is made to evaluate the recent studies in the management of MetS with herbal alternatives, and to explore the possibility of their use as therapeutic treatments or supplements.

Abstract BACKGROUNDTo observe the correlation between serum apolipoprotein A-1(apoA-1) and inflammatory factors in patients with gouty arthritis. METHODSFrom February to September 2020, 97 patients with gout (gout group) and 70 healthy controls (control group) were selected as the study subjects, who were admitted to the outpatient department of Beijing lu he hospital affiliated to capital medical university.97 patients in the gout group were in the acute phase. Serum concentrations of apoA-1, NLRP3 inflammasome (NACHT-LRR-PYD protein 3,NLRP3), interleukin-1 (IL-1) and interleukin-9 (IL-9) were detected. The correlation of serum apoA-1 concentration, gout related inflammatory factors (NLRP3, IL-1 beta, IL-9) and other clinical and laboratory indicators was analyzed. RESULTSThe serum apoA-1 concentration in the gout group was lower than that in the control group (P<0.05). With the increase of serum uric acid level, serum apoA-1 concentration decreased(R2=-0.3160,P<0.05). Multiple linear analyses were performed to increase blood glucose, blood lipid, liver and kidney, etc, and the correlation remained(OR=-3.36,P<0.05). With the increase of serum IL-1 beta concentration, serum apoA-1 concentration decreased(R2=-0.3993,P<0.05). Multiple linear analyses were performed to increase blood glucose, blood lipid, liver and kidney, etc., and the correlation remained(OR=-2.95,P<0.05). CONCLUSIONSIn the acute stage of gout, as the serum uric acid level increases, the serum IL-1β concentration increases and the apoA-1 concentration gradually decreases, which may indicate that apoA-1 participates in the inflammatory response of gout to a certain extent.

The present study was conducted to show the long term effect of low glycemic index (GI) finger millet (Eleusine coracana) buns (GI = 36.57) on diabetics. This case control clinical trial was conducted on 30 type 2 diabetic subjects who were equally divided into 2 groups: experimental group (EG) and control group (CG). The EG patients were supplemented with four (200 g) finger millet flour buns for 60 days in comparison to control group without having any buns supplementation. All the samples supplemented to experimental group provided 23-25g of total dietary fibre per day. The metabolic parameters studied were serum glucose, serum cholesterol, serum LDL, serum HDL, serum triglycerides and serum VLDL. Blood glucose levels (both fasting and post prandial) and lipid profile (at fasting) of the subjects were assessed at baseline and after 60 days of supplementation. Significant reduction was recorded in fasting blood glucose (13.75%), post prandial blood glucose (14.43%), serum cholesterol (4.41%) and serum LDL (11.22%) and also a decrease in serum triglycerides (5.11%) and VLDL (4.74%). Serum HDL increased significantly by 14.98 per cent. Thus, it is concluded that finger millet as a low GI food product leads to modest improvement in long-term glycemic and lipidemic control in type 2 diabetics.

The study was aimed at exploring the application value of the CT image based on a filtered back projection (FBP) algorithm in the diagnosis of patients with diabetes complicated with tuberculosis and at analyzing the influence of dietary nursing on patients with diabetes complicated with tuberculosis. In this study, the FBP algorithm was used to optimize CT images to effectively obtain reconstructed ROI images. Then, the deviation from measurement values of reconstructed images at different pixel levels was analyzed. 138 patients with diabetes complicated with tuberculosis were selected as research subjects to compare the number of lung segments involved and the CT imaging manifestations at different fasting glucose levels. All patients were divided into the control group (routine drug treatment) and observation group (diet intervention on the basis of drug treatment) by random number table method, and the effect of different nursing methods on the improvement of patients’ clinical symptoms was discussed. The results showed that the distance measurement value decreased with the increase in pixel level, there was no significant difference in the number of lung segments involved in patients with different fasting glucose levels ( P > 0.05 ), and there were statistically significant differences in the incidence of segmental lobar shadow, bronchial air sign, wall-less cavity, thick-walled cavity, pulmonary multiple cavity, and bronchial tuberculosis in patients with different fasting glucose levels ( P < 0.05 ). Compared with the control group, 2 h postprandial blood glucose level in the observation group was significantly improved ( P < 0.05 ), there was a statistical significance in the number with reduced pleural effusion and the number with reduced tuberculosis foci in the two groups ( P < 0.05 ), and the level of hemoglobin in the observation group was 7.1 ± 1.26 , significantly lower than that in the control group ( 8.91 ± 2.03 , P < 0.05 ). It suggested that the changes of CT images based on the FBP reconstruction algorithm were correlated with fasting blood glucose level. Personalized diet nursing intervention can improve the clinical symptoms of patients, which provides a reference for the clinical diagnosis and treatment of patients with diabetes complicated with tuberculosis.

Objective. To investigate clinical benefit and safety of neoadjuvant chemotherapy (NAC) plus bevacizumab combined with total mesorectal excision (TME) in treating patients with BRAF-mutated locally advanced rectal cancer (LARC). Methods. This study included LARC patients with BRAF mutation admitted to the Oncology Department of Fujian Medical University Cancer Hospital, Fujian Cancer Hospital, between June 2013 and December 2018. Patients in the control group received a standard treatment regimen of TME combined with NAC ( n = 45 ), and patients in the observation group received NAC plus bevacizumab combined with TME ( n = 55 ). The short-term clinical efficacy of the two groups after NAC treatment was observed and compared, including differences in the pathological downstaging rate. The incidence of perioperative complications and adverse reactions during neoadjuvant therapy was compared to evaluate the safety of the treatment. Besides, the relapse-free survival (RFS) and overall survival (OS) of patients were analyzed to evaluate the long-term clinical benefit of the treatment. Results. Compared with the control group, the ypT staging rate ( p = 0.014 ) in the observation group was markedly lower. In addition, patients in the observation group had a prominently lower overall incidence of complications ( p < 0.001 ) during the perioperative period and a remarkably lower incidence of leukopenia ( p = 0.037 ) during neoadjuvant therapy. In terms of long-term clinical benefit, the RFS of patients in the observation group was evidently longer ( p = 0.037 ) than that in the control group. Conclusion. Compared with TME plus NAC treatment, the short-term and long-term clinical benefits are higher and safety is more favorable of NAC plus bevacizumab combined with TME in treating LARC patients.
