The work shows that the medieval Western European city was characterized by an extremely low sanitary condition, manifested in the absence of a sewerage system and water supply, in the absence of street paving, unsanitary conditions of houses and streets. The extremely low level of personal hygiene of the inhabitants of the medieval city, a high level of infant mortality, widespread alcoholism, and low life expectancy are shown. It was found that one of the leading reasons for the low sanitary state of medieval cities and the level of personal hygiene was the unbiblical teaching of the medieval church about the immortality of the soul, according to which the flesh needed constant mortification and neglect, and caring for it and the comfort of dwellings was considered a mortal sin.

Lung cancer is the leading cause of mortality from malignant tumors all over the world. Since most patients at the time of diagnosis already have stage III-IV of the disease, the search for new effective treatment strategies for advanced NSCLC is the most important problem of modern oncology. The results of the study of the anti-PD1 monoclonal antibody pembrolizumab were a real breakthrough in the treatment of NSCLC. In the KEYN0TE-001 study, the expression of PD-L1 on tumor cells was validated as a predictive biomarker of the drug's efficiency. Pembrolizumab demonstrated the possibility of achieving long-term objective responses, and a 4-year 0S with all histological types in the subgroup of pre-treated patients with PD-L1 expression> 50% was 24.8% and 15.6% in the PD-L1> 1% group. In a phase 2/3 randomized study KEYN0TE-10 in the 2nd line treatment of NSCLC with PD-L1 expression > 1% pembrolizumab significantly increased life expectancy compared to docetaxel and confirmed the possibility of longterm duration of objective responses, even after cessation of treatment. Then the focus of research shifted to the 1st line of treatment. About 30% of patients with NSCLC have a high level of PD-L1 expression on tumor cells and demonstrate the most impressive response to pembrolizumab therapy. A randomized phase 3 study KEYN0TE-024 compared the effectiveness of pembrolizumab monotherapy with a standard platinum combination in patients with advanced NSCLC with a high level of PD-L1 expression without EGFR mutations or ALK translocation. Compared with the platinum doublet the administration of pembrolizumab significantly increased all estimated parameters, including the median of progression-free survival (mPFS was 10.3 months versus 6 months; HR = 0.50; 95% CI 0.37-0.68, p < 0.001), the objective response rate (ORR 44.8% versus 27.8%), duration of response (in the pembrolizumab arm the median was not reached, in the chemotherapy (CT) group - 6.3 months). Despite the approved crossover, the use of pembrolizumab in the 1st line of treatment more than doubled the life expectancy of NSCLC patients with high PD-L1 expression as compared to CT: the median overall survival (OS) was 30.0 months versus 14.2 months (HR = 0.63, p = 0.002), 1-year OS 70.3% versus 54.8%; 2-year OS - 51.5% versus 34.5%. The remaining population to study were untreated patients with any level of PD-L1 expression. A randomized phase 3 study KEYNOTE-189 evaluated the effectiveness of adding pembrolizumab to the platinum combination in the 1st line treatment of non-squamous NSCLC without EGFR and ALK mutations with any PD-L1 expression. The addition of pembrolizumab to the standard 1st line CT significantly increased all estimated efficacy indicators including OS, PFS and ORR. After a median follow-up of 10.5 months the median OS in the pembrolizumab combination group was not reached and in CT group was 11.3 months. The estimated 12-months survival was 69.2% and 49.4% respectively (HR = 0.49; 95% CI 0.38-0,64; p <0.001). The median PFS was 8.8 months versus 4.9 months, alive 1 year without progression 34.1% and 17.3% of patients respectively (HR = 0.52; p <0.001). The ORR in the group with pembrolizumab reached 47.6% versus 18.9% in CT group, moreover the tumor regressions were much longer. Finally a randomized 3-phase study KEYN0TE-407 evaluated the effectiveness of adding pembrolizumab to 1st-line CT of NSCLC with squamous histology with any PD-L1 expression. As the first analysis showed, the addition of permboli-zumab significantly increased OS of patients with squamous NSCLC, median OS 15.9 months versus 11.3 months in the groups of pembrolizumab + CT and placebo + CT respectively (HR = 0.64; 95% CI 0,49-0.95; p = 0.0006), median PFS 6.4 months and 4.8 months respectively (HR = 0.56; 95% CI 0.450.70; p <0, 0001) and OrR 57.9% versus 38.4%, the median response duration 7.7 months versus 4.8 months. Thus, the convincing advantages of using pembrolizumab in 1st line therapy were demonstrated in 3 randomized phase 3 studies: in monotherapy of NSCLC of any histological subtype with high PD-L1 expression, and in combination with CT in squamous and non-squamous hystologies regardless of the level of PD-L1 expression.

Since ancient times, the need for healthy water has resulted in the development of various kinds of water supply systems. From early history, civilizations have developed water purification devices and treatment methods. The necessity for fresh water has influenced individual lives as well as communities and societies. During the last two hundred years, intensive and effective efforts have been made internationally for sufficient water quantity and quality. At the same time, human life expectancy has increased all over the globe at unprecedented rates. The present work represents an effort to sketch out how water purity and life expectancy have entangled, thus influencing one another. Water properties and characteristics have directly affected life quality and longevity. The dramatic increase in life expectancy has been, indisputably, affected by the improvement in water quality, but also in other concomitant factors, varying temporally and spatially in different parts of the world throughout the centuries. Water technologies and engineering have an unequivocal role on life expectancy. In some cases, they appear to have taken place earlier than the progress of modern medicine. Among these, improved sanitation, personal hygiene, progress in medicine, and better standards of economic living have played the greatest roles.

During the 1970s the Soviet Union experienced rising infant mortality rates and a corresponding levelling off of earlier increases in life expectancy. Several Western critics have misrepresented or exaggerated these statistics, suggesting that these trends indicate a general breakdown in the Soviet health care system as well as the failure of the Soviet form of socialism. This paper examines life expectancy and infant mortality data by Soviet republic, showing that rates are not uniform throughout the U.S.S.R. and in many cases compare favorably with those in Western European countries and the United States. It is suggested that the infant mortality problem in the U.S.S.R. is a temporary negative consequence of rapid progress in the areas of industrialization, employment of women, and socialization of child care. It is concluded that improvements in public health education, the quality of child care facilities, and the manufacture and distribution of infant formula will contribute to the rapid resolution of this problem.

Background: Inadequate sanitation has direct effect on health of individual, family, communities and nation as a whole. Objective: To assess the personal hygiene and sanitary condition of the Katahari Village Development Committee (VDC). Methods: The cross sectional study was done in Katahari VDC of Morang district. A total of 80 households were randomly selected from two wards of VDC. The data were collected by interview method using interview schedule. Data were entered in Excel sheet and analyzed on SPSS program. Results: Many respondents (61%) were unable to read and write, 33% involved in private job in various factory. Knowledge of sanitation was high (90%) but only 65% of them were using soap water for hand washing. Sixty percent had no toilet facilities. There was significant association between education and toilet facilities among community people. Land holding and type of family had no significant association with toilet facilities. Mortality is a critical measure of population’s health and public health systems. Infant mortality, for example, indicates quality of life, accessibility to primary healthcare and the overall health status of a country. Reduction in infant mortality shows improvement in the health status. No credible information about mortality in South Africa because the two previous censuses’ data from Statistics South Africa (StatsSA) were not reliable, this study makes attempt to bridge the gap in the lack of knowledge. Th is study uses South African General Household Survey (SAGHS) data, to fi nd the level and trend of mortality and their implications. Data for the years, 2012, 2013 and 2015, have been used. Demographic and statistical methods, including an evaluation of data quality using UN joint score, and construction of model life tables. The results indicated that the infant mortality rate (IMR) was 43 per 1000 in 2012, 36 per 1000 in 2013 and 21 per 1000 in 2015. Th is study further indicated that the general health status of South African population improved marginally from 2012 to 2015 because the life expectancy in creas ed by 7 years for the males, and by 8 years for females, between those years. The study results that SAGHS data are reliable, mortality is decreasing with increasing life expectancy. The study recommends that more proactive measures need to be put in place to improve the health status of the population, especially the children because the IMR is still quite high and creates concerns.

As a result of nature resources intensive use, most of ecosystems have beenconverted. Anthropogenic impact includes changes of forest stands structure andtheir spatial specificity in the forest area. Accordingly the sanitary state of Norwayspruce young forest stands can be affected by different risk impact factors ofmanagement. The aim of the research was to analyze the spruce Picea abies (L. )Karst. young forest stands sanitary condition depending on forest plots spatialspecificity and location in the forest areas. The data were collected in 4 regions ofLatvia in spruce young forest stands (1 - 40 years old). The research was conductedin young natural and artificial stands (pure – 44, mixed – 42). In total 502 sampleplots with a total area of 28250 mwere installed. The particular plot size (25, 50,100 and 200 m) were selected depending on the stand average tree height, whiletheir number depended on the forest stand area. A total area of investigated foreststands were 127. 5 hectares. Results showed that the expression of spatial specificsdepended on risk factors and their intensity, as well as the environmentalcharacteristics. Damages caused by abiotic risk factors at different forest standswere not the same regarding intensity, nature and volume, but more or less closelywere related to all site conditions. Spatial specificity of forest stands area (regularand irregular), as well as their location in the forest massif significantly affects thespruce young forests sanitary status (respectively p=0. 027 and p=0. 002). Differentrisk factors damage to forests, bordering with spruce or pine young growths,cutovers and various types of infrastructure, were identified as much moreimportant.

The paper analyzes the socio-economic and demographic indicators of life expectancy in the countries of the world. Methods of regression analysis and machine learning are used. Statistically significant indicators that affect life expectancy around the world have been identified. When analyzing the data using machine learning methods, 13 of the 14 analyzed indicators were statistically significant. Significant indicators, in addition to those selected in the regression analysis, were 3: the under-five infant mortality rate (per 1,000 live births), the Net Barter Terms of Trade Index (2000 = 100), and Imports of goods and services (in % of GDP) (in the regression analysis, only the infant death rate was significant). In addition, it should be noted that there is a significant decrease in the under-five infant mortality rate (per 1,000 live births) for the EU, CIS and South-East Asian countries compared to the border set in the study for all countries: 4.65 vs. 34.9, a decrease in the birth rate from 2.785 to 1.85, a sharp increase in exports of goods and services: from 23.17 to 80.59, a halving in imports of goods and services, a drop in population growth from 2.105 to 0.85. The performed statistical analysis strongly supports the use of machine learning methods in identifying statistically significant relationships between various indicators that characterize the development of countries, if there are gaps in the data.

Secularization is accelerating. From 1981 to 2007, more than two-thirds of the publics for which we have data became more religious, but then a major shift occurred: from 2007 to 2020, more than four-fifths of these publics became less religious. Up to 2007 the U.S. showed little change, but since then it showed the largest shift of any country away from religion and now ranks among the world’s least religious publics. One generally overlooked reason for accelerating secularization is that, for centuries, most religions encouraged pro-fertility norms that limit women to producing as many children as possible and discourage any sexual behavior not linked with reproduction. These norms were needed when facing high infant mortality and low life expectancy but now are rapidly giving way to individual-choice norms supporting gender equality and tolerance of divorce, abortion, and LGBTQ people. Pro-fertility norms are so strongly linked with religion that abandoning them undermines religiosity.

According to disease avoidance theory, selective pressures have shaped adaptive behaviours to avoid people who might transmit infections. Such behavioural immune defence strategies may have social and societal consequences. Attractiveness is perceived as a heuristic cue of good health, and the relative importance of attractiveness is predicted to increase during high disease threat. Here, we investigated whether politicians' attractiveness is more important for electoral success when disease threat is high, in an effort to replicate earlier findings from the USA. We performed a cross-sectional study of 484 members of the House of Commons from England and Wales. Publicly available sexiness ratings (median 5883 ratings/politician) were regressed on measures of disease burden, operationalized as infant mortality, life expectancy and self-rated health. Infant mortality in parliamentary constituencies did not significantly predict sexiness of elected members of parliament ( p  = 0.08), nor did life expectancy ( p  = 0.06), nor self-rated health ( p  = 0.55). Subsample analyses failed to provide further support for the hypothesis. In conclusion, an attractive leader effect was not amplified by disease threat in the UK and these results did not replicate those of earlier studies from the USA concerning the relationship between attractiveness, disease threat and voting preference.

Bangladesh is a country having high density of population in the world. Its fertility rate ranges from 4.1 to 5.49 and life expectancy is 66years while the total fertility rate of Asia is 2.2 and life expectancy 70 years. This descriptive type of cross sectional survey was carried out among 240 married women of reproductive age from July to December, 2012 in some villages of Keranigonj, Upazila, Dhaka. 73(31%) were practicing some methods of contraception, while 167 (69%) were not using it. OCP (Oral Contraceptive Pill) was the commonest method of contraception followed by Condoms 12(5%), Injectable 12(5%), Implant 12(5%) & Tubectomy 6(3%). None was found using IUCD and Traditional method ( withdrawal, rhythm method ) and emergency contraceptive method. The use of contraceptive was more common in grand multipara (p<0.01), >35 years old ladies (p<0.05). Non users of contraceptives in this study were 167 (69%) and the major reason for the non use was intention to have more children 53(31.46%) followed by pressure from the husband 21(12.35%), prohibition by the religion 18 (10.9%) and desire for son 17 (10.11%). Among the 73 contraceptive users 38 (52%) experienced side effects with the use of contraceptives. The commonest side effects were menstrual irregularities 17(23.8%) followed by change in body weight 8(11.19%). Frequency of contraceptive use was found comparatively low among rural married women despite high level of awareness. Desire for larger family, religious concerns and fear of side effects were the main factors responsible for non users. Religious scholars must play their role in clarifying many aspects regarding contraceptives.
