Rheumatoid arthritis (RA) is a chronic inflammatory and systemic auto immune disease, affecting people predominantly between the ages of 20-60 years with unpredictable course. About 1% of the world's population is afflicted by rheumatoid arthritis and is two to three times more common in women than men. This is a long lasting disease that can affect joints in any part of the body, most commonly the hands, wrists, and knees. Popularity of medicinal plants is increasing day by day due to side effects of allopathic medicines. Herbal medicinal plants have been used as major sources of cure of human diseases since time immemorial. Today, one fourth of world population depends on traditional medicine and 80% of the population relies on indigenous herbal medicinal plants. Even today most of the people lives in different developing countries depend on the plant derived medicines for the first line of primary health care because of least or no side effects.

Background: Fever, is known as pyrexia, may occur due to infection, inflammation, or any tissue damage and disease states. Normally, the infected or damaged tissue initiates the enhanced formation of pro-inflammatory mediators like cytokines which further increases the synthesis of prostaglandin E2 (PgE2) near the hypothalamic area and thereby trigger the hypothalamus to elevate the body temperature. Objective: Antipyretics are the agents which reduce the elevated body temperature. The most commonly used antipyretic agent, paracetamol, may be fatal due to its side effects. Methods: In this review paper, Chemical Abstracts, Google Scholar, PubMed, and Science Direct were the sources for the published article to collect information regarding antipyretic activity. Results: This review compiles the antipyretic plants that may be useful to treat fever due to various diseases. Conclusion: These medicinal plants could be good alternatives for traditional allopathic antipyretics.

The scope of Unani medicine is increasing day by day as people are becoming more interested in herbal and organic world. However, Unani (herbal) knowledge needs to be coupled with scientific documented research done to verify its efficacy. The aim of this paper is to provide updated knowledge on hypertension and its management in the Unani system of medicine. Hypertension is a major health hazard globally especially in developed countries. In the Unani system of medicine, there are mudirrat (diuretics) to reduce body fluids and minerals, and musakkin (relaxants) and munawwim (sedatives) are also advised by Unani physicians to reduce anxiety. Unani physician, Razi recommends venesection for this particular ailment. Unani physicians have given the concept of hypertension as “Imtila-ba-Hasb-ul-Auiya” and have said that this occurs due to sue-e-mizaj damwi and comes under the heading of Imtila. Literally ‘Imtala’ means gathering and fullness of the body with madda (fluids). To be precise, it means there is a buildup of normal or abnormal fluids in the body. Unani physicians were all aware of the concept of Zaghta e damwi (blood pressure). Systole as ‘Zaghta-e-Inqabazi’ and diastole as ‘Zaghta-e-Inbesati’ is well depicted in the Unani literature. Most of the world’s population, mainly in developing countries, use herbal medicines for primary health care because of their ancient tradition, culture, and minor side effects. Various kinds of research have been conducted regarding the hypotensive and antihypertensive therapeutic values of local medicinal plants in the recent past, and they have provided evidence for the antihypertensive effects of some of these plants.

Objective: Eugenia caryophyllus, Zingiber officinale and Nyctanthes arbor-tristis are medicinal plants; these plants are used traditionally from ancient year in the various herbal medicinal system such as Ayurvedic, Homeopathic and Siddha. The clove buds of Eugenia caryophyllus, rhizomes of Zingiber officinale and dried leaves of Nyctanthes arbor-tristis have anti-inflammatory analgesic activity. The present research was focus on the formulation of polyherbal cream and their evaluation by using various evaluation parameters of the present research are to formulate polyherbal cream and to evaluate the polyherbal cream. Methods: the slab method was used for the preparation of cream. Results: The evaluation parameters are coming under this heading physical parameter like color was slightly white green, the odor was characteristics, consistency was smooth and the state was semisolid. PH of the cream was 6.5; Spreadability was 7.4g. cm/sec time required for this test was 15 sec, Washability was easily washable, the cream was nonirritant, viscosity of the formulated cream was 39010 cps and no phase separation was observed during storage of polyherbal cream. Conclusion: This cream formulation was used in rheumatoid arthritis to reduce joint pains. This cream formulation was o/w type of emulsion; hence this formulation was easily washed with plane water after application.

Ayurvedic texts have mentioned hepatocellular jaundice as Kamala. Due to frequent intake of Pittakara Aahara (spicy and hot food), it leads to vitiation of Pitta Dosha and Virechana1 (purgation) is the first line of treatment for Pittadushti. Ayurvedic line of management Ac- cording to lolimbraj (Vaidya jivana)2 i.e., Nasya therapy and According to Chakradatta Anjana3 therapy in addi- tion with above mentioned take 3 gm Triphala Churna with Lukewarm water at bedtime for Koshta shuddhi. Nasya and Anjana both are successful in Kamala by removing toxic waste from the body, and by correcting Agni (digestive fire). In our ancient classics, single drugs along with compound drugs have been mentioned in Kamala. These drugs have Kamalahara properties. This results in better circulation and nourishment of the organs and the diseases will subside. Kamala is a disease caused by an overabundance of Mala Ranjak Pitta. Nasya cleanses and energies the tissues and organs of the head and neck. Shodhan Nasya is a form of Nasya that promotes secretions and removes toxins from the body. Devadali Phal Nasya is a form of Shodhan, particularly Rechna Nasya, that causes accumulated Mala Ranjak Pitta to be excreted through the nose. Nasya is referred to as "Nastah Pracchar- dan" by Charaka4. It means Nasya is shodhan karma, which explains Nasya's position at the systemic level and why sr. bilirubin levels fall. Dronpushpi is Doşakarma Kaphavātaśamaka, Pittasamśodhaka Property.It is having Katu, Lavaņa, Madhura Rasa. It is laxative, angmintic, stimulant and febrifuge. In today's era, most of the diseases are treated by allopathic drugs. The reliance on allopathic drugs for the treatment of most diseases is gradually increasing day by day due to their rapid effects and the immediate relief to patients. However, these drugs induce some serious side effects in patients, and in some cases, the patient may die. Thus, the interest of researchers is growing day by day towards medicinal plants for the treatment of diseases. It has also been viewed that some herbal plants have great therapeutic and pharmacological effects in the treatment of Alzheimer's disease. The initial studies, which have been carried out by different researchers, demonstrated very valuable results and enlighten a ray of hope for the treatment of Alzheimer’s disease which is associated with CNS disfunction. In this article, the author focused on those plants which are especially used in the treatment of Alzheimer's disease.

Acceptor of contraceptive injection increased from 11.7% to 27.8% since 2010-2013. Contraceptive injection of Depo Medroxy Progesterone Acetate (DMPA) is given every 3 months by intramuscular. The long-term use of 3-month contraceptive injetion for more than two years can cause side effects such as menstrual disorders and decreased libido due to accumulation of progesterone hormone in the body that suppressed estrogen effect. This study aimed to analyze the association in long-term use of 3-month contraceptive injetion with menstrual disorders and decreased libido in Puskesmas (Primary health care) Puri, Mojokerto Regency in 2016. The study was observational analytics with cross sectional approach with sample 73 contraceptive acceptors. The data were analyzed by using univariate, bivariate and multivariate analysis with logistic regression and multivariate general model. The results showed that the duration of use of 3-month contraceptive injetion was significant with side effect of menstrual disorders (PR=0.142; 95%CI: 0.040-0.502) and decreased libido (PR=0.275; 95%CI: 0.100-0.756). Conclusion: menstrual disorders and decreased libido associated with the duration of use of 3-month contraceptive injetion.

Modernization and acculturation of regional culture can affect community knowledge in the use of local resources as a source of treatment, including in emergencies that occur on a daily basis. Local resources need to be studied in detail regarding the benefits, methods of use and side effects. This study aims to explore community knowledge about local wisdom (ethnomedicine) in handling daily emergency problems in the people of Ternate. The research method uses descriptive with purposive sampling technique. The total sample was 40 people who experienced using local wisdom in handling daily emergencies. Data obtained through questionnaire, interview, observation and documentation. The results showed that there were 11 emergency problems occurred that dealt with using 108 herbs. From this herb, 22 types of medicinal plants were identified. The need for educative improvement on daily emergency problems that occur in the community and how the use of local wisdom as an alternative to the first treatment is done. Related agencies need to provide support for the preservation of family medicinal plants which in addition to this beauty are also saved many benefits.

Abstract Background Several studies have shown subcutaneous methotrexate (s/c MTX) to be superior to oral for treatment of rheumatoid arthritis. However s/c MTX is rarely started first line. Methods We audited the first 67 patients started on s/c MTX at diagnosis and compared to a previous cohort of treat-to-target patients started on oral MTX. Results There was a significant difference in adherence: for every 1 patient staying on oral, 1.5 patients stayed on s/c MTX. There was no difference in final numbers in remission, escalation to biologics, or time to remission. Nearly half the patients on oral MTX had switched to s/c at 1 year, either for side effects or inefficacy. Conclusion Adherence to s/c MTX was better than adherence to oral MTX in our study. The large number of switches from oral to s/c suggests that adherence is improved due to improved tolerability and reduced side effects of s/c MTX. Disclosures C. Bevington None. K. Achilleos None.
