Tingkat Ketergantungan dan Lama Perawatan Pasien Rawat Observasi di IGD

Ketidakseimbangan jumlah pasien dan ketersediaan ruang rawat dapat berakibat pada memanjangnya masa rawat pasien di Instalasi Gawat Darurat (IGD). Hal tersebut berdampak pada bertambahnya aktivitas dan beban kerja perawat selama pasien menunggu ketersediaan ruang rawat inap di ruang IGD. Berdasarkan situasi tersebut, penting untuk mengidentifikasi tingkat ketergantungan pasien selama periode rawat observasi dan rata-rata waktu yang digunakan untuk perawatan pasien rawat observasi selama pasien menjadi tanggung jawab perawat IGD. Penelitian deskriptif dilakukan melalui observasi terhadap pasien yang menjalani rawat observasi di IGD dan intervensi keperawatan yang diterima, serta jumlah waktu yang dipergunakan dalam perawatan tersebut. Teknik consecutive sampling dipergunakan untuk menetapkan pasien sebagai sampel penelitian.dan sebanyak 74 pasien rawat observasi dilibatkan dalam penelitian ini. Data dianalisis secara deskriptif. Hasil penelitian menunjukkan bahwa sebaran tingkat ketergantungan pasien rawat observasi adalah ketergantungan minimal (37,84%), sedang (25,68%), agak berat (24,32%), dan maksimal (12,16%). Rata-rata waktu yang digunakan untuk perawatan pasien rawat observasi 19 menit/perawat/shift dan total waktu 3,72 jam perhari. Implikasi penelitian bagi rumah sakit yaitu perlu dilakukan evaluasi terhadap adanya peningkatan beban kerja perawat IGD dan perencanaan kebutuhan ruang transientdi luar IGD.Kata kunci:Beban kerja perawat, gawat darurat, ketergantungan pasienAbstractAn imbalances between number of patients beimg admitted in Emergency Department (ED) and the availability of beds in it’s department have an impact on the length of stay of patient who actually have to be discharged from ED. This problem will affect to the additional nurses activity and the workload, as these pastient should wait to the availability of rooms for the inpatient care in the transient rooms. Based on this situation, it is important to identify the level of dependency of patient during the observation period in transient rooms, and the time consumtion for delivering nursing care service, since it’s being the responsibility of ED nurses. This data was collected through observations and analyzed with descriptive analysis (percentage). Consecutive sampling technique was administered 74 patients who receives nusing service in this transient room involved in this study. The results of this study obtained that distribution of level dependency of observed patients was minimal dependency (37.84%), medium (25.68%), (24.32%) is higher dependency, and maximal dependency (12.16%). The average time consumtion for nursing care delivery is 19 minutes/nurses/shift, and the total time consumtion is 3,72 hours/day. The implication of this study is for the Hospital need to evaluate the additional workload of the ED nurses, and need to plan the availability of transient room out side the ED.Key words:Nursing workload, emergency, patient dependency

Changes in patients' care mandate that nursing care be amenable to change. With the advent of new technology, systems of care delivery are evolving. Blood transfusion practices that were once considered standards are being reexamined and redefined. Nurses, in their roles as patients' advocates and educators, must be cognizant of changes in practice and be prepared to deliver competent care. The effectiveness of techniques of bloodless medicine and surgery originally designed to serve the needs of Jehovah's Witnesses is accepted today, and other groups of patients are choosing this type of care. Furthermore, as blood conservation strategies are refined and perfected, they most likely will be used by a greater number of patients undergoing cardiac surgery. Specific guidelines have been developed for point-of-care service for patients undergoing bloodless cardiac surgery. Nursing care throughout the hospital stay, from preadmission to discharge, is tailored to meet these patients' emotional and physical needs. Quality comprehensive nursing care is required to make bloodless medicine and surgery programs successful.

The Parents' satisfaction of premature babies is very important as one of the basic steps in determining the quality of nursing service. This study aims to identify parental satisfaction in the perinatology. Consecutive sampling technique was conducted to select 59 parents of premature babies as research respondents. Instrument in this study uses EMPATHIC N. The data was analyzed with Pearson test. The results showed that the average of premature babies’ care score was 161.93 and the average of parents’ satisfaction score was 280.07. Nursing care should be evaluated to improve the performance of nurses and the quality of care of premature babies and parents’ satisfaction.

ABSTRACT Background: The patient’s statement of the nurse’s poor and needy performance reflects dissatisfaction. Communities need patient-focused services, and the success of nursing care is determined mainly by choice of nursing care delivery models. This study aimed to determine implementation of integrated nursing models toward patient satisfaction in Mitra Medika Amplas Hospital, Medan, North Sumatra. Subject dan Method: This was a quasi-experiment with two group comparison conducted at Mitra Medika Amplas Hospital, Medan, Indonesia. A sample of 77 patients was selected by consecutive sampling. The data were collected by questionnare and analyzed by t-test. Result: The results showed that the level of satisfaction in the intervention group stated that 60 people were satisfied (78%) while the control group stated that 4 people were satisfied (5%). It was statistically significant (p<0,05). Conclusion: The level of patient satisfaction based on the dimensions of tangible quality, empathy, reliability, responsiveness and assurance was higher in the intervention group than in the control group. <p class="Normal1" align="center"><strong>Abstract</strong></p><p class="Normal1" align="center"><strong> </strong></p><p>Diabetes Mellitus (DM) is a chronic condition which requires various kinds of lifetime treatment which cause grief. Grief has stages which can be passed though normally with the attainment of acceptance stage. Acceptance attainment renders a description maintaining that grief in its process is undergone normally through uncomplicated grief reaction and emotional responsiveness. A diabetics who manages to attain the acceptance stage will have a better glycemic control, while a diabetics with a low acceptance stage may experience an ineffective coping, improper self-treatment, and distress increase, all of which end up in a bad glycemic control which causes physical complications. This suggests that a description of s’ acceptance stage is important to identify. Accordingly, the aim of this research is to identify and analyze the description of diabetics’ acceptance of DM diagnoses. The method utilized is descriptive-observational, the data collected using the ‘Acceptance of Disease and Impairments Questionnaire’ (ADIQ). The analysis employed in this research is a descriptive analysis. The sampling technique used is consecutive sampling. The research result shows that, based on the diagram of ADIQ administered on 20 s, there are 30% found to be in the stage of acceptance, while there exist 70% to be in the stage of not yet attaining acceptance, which comprises denial (20%), resistance (10%), and sorrow (40%). Failure in attaining the acceptance stage is caused by the s’ still being in the states of denial, resistance, and sorrow. Based on the research result, it can be concluded that the s dominantly stay at the stage of not yet attaining acceptance. The diabetics who have attained acceptance may still be exposed to the risk of undergoing a maladaptive response which may trigger depression, anxiety, worsened physical health, self-treatment discontinuation, powerlessness, low self-esteem, social isolation, even suicidal drive. This is all caused by grief which constitutes a fluctuate condition where every diabetes can contribute to describing his or her condition at every stage.</p><p> </p><strong><em>Keywords</em></strong><em>: stages of grief, acceptance, denial, resistance, sorrow</em>

Lung tuberculosis (TB) is one of the diseases that need special attention in Indonesia; this is because Indonesia is ranked third after the world of India and China with patients about 10% of the total number of TB patients in the world. This research aims to identify the picture of behavior parents in the provision of drugs in children with TB in Children Polyclinic RSUD. Dr. Pirngadi Medan. This study uses descriptive design with the number of samples of 42 respondents. Samples were taken using consecutive sampling technique. The data were collected using a knowledge, attitude, and action questionnaire. The data collected were analyzed using descriptive statistics, then the result of data analysis is presented in frequency distribution table and percentage. The result of this research indicates that the majority of respondents have good knowledge as much as 85.7%, and the majority of respondents have good attitude of 76.2% while the majority of respondents have an adequate action of 52.3%. The results of this study can be used as information and reference for child and community nurses in providing nursing care, especially health education and monitoring of treatment for patients with pulmonary TB in undergoing treatment.

Perawat mempunyai kontribusi yang sangat penting dalam melaksanakan pelayanan kesehatan pada umumnya dan keperawatan pada khususnya. Kondisi pelaksanaan suhan keperawatandan pelaksanaan dokumentasi keperawatan di Indonesia masih perlu ditingkatkan. Dalam pelaksanaandokumentasi keperawatan perawat srring mengeluh terhadap dokumentasi yang memakan waktu, terlalu banyak memerlukan tulisan dan banyaj perawat belum sepenuhnya faham menuliskannya. Sistem informasi yang berbasis computer sangat praktis karena mampu menyimpan data yang sangat banyak dalam sebuah kotak kecil/hard disk yang berukurab 15x10x5 cm. sistem informasi juga dirancang untuk mengikuti era globalisasi sehingga perawat di Indonesia tidak tertinggal dengan perawat yang di luar negeri. Sistem informasi manajemen berbasis computer sangat banyak keuntungannya tapi perlu dipersiapkan dengan matang sehingga diperoleh sistem informasi yang efektif, handal, akurat dan terpercaya. Nurses have important contribution to deliver health care services in general and nursing care service in particular. Nursing care delivery system includes nursing documentation in Indonesia needs to be improved. In implementing the nursing documentation, many nurses complained of insufficient time, too wordy, and lack of understanding in writing the documentation. He proposed solution is to implement computer based management information system. Computer based management Information System (MIS) is very practical since it has capability to keep bunches of data in a small box/hard disk of 15x10x5 cm. computer based MIS is designed to empower Indonesian nurses in documentation, so they will not be left behind by their colleagues from other countries. Computer based MIS has many advantages, how ever it’s still to be comprehensively prepared in order to produce an effective, accurate, achievable, manageable and trusted information system.

Abstrack: Nurse’s performance is a quality and quantity work result accomplished by a nurse in fulfilling her duties according to her responsibilities in providing nursing care to the patient overall. There is some factors that influence the nurse performance,one of the factor is leadership style. The research purpose is to identify the leadership style og head room to the nurse and nurse performance to the patient and to analyze whether there is any relation of leadership style of head room with nurse performance in inpatient room of GMIM Pancaran Kasih Hospital Manado. The research design used is descriptive analysis method through cross-sectional. The sample consists of 61 respondents obtained by using non random sampling technique with purposive sampling method. The result of the research significant result was obtained at ρ-value=1,000 >(0,05). The condusion is that there is no relation between leadership style of head room with nurse performance in inpatient room of GMIM Pancaran Kasih Hospital Manado.Keywords: Head office leadership style, nurse performanceAbstrak: Kinerja perawat adalah hasil kerja secara kualitas dan kuantitas yang dicapai oleh seorang perawat dalam melaksanakan tugasnya sesuai dengan tanggung jawabnya dalam memberikan asuhan keperawatan kepada pasien secara menyeluruh. Desain penelitian yang digunakan Deskriptif Analitik dengan pendekatan Cross Sectional. Sampel berjumlah 61 responden yang didapat dengan menggunakan tehnik non random sampling dengan metode purposive sampling. Hasil penelitian didapatkan nilai signifikan p-value = 1,000 > (0,05) Kesimpulan bahwa tidak terdapat hubungan antara gaya kepemimpinan kepala ruangan dengan kinerja perawat diruang rawat inap RSU GMIM Panacaran Kasih Manado.Kata Kunci : Gaya Kepemimpinan Kepala Ruangan, Kinerja Perawat

Gastric emptying in premature infants is slower than aterm infant aterm for 12 hours and some cases reach 22-36 hours. This slow emptying of the stomach causes to increase the volume of gastric residual and raising the risk for infants to experience gastroesophageal reflux, vomiting, aspiration and necrotizing etherocolitis. This study aims to analyze the effectiveness of positioning on gastric emptying show from gastric residual in premature infants in the Perinatology  RSUPN Cipto Mangunkusumo. The research design used in this research is quasi experiment on 20 premature infants with consecutive sampling technique. The data analysis used Wilcoxon test for gastric residual. The study showed a difference in average abdominal distensionand  gastric residual  in right lateral position, left lateral positions and pronation is significant so that it can be concluded effective positioning to accelerate gastric emptying. Based on these results then the positioning can be recommended to be an alternative nursing care in overcoming the problem of gastric emptying in premature infants.

Currently, cancer is the top ten cause of child mortality in Indonesia. Chemotherapy is an effective intervention to treat cancer children, however, has side effects, including mucositis. The prevalence of mucositis in cancer children is estimated to reach 45−80%, which has implications for the children’s’ quality of life. The purpose of this study was to identify the occurrence of mucositis in cancer children who received chemotherapy treatment. The study was conducted at Dr. RSUP Hasan Sadikin Bandung. This research method was descriptive quantitative. The consecutive sampling technique was used to determine the respondent as the study sample so that 60 respondents were obtained. The process of collecting data used the Oral Assessment Guide (OAG) study instrument in the form of a checklist sheet. Data were analyzed using frequency distribution and percentage. The results showed that almost all cancer children with chemotherapy treatment experienced mucositis which is 53 people (88.3%) and a small proportion of 7 people (11.7%) did not experience mucositis. Almost all cancer children who get chemotherapy treatment experience mucositis. Nurses as health workers who are most often with patients should be able to improve nursing care for cancer children who receive chemotherapy treatment in minimizing the occurrence of mucositis by conducting routine mucositis assessment and oral care to improve the quality of life in cancer children

Abstract Background Internationally increasing demand for emergency care is driving innovation within emergency services. The Alternative Pre-Hospital Pathway (APP) Team is one such Community Emergency Medicine (CEM) initiative developed in Cork, Ireland to target low acuity emergency calls. In this paper the inception of the APP Team is described, and an observational descriptive analysis of the APP Team’s service data presented for the first 12 months of operation. The aim of this study is to describe and analyse the APP team service. Methods The APP Team, consisting of a Specialist Registrar (SpR) in Emergency Medicine (EM) and an Emergency Medical Technician (EMT) based in Cork, covers a mixed urban and rural population of approximately 300,000 people located within a 40-min drive time of Cork University Hospital. The team are dispatched to low acuity 112/999 calls, aiming to provide definitive care or referring patients to the appropriate community or specialist service. A retrospective analysis was performed of the team’s first 12 months of operation using the prospectively maintained service database. Results Two thousand and one patients were attended to with a 67.8% non-conveyance rate. The median age was 62 years, with 33.0% of patients aged over 75 years. For patients over 75 years, the non-conveyance rate was 62.0%. The average number of patients treated per shift was 7. Medical complaints (319), falls (194), drug and alcohol related presentations (193), urological (131), and respiratory complaints (119) were the most common presentations. Conclusion Increased demand for emergency care and an aging population is necessitating a re-design of traditional models of emergency care delivery. We describe the Alternative Pre-Hospital Pathway service, delivered by an EMT and an Emergency Medicine SpR responding to low acuity calls. This service achieved a 68% non-conveyance rate; our data demonstrates that a community emergency medicine outreach team in collaboration with the National Ambulance Service offering Alternative Pre-Hospital Pathways is an effective model for reducing conveyances to hospital.
