Breastfeeding is known as the only one form of human feeding that formed during the biological evolution. However, when breastfeeding is impossible, the only solution is to use modern adapted milk formulas to nourish infants. One of the most challenging issues is the adaptation of the formula protein quantity and quality to those in breast milk. Reducing the protein content in the adapted formula "Malutka Premium" prevents protein overload of the immature metabolic system of the child. Fats also play an important role in the nutrition of children as they perform two main functions in the body: they serve as structural components of biological cell membranes and energy material. The fatty component of "Malutka Premium" adapted formula is represented by 50% vegetable oils, which provides the required level of polyunsaturated fatty acids. The carbohydrate component of the adapted formula "Malutka Premium1" is represented by lactose; the adapted formula "Malutka Premium 2" also contains dextrinmaltose (30%). Prebiotics oligosaccharides and five most important nucleotides are added to the composition of "Malutka Premium" that enables to normalize the composition of the intestinal microflora and to intensify the digestive processes. Clinical observations of children receiving formulas with oligosaccharides and nucleotides have shown their high efficacy. Children gained weight better and were found as less likely to have functional digestive disorders. Introducing "Malutka premium with the addition of cereals" formulas to the child's diet enables to choose the most appropriate formula taking into account the peculiarities of the child's digestion. Feeding infants with domestic milk formula ensures the balanced intake of all necessary substances required in accordance with the age and allows to parents and paediatricians solve many problems in the nutrition of both healthy children and children with special nutritional needs.

Binge-eating disorder, recently accepted as a diagnostic category, is differentiated from bulimia nervosa in that the former shows the presence of binge-eating episodes and the absence of compensatory behavior. Epigenetics is a conjunct of mechanisms (like DNA methylation) that regulate gene expression, which are dependent on environmental changes. Analysis of DNA methylation in eating disorders shows that it is reduced. The present study aimed to analyze the genome-wide DNA methylation differences between individuals diagnosed with BED and BN. A total of 46 individuals were analyzed using the Infinium Methylation EPIC array. We found 11 differentially methylated sites between BED- and BN-diagnosed individuals, with genome-wide significance. Most of the associations were found in genes related to metabolic processes (ST3GAL4, PRKAG2, and FRK), which are hypomethylated genes in BED. Cg04781532, located in the body of the PRKAG2 gene (protein kinase AMP-activated non-catalytic subunit gamma 2), was hypomethylated in individuals with BED. Agonists of PRKAG2, which is the subunit of AMPK (AMP-activated protein kinase), are proposed to treat obesity, BED, and BN. The present study contributes important insights into the effect that BED could have on PRKAG2 activation.

Abstracts. Venereology and Dermatology. Hugo Bauer (D. Z. Bd. 57, H. 4, 29), aiming to study the secretion of myosalvarsan'a, took under observation 4 syphilitics treated with injections of the drug. Excretion of myosalvarsan'a, as well as its absorption, is similar to 'neo' and sulfoxylsalvarsan'y. Most of the arsenobenzol'a introduced into the body is released within a "short time" after injection; four weeks after the cessation of treatment, the body is freed from the maximum amount of introduced As. The author believes that the clinical observations, on the basis of which it is argued that if certain intervals are observed between courses, the cumulative action of As does not occur, are completely fair. The author gives a detailed technique for determining As in selections and supplies the article with visual tables and curves of his own observations.

Antibody titers to diphtheria, pertussis, tetanus, and poliomyelitis (types I to III) were measured in previously vaccinated children with acute lymphocytic leukemia in remission after cessation of therapy. The response to revaccination one year after therapy was stopped was also studied. The patients' antibody titers were compared with those of healthy children, matched for age and sex. Two groups of patients were studied: one group (group A, N = 30) was given two drugs (6-mercaptopurine, methotrexate); the other group (group B, N= 19) was given three drugs (6-mercaptopurine, methotrexate, and cyclophosphamide) for maintenance treatment. In general, the patients' antibody titers were lower than those of healthy children, but in most patients they were still at levels considered to be protective. No significant differences in antibody levels between the two patient groups were found. A spontaneous rise in antibody titers in the first year after termination of therapy was not observed. After revaccination the rise in antibody titers was correlated with preexisting antibody titers in the same way in patients as in healthy children, and the antibody titers in patients and in healthy control subjects were on roughly the same level.

Introduction Ventricular septal defect (VSD) is an opening in the interventricular septum. 30-50% of patients with congenital heart disease have VSD. Objective The aim of the study was to determine the dependence of the left ventricular diastolic dimension (LVD), left ventricular systolic dimension (LVS), shortening fraction (SF), left atrium (LA), pulmonary artery truncus (TPA) on the body surface and compare their values among experimental, control and a group of healthy children. Values of maximal systolic gradient pressure (Pvsd) of VSD were compared with children from one experimental and control group. Method Children were divided into three groups: experimental (32 children with VSD that were to go to surgery), control (20 children with VSD who did not require surgery) and 40 healthy children. Measurements of LVD, LVS, SF, LA, TPA were performed in accordance to recommendations of the American Echocardiographic Association. The value of Pvsd was calculated from the maximal flow velocity (V) in VSD using the following formula: Pvsd=4xV? (mm Hg). Results For children from the experimental group, the relationship between the body surface and the variability of the LVD was explained with 56.85%, LVS with 66.15%, SF with 4.9%, TPA with 58.92%. For children from the control group, the relationship between the body surface and the variability of LVD was explained with 88.8%, LVS with 72.5%, SF with 0.42%, PA with 58.92%. For healthy children, the relationship between the body surface and the variabilitiy of the LVD was explained with 88.8%, LVS with 88.78%, SF with 5.25% and PA with 84.75%. There was a significant statistical difference between average values of Pvsd in the experimental and control group (p<0.02). Conclusion The presence of the large VSD has an influence on the enlargement of LVD, LVS, SF, TPA. The enlargement of the size of the pulmonary artery depends on the presence of VSD and there is a direct variation in the magnitude of the shunt. There is a relationship and significant dependence of the LVS and LVD on the body surface. There is no statistically significant dependence between SF and body surface.

Background: Osteoradionecrosis (ORN) post dental extraction is post dentalextraction complication with post radiation cancer theraphy. Objective : to present rare case, ORN post dental extraction with post radiation Ca nasopharing therapy 3 years ago.Case Management: A 54 years old patient reported to the dental out-patient department with a chief complaint of pus discharge from right buccal since post dental extraction 6 months ago. He gave a history of a nasopharing Carcinoma with histopatology as squamous cell carcinoms 3 years ago and radio therapy but no surgery. Intraoral examination, exposed necrotic bone found from right lower retromolar area 46 with pus discharge. Radiographic view was likely squester. Local surgical debridement and the sequestrectomy was undertaken with general anaesthesia. Antibiotic injection treatment was ceftriaxon 2x 1gram, infus metronidazol 3x500 mg and ketorolac 3x1 ampul, the patient was treated for 3 days and educated to maintain his oral hygiene with povidone iodine gargle.Discussion: Osteoradionecrosis (ORN) is late effect of radiation therapy thatresults in irreversible tissue death, which is clinically observed as bony exposure for more than 3 months duration. The mandible is affected more often than the maxilla or any other bones of head and neck region. The incidence of ORN in the mandible is reported to be between 2% and 22% and most often affects the body of the mandible. Ideal time is one year minimal post radiotherapy to get maximal vascularization for optimal healing. But immunity factor and radiation doses can trigger emergense ORN.Conclusion: Need time consideration, clinic analysis and pathologys before doing dental extraction for post radiotherapy cancer cases to prevent ORN.

Obesity is a medical condition arises as a result of an imbalance between lipolysis and lipogenesis which leads to accumulation of excess fat. Obesity is linked with major health complications such as diabetes, cardiac disorder, cancer, hypertension, sleep apnoea, etc. Weight loss through medication and healthy food balance can prevent obesity and associated disorders. Though, diet based therapy and drugs have short term effect with lacunae. At present, no reliable and established oral supplements are available to take care of obesity. Nanotechnology is a briskly emerging area of science and technology for food modulation at molecular and atomic level. The unique size and superior properties of nanomaterial have huge application in biology and medicine for food and drug development. Nanotechnology is playing an important role in the food industry for unindustrialized improved food, uptake, absorption, and bioavailability of nutrients to the body. Balanced or better-modified nutrients food, restricted calorie measures and commercial availability is geared through Nanoscience for control of obesity. Nanoparticle Targeted drug therapy, particularly for adipose tissue, may provide a new generation formulation for therapeutics of obesity.

In fulfillment of these needs sometimes human forget to maintain their health, good attitude so that the position in work is not ergonomic which can lead to problems when carrying out activities. Lower back pain is a clinical symptom that is characterized by pain or a discomfort feeling in lower back area. In the United States, the incidence of lower back pain region in the past one year is 15% -20%. This study aims to see whether there were differences in giving of Mc Kenzie and Core Stability Exercise toward Functional Activity of Patients with Lower Back Pain. This study is quasi experiment with a sample of patients with lower back pain, with purposive sampling technique. The results obtained are p-value ? <(0,000 <0,05), it means that there is a difference in giving of Mc Kenzie Exercise toward the increase of Functional Activity of Patients with Lower Back Pain, that uses body movements that is directed to extension, to provide strengthening and reflecting on the extensor and flexor muscles of the lumbar joint, while the Core Stability Exercise has the ability to regulate the position and movement in the central region of the body, and it can be concluded that there is differences in Giving of Mc Kenzie Exercise toward the increase of Functional Activity of Patients with Lower Back Pain. It is recommended for patients who experience lower back pain so that keep always repeat the exercises that suggested by physiotherapist, for further research so that choose more samples.

In this paper it is presented a comparative theoretical study - performed by finite element analysis (FEA), of the tension state that appears at the level of the spine, as well as in the areas adjacent to it, under the following conditions: a) the existence of a comminutive fracture at the level of the vertebra T11; b) of the external immobilization of the body through a Lombax orthesis (LO) or of the internal immobilization of the column by means of a spinal fixator (SF); c) of performing some basic flexion or extension movements. In this study, both flexural force and extension force varied on three levels. The results of the comparative theoretical study were supplemented with the clinical observations obtained from a number of 52 patients who had suffered comminutive fractures in the T11 vertebra and who were treated - by external immobilization, in the Lombax orthesis or were surgically treated - by applying a trapped spinal fixator on the T10-T12 vertebrae. The study had demonstrated that, by immobilizing the body in the Lombax orthesis, following the application of the flexion or extension movement, the mobility of the body is low, the bone fragments move less distances by about 12% against to their displacement under the condition of immobilizing the column by a spinal fixator. Although, in the latter case, the displacements of the bone fragments are greater, the elastic behaviour of the spinal fixator determines the returning of the fragments and of the spine to the initial position as well as the distance between the vertebrae. Clinical investigations on patients treated by external immobilization of the body in the Lombax orthesis show that the values of the local kyphosis angle (LKA) are about 30% lower than the values of the same parameter obtained under the surgical treatment.

ObjectiveWe report a case of a 58-year-old gentleman who was hospitalised intermittently for one year due to treatment resistant schizophrenia. Prior to hospitalisation he had been prescribed standard antipsychotics for decades without full resolution of positive psychotic symptoms. During his final admission lasting six months he was guarded, suspicious, irritable, constantly paced the corridor and displayed thought block and paranoid persecutory delusions. He would not enter the assessment room or allow any blood or ECG monitoring, however, he was compliant with oral medication. He was successfully treated with high dose olanzapine (40mg/day) and was discharged to the community. The aim of this study is to bring awareness and add to the body of evidence for the use of high-dose olanzapine in patients with treatment resistant schizophrenia in whom a trial of clozapine is not possible.Case reportThe patient gave written consent for this case report to be written and presented. An extensive literature review was performed and key papers were identified. Discussion focuses on the key areas in the literature.DiscussionThis case demonstrates that high-dose olanzapine can be used effectively as an alternative to clozapine in treatment resistant schizophrenia.ConclusionThis case highlights the need for further evaluation of high-dose olanzapine as an alternative to clozapine in patients with treatment-resistant schizophrenia.

Összefoglaló. Bevezetés: A kutatócsoport 99 fő, cerebralis paresisben (CP) szenvedő gyermek (8–18 éves) önállóan közölt életminőségét értékelte, és az eredményeket összehasonlította egy 237 fős kontrollcsoport adataival, amelyek hasonló életkorú, egészséges általános populációhoz tartozó gyermekektől származtak. Célkitűzés: A kutatás célja annak megismerése volt, hogy a CP-ben szenvedő gyermekek hogyan vélekednek egészségi állapotukról és társadalmi helyzetükről. Módszer: Életminőség-kérdőív alkalmazása. Betegségspecifikus és társadalmi-demográfiai mutatók mérése, kiértékelése. Eredmények: Az CP-ben szenvedő gyermekek és szüleik az egészséggel kapcsolatos életminőséget rosszabbnak ítélték meg, mint társaik. Eredményeink azt mutatják, hogy a női nem, a rosszabb motoros funkció és a komorbiditások (epilepszia, incontinentia és intellektuális károsodás) negatív hatású. A szülői vélemény alkalmas volt proxyjelentésként a korreláció mért erőssége miatt. Figyelemre méltó, hogy az agyi bénulás típusai közül az egyoldali spasticus CP-ben szenvedő gyermekek életminőség-értéke a legalacsonyabb. A válaszadók valószínűleg a test két oldala között lévő funkcionális különbséget érezték. A szellemi fogyatékosság a betegpopuláció több mint felénél fordult elő. Testvéreik között a mentális betegség 5,7-szer gyakoribb. A CP-s gyerekek családi környezete sokkal hátrányosabb volt, mint az egészséges gyermekeké. A kutatás eredményei alapján megállapítható, hogy a szülő alacsonyabb iskolai végzettsége és munkaerőpiaci inaktivitása, valamint az egyszülős család a CP-s gyerekeknél szignifikánsan magasabb arányban fordult elő, és ezek a tényezők negatív hatást gyakoroltak az életminőségre. Következtetés: A fogyatékkal élő gyermekek életminőségét a betegség és a szociodemográfiai környezet egyaránt befolyásolja. Orv Hetil. 2021; 162(7): 269–279. Summary. Introduction: Self-reported health-related quality of life (HRQoL) of 99 children (8–18 years) with cerebral palsy (CP) was assessed and compared with 237, age-matched healthy control children from the general population. Objective: The aim was to find out the opinions of children with CP about their health status and social condition. Method: Assessment of quality of life questionnaire was carried out. Measurements of disease-specific and sociodemographic variables were done. Results: Children with CP and their parents rated HRQoL poorer than their counterparts. Our results show that female sex, worse gross motor function and comorbidities (epilepsy, incontinence and intellectual impairment) had negative impact. The parental opinion was suitable as proxy report because of the measured strength of the correlation. Among the types of CP, interestingly, children with unilateral spastic CP had the poorest HRQoL. They were likely to feel a functional difference between the two sides of the body. Intellectual disability occurred in more than half of our patient population. Among their siblings, mental illness is 5.7 times more common. The family environment was much more disadvantageous than in the case of healthy children. As our study shows, lower education, inactive status in the labour market and single-parent family occurred at a much higher rate and worsened the quality of life.
