ABSTRAK Gout merupakan penyakit metabolik yang disebabkan oleh kelebihan kadar senyawa urat didalam tubuh, baik karena produksi berlebih, eliminasi yang kurang, atau peningkatan asupan purin. Salah satu terapi nonfarmakologik yang telah terbukti mampu mengatasi berbagai permasalahan kesehatan adalah akupresur. Akupresur yaitu tindakan pemberian tekanan ke titik khusus pada tubuh untuk mengurangi peningkatan kadar asam urat, menghasilkan relaksasi, dan mencegah atau mengurangi rasa mual. Tujuan pelaksanaan pengabdian kepada masyarakat ini untuk mendapatkan pengalaman dan kompetensi lebih dalam pemberian asuhan keperawatan komprehensif pemberian terapi Akupresure terhadap penurunan kadar asam urat. Adapun kegiatan yang dilakukan berupa penyuluhan menggunakan leaflet dan demonstrasi terapi akupresure. Terdapat penurunan kadar asam urat setelah dilakukan terapi akupresure pada penderita asam urat. Terapi akupresure sangat efektif dalam menurunkan kadar asam urat.                                                                                                Kata Kunci: Akupresure, Asam urat, Kadar asam urat  ABSTRACT Gout is a metabolic disease caused by excess levels of urate compounds in the body, either due to overproduction, lack of elimination, or increased intake of purines. One non-pharmacologic therapy that has been proven able to overcome various health problems is acupressure. Acupressure is the act of applying pressure to a special point on the body to reduce the increase in uric acid levels, produce relaxation, and prevent or reduce nausea. The purpose of the study was to gain more experience and competence in the provision of comprehensive nursing care in the provision of acupressure therapy for decreasing uric acid levels. The activities carried out in the form of counseling using leaflets and demonstrations of acupressure therapy. There was a decrease in uric acid levels after doing acupressure therapy in gout sufferers. Thus, acupressure therapy was very effective in reduces uric acid levels. Keywords: Acupressure, Gout, Uric Acid Levels

The article is devoted to the creation of a carbohydrate product with rosehip extract in the form of sweets (glucose Fudge) with a therapeutic and preventive effect aimed at restoring the body of people exposed to intense physical and mental stress and the effects of damaging environmental factors. As carbohydrates in the recipe, simple carbohydrates (glucose) and complex carbohydrates in the form of starch molasses containing (maltose, tri - Tetra - sugars, dextrins) are used. Glucose in the formulation is the main energy ingredient, sweetener and structure-forming component that gives the product a solid consistency due to crystallization. The medicinal effect of the product is provided by rosehip extract. Numerous published studies have established its vitamin, immunostimulating and antioxidant activity, inhibition of uric acid formation and obesity, which are associated with flavonoids, other phenolic compounds, and vitamins. The list of medicinal properties of rosehip extract shows the feasibility of using it in additional nutrition of people in order to prevent diseases. For the formulation of our sweets, the extraction of crushed rosehip fruits was carried out three times with hot water at a temperature of 65-700C, followed by concentration of the extract under vacuum to 25-30% SV. The resulting extract-concentrate was introduced into the formulation in a dosage of 5.5% SV per 100 g of SV glucose Fudge, which provided the necessary dosage for the product when used from 3 (for children) to 10 sweets a day (for adults) for the prevention of colds.

Background: Cigarette smoke free radicals can cause cellular damage and different diseases. All the body fluids have antioxidants which protect against free radicals. Objective: The aim of this study was to evaluate salivary total antioxidant capacity and peroxidase, uric acid and malondialdehyde levels in smokers and a nonsmoking control group. Methods: Unstimulated saliva was collected from 510 males. A total of 259 subjects were current smokers and 251 were non-smokers. The levels of salivary total antioxidant capacity, uric acid, peroxidase and malondialdehyde were measured using standard procedures. Data were analyzed with t test and ANOVA. Results: The smokers were younger and dental hygiene index was higher than healthy nonsmoking controls. The mean total antioxidant capacity in smokers and nonsmokers was 0.13±0.07 and 0.21±011, respectively (P=0.001). Smokers had significantly lower peroxidase and uric acid levels than healthy controls. In addition, the mean malondialdehyde levels in the smokers and nonsmokers were 4.55 ±2.61 and 2.79 ±2.21, respectively (P=0.001). Conclusion: Cigarette smoke produces free radical and oxidative stress, causing many side effects. Salivary antioxidant levels decreased and malondialdehyde levels increased in smokers, indicating the high oxidative stress among smokers compared to nonsmokers. Cigarette smoke had deleterious effects on main salivary antioxidants levels.

AbstractCombinations of fructose- and fat-rich diets in experimental animals can model the human metabolic syndrome (MS). In rats, the increase in blood pressure (BP) after diet manipulation is sex related and highly dependent on testosterone secretion. However, the extent of the impact of diet on rodent hypophysial-testicular axis remains undefined. In the present study, rats drinking a 10% fructose solution or fed a high-fat (35%) diet for 10 weeks had higher plasma levels of luteinizing hormone (LH) and lower plasma levels of testosterone, without significant changes in circulating follicle-stimulating hormone or the weight of most reproductive organs. Diet manipulation brought about a significant increase in body weight, systolic BP, area under the curve (AUC) of glycemia after an intraperitoneal glucose tolerance test (IPGTT), and plasma low-density lipoprotein cholesterol, cholesterol, triglycerides, and uric acid levels. The concomitant administration of melatonin (25 μg/mL of drinking water) normalized the abnormally high LH levels but did not affect the inhibited testosterone secretion found in fructose- or high-fat-fed rats. Rather, melatonin per se inhibited testosterone secretion. Melatonin significantly blunted the body weight and systolic BP increase, the increase in the AUC of glycemia after an IPGTT, and the changes in circulating lipid profile and uric acid found in both MS models. The results are compatible with a primary inhibition of testicular function in diet-induced MS in rats and with the partial effectiveness of melatonin to counteract the metabolic but not the testicular sequelae of rodent MS.

Although the concept ‘spiritual nursing care’ has its roots in the history of the nursing profession, many nurses in practice have difficulty integrating the concept into practice. There is an ongoing debate in the empirical literature about its definition, clarity and application in nursing practice. The study aimed to develop an operational definition of the concept and its application in clinical practice. A qualitative study was conducted to explore and describe how professional nurses render spiritual nursing care. A purposive sampling method was used to recruit the sample. Individual and focus group interviews were audio-taped and transcribed verbatim. Trustworthiness was ensured through strategies of truth value, applicability, consistency and neutrality. Data were analysed using the NUD*IST power version 4 software, constant comparison, open, axial and selective coding. Tech’s eight steps of analysis were also used, which led to the emergence of themes, categories and sub-categories. Concept analysis was conducted through a comprehensive literature review and as a result ‘caring presence’ was identified as the core variable from which all the other characteristics of spiritual nursing care arise. An operational definition of spiritual nursing care based on the findings was that humane care is demonstrated by showing caring presence, respect and concern for meeting the needs not only of the body and mind of patients, but also their spiritual needs of hope and meaning in the midst of health crisis, which demand equal attention for optimal care from both religious and nonreligious nurses.

The bacteria that inhabit us have emerged as factors linking immunity and metabolism. Changes in our microbiota can modify obesity and the immune underpinnings of metabolic diseases such as Type 2 diabetes. Obesity coincides with a low-level systemic inflammation, which also manifests within metabolic tissues such as adipose tissue and liver. This metabolic inflammation can promote insulin resistance and dysglycaemia. However, the obesity and metabolic disease-related immune responses that are compartmentalized in the intestinal environment do not necessarily parallel the inflammatory status of metabolic tissues that control blood glucose. In fact, a permissive immune environment in the gut can exacerbate metabolic tissue inflammation. Unravelling these discordant immune responses in different parts of the body and establishing a connection between nutrients, immunity and the microbiota in the gut is a complex challenge. Recent evidence positions the relationship between host gut barrier function, intestinal T cell responses and specific microbes at the crossroads of obesity and inflammation in metabolic disease. A key problem to be addressed is understanding how metabolite, immune or bacterial signals from the gut are relayed and transferred into systemic or metabolic tissue inflammation that can impair insulin action preceding Type 2 diabetes.

Uric acid is the final product of purine metabolism, and excessive serum uric acid can cause gouty arthritis and uric acid nephropathy. Therefore, lowering the uric acid level and alleviating inflammation in the body are the key points to treating these diseases. A stable nanosuspension of peptide BmK9 was prepared by the precipitation-ultrasonication method. By combining uricase on the surface of a positively charged carrier, a complex consisting of neutral rod-shaped BmK9 and uricase nanoparticles (Nplex) was formed to achieve the delivery of BmK9 and uricase, respectively. The formulation of Nplex has a diameter of 180 nm and drug loading up to 200%, which releases BmK9 and uricase slowly and steadily in drug release tests in vitro. There was significantly improved pharmacokinetic behavior of the two drugs because Nplex prolonged the half-life and increased tissue accumulation. Histological assessments showed that the dual drug Nplex can reduce the inflammation response in acute gouty arthritis and chronic uric acid nephropathy in vivo. In the macrophage system, there was lower toxicity and increased beneficial effect on inflammation with Nplex than free BmK9 or uricase. Collectively, this novel formulation provides a dual drug delivery system that can treat gouty arthritis and uric acid nephropathy.

ABSTRAK Pandemi COVID-19 merupakan bencana non alam yang berdampak tidak hanya pada kesehatan fisik saja tetapi berdampak pula terhadap kesehatan jiwa dan psikososial masyarakat. Dampak kesehatan jiwa dan psikososial ini dapat menurunkan sistem imunitas tubuh manusia khususnya kelompok rentan yaitu kelompok lansia. Semakin seseorang merasa cemas yang berlebihan, maka tubuh mengalami kegagalan dalam menghasilkan antibodi untuk melawan virus.. Klien yang terdiagnosis positif COVID-19 merasa sangat takut dengan bayangan kematian, dan secara sosial diisolasi yang akan menimbulkan stigma sehingga mungkin terjadi ansietas dan depresi yang bila berlanjut dapat menyebabkan keputusasaan yang akan memperburuk kondisi fisik. Pemberdayaan generasi muda dalam hal ini adalah siswa SMAN 1 Sumberpucung dalam mendukung kesehatan jiwa dan psikosial lansia, yaitu lansia yang berada dalam keluarga siswa. Metode yang digunakan di masa pandemic COVID-19 dengan kegiatan atau pelatihan DKJPS (Dukungan Kesehatan Jiwa dan Psikososial) dievaluasi selama 1,5 bulan berupa melakukan upaya promosi kesehatan (peningkatan imunitas fisik dan imunitas jiwa) dan pencegahan masalah kesehatan (pencegahan penularan dan pencegahan masalah kesehatan jiwa) yang pada akhirnya disebut sebagai Adaptasi Kebiasaan Baru (AKBmelalui virtual menggunakan zoom dan platform Whatsapp Group Chat dengan metode ceramah, Tanya jawab, diskusi dan demonstrasi. Hasil Jumlah audience: 50 siswa dan jumlah lansia dan pra lansia: 11 orang Kata Kunci : Kesehatan Jiwa, Psikososial, Generasi Muda, Lansia  ABSTRACT The COVID-19 pandemic is a non-natural disaster that has an impact not only on physical health but also on the mental and psychosocial health of the community. This mental and psychosocial health impact can reduce the immune system of the human body, especially vulnerable groups, namely the older adults. The more a person feels excessively anxious; the body fails to produce antibodies to fight the virus. Clients who are diagnosed positive for COVID-19 feel very afraid of the image of death and are socially isolated which will lead to stigma so that anxiety and depression may occur if it continues. Can cause despair that will worsen the physical condition. The empowerment of the younger generation in this case is the students of SMAN 1 Sumberpucung in supporting the mental and psychosocial health of the older adults, which was the older adults who are in the student's family. The method used during the COVID-19 pandemic with DKJPS (Mental Health and Psychosocial Support) activities or training was evaluated for 4 weeks in the form of health promotion efforts (increasing physical immunity and mental immunity) and prevention of health problems (prevention of transmission and prevention of mental health problems. ) which is finally referred to as New Habit Adaptation (AKB through virtual using zoom and the Whatsapp Group Chat platform with the methods of lecturing, question and answer, discussion and demonstration Results Number of audiences: 50 students and the number of older adults and pre-elderly: 11 people Keywords: Mental Health, Psychosocial, Young Generation, Older Adults

<p><em>Many companies often pay less attention to the health of their employees during the work, it can seen from the number of employees exposed to the risk of diseases or health problems in retirement due to the imbalance between the needs of body and energy consumption that can lead to decreased productivity, especially labor-intensive companies one of them on the CV. X. The purpose of this study is to calculate the number of calories needed healthy body for employees of transport workers to regulate the pattern of food intake of the right transport workers so that caloric needs can be met. Data is collected by interview, observation and documentation study. The results showed that the work performed by transport workers was 1.5 hours including heavy work, 5 hours of moderate work and 1.5 hours of rest with an average caloric needs of 1253,601 cal / 8 hours of work, the body will experience a lack of calories when they work more or less about 6.5% (106,8576 cal / 8jam) of food they have eaten daily, then improved the food menu with an average of 1284,013 cal / 8 hours, an increase of 9.63% with the ratio of work productivity from before the improvement of 73.99% to 74.81%, resulting in an increase of 0.81%, implying on the convenience of laborers</em></p>

Modern approaches to protecting public health in the world are increasingly guided by the concept of preventable loss of health, which provides for the concentration of health efforts on the prevention of morbidity and disability. The nosological spectrum of causes of disability includes a large list of diseases, among which the leading ones are chronic non-communicable diseases, which are characterized by an epidemic type of spread. The highest level of disability is observed in connection with diseases of the circulatory system, malignant neoplasms, diseases of the musculoskeletal system and connective tissue. Of the total number of citizens recognized as disabled, a significant number are disabled people of the first and second groups. The significance of the disability problem is aggravated by the special demographic situation of the "aging" of the population, which is associated with the accumulation of pathology of various major body systems. Persistent dysfunctions of the body due to diseases, consequences of injuries and defects can lead to disabilities. The development of the UN international legal framework (UN Convention on the Rights of Persons with Disabilities, "International Classification of Functioning, Disability and Health") contributed to the formation of social policy in Russia aimed at protecting the rights of persons with disabilities. Medical and social rehabilitation and habilitation of persons with disabilities is the basis of social protection of citizens with health problems. The implementation of the main directions of medical and social rehabilitation and habilitation of persons with disabilities (medical, professional social) involves the use by persons with disabilities of technical means of rehabilitation that can compensate for impaired or lost functions of the body, which helps to overcome the limitations of life and socialization of citizens with persistent health problems.
