Carotenoids are pigments contained mainly in fruit and vegetables (F&V) that have beneficial effects on cardiometabolic health. Due to their lipophilic nature, co-ingestion of fat appears to increase their bioavailability via facilitating transfer to the aqueous micellar phase during digestion. However, the extent to which high fat intake may contribute to increased carotenoid plasma concentrations is still unclear. The objective was to examine the degree to which the consumption of different amounts of both carotenoid-rich foods and fats is associated with plasma carotenoid concentrations within a Mediterranean lifestyle context (subsample from the PREDIMED-Plus study baseline) where consumption of F&V and fat is high. The study population was categorized into four groups according to their self-reported consumption of F&V and fat. Carotenoids were extracted from plasma samples and analyzed by HPLC-UV-VIS-QqQ-MS/MS. Carotenoid systemic concentrations were greater in high consumers of F&V than in low consumers of these foods (+3.04 μmol/L (95% CI: 0.90, 5.17), p-value = 0.005), but circulating concentrations seemed to decrease when total fat intake was very high (−2.69 μmol/L (−5.54; 0.16), p-value = 0.064). High consumption of F&V is associated with greater systemic levels of total carotenoids, in particular when fat intake is low-to-moderate rather than very high.

Introduction: Chocolate intake has shown potential cardiometabolic health benefits in recent studies. We recently reported an association between chocolate intake and longer telomere length. Epigenetic age acceleration, an emerging marker of biological aging, predicts cardiovascular morbidity and mortality. But the relationship between chocolate intake and epigenetic aging has not been investigated. Hypothesis: We examined the association between chocolate intake and epigenetic aging among African Americans. Methods: A genome-wide methylation scan was performed among 50 overweight and obese African Americans using the Illumina Human Methylation 850K Bead Chip on peripheral blood mononuclear cell DNA. DNA methylation ages were calculated according to the Horvath and the Hannum clocks. Methylation-based age acceleration (ΔAge) was extracted from a linear regression of DNA methylation age on chronological age. Chocolate intake frequency over the past year was obtained by questionnaire, along with other major cardiovascular risk factors. Results: Fifty participants (aged 26.1 ± 9.3 years, 16% male) were included in the study. Fifteen (30%), 20 (40%), and 15 (30%) participants consumed less than 1 serving/month, 1-3 servings/month, and ≥1 serving/week of chocolate, respectively. Chocolate intake of ≥1 serving/week was associated with decreased Δage in both Horvath clock (β= -3.31, p-value = 0.046) and Hannum clock (β= -3.59, p-value = 0.096) after adjustment of age, sex, body mass index, 25(OH)D and CD4%. Conclusions: Chocolate intake may be associated with decreased epigenetic aging, with randomized controlled trials needed to establish any beneficial effects of chocolate intake on epigenetic aging processes.

There are about 2.8 million incident of perineal rupture in maternal physiological labor. In 2050,it is estimated that the incidence of perineal rupture can be 6.3 million if it is not accompanied by a good midwifery care. In 2016, in Trawas, there was (89%) perineal rupture in primiparas and (57%) perineal rupture in multiparas. Perineal rupture incidences due to parity were still very high. This study aims to analyze the relationship between parity and the incidence of perineal rupture . It is quantitative studyusing a cross sectional approach, by using analysis of physiological maternity women  medical record data from January to April 2017 of 130 peoplein Trawas Public Health Center, Mojokerto Regency.The dataanalysis used was Chi - Square , indicated by p value = 0,000 with ɑ = 0,05. It means that the value of p <ɑ, so H1 is accepted. It can be concluded that there is a relationship between parity and the incidence of perineal rupture on physiological maternity women in Trawas Public Health Center,Mojokerto Regency. Midwives can apply collaboration with patients and their families to have physical and psychologicalpreparation with an alternative of hypnobirthing methods.

Melatonin is a hormone secreted in the pineal gland with several functions, especially regulation of circadian sleep cycle and the biological processes related to it. This review evaluates the bioavailability of melatonin and resulting metabolites, the presence of melatonin in wine and beer and factors that influence it, and finally the different benefits related to treatment with melatonin. When administered orally, melatonin is mainly absorbed in the rectum and the ileum; it has a half-life of about 0.45–1 h and is extensively inactivated in the liver by phase 2 enzymes. Melatonin (MEL) concentration varies from picograms to ng/mL in fermented beverages such as wine and beer, depending on the fermentation process. These low quantities, within a dietary intake, are enough to reach significant plasma concentrations of melatonin, and are thus able to exert beneficial effects. Melatonin has demonstrated antioxidant, anticarcinogenic, immunomodulatory and neuroprotective actions. These benefits are related to its free radical scavenging properties as well and the direct interaction with melatonin receptors, which are involved in complex intracellular signaling pathways, including inhibition of angiogenesis and cell proliferation, among others. In the present review, the current evidence on the effects of melatonin on different pathophysiological conditions is also discussed.

ObjectiveTo investigate whether milk polar lipids (PL) impact human intestinal lipid absorption, metabolism, microbiota and associated markers of cardiometabolic health.DesignA double-blind, randomised controlled 4-week study involving 58 postmenopausal women was used to assess the chronic effects of milk PL consumption (0, 3 or 5 g-PL/day) on lipid metabolism and gut microbiota. The acute effects of milk PL on intestinal absorption and metabolism of cholesterol were assessed in a randomised controlled crossover study using tracers in ileostomy patients.ResultsOver 4 weeks, milk PL significantly reduced fasting and postprandial plasma concentrations of cholesterol and surrogate lipid markers of cardiovascular disease risk, including total/high-density lipoprotein-cholesterol and apolipoprotein (Apo)B/ApoA1 ratios. The highest PL dose preferentially induced a decreased number of intestine-derived chylomicron particles. Also, milk PL increased faecal loss of coprostanol, a gut-derived metabolite of cholesterol, but major bacterial populations and faecal short-chain fatty acids were not affected by milk PL, regardless of the dose. Acute ingestion of milk PL by ileostomy patients shows that milk PL decreased cholesterol absorption and increased cholesterol-ileal efflux, which can be explained by the observed co-excretion with milk sphingomyelin in the gut.ConclusionThe present data demonstrate for the first time in humans that milk PL can improve the cardiometabolic health by decreasing several lipid cardiovascular markers, notably through a reduced intestinal cholesterol absorption involving specific interactions in the gut, without disturbing the major bacterial phyla of gut microbiota.Trial registration numberNCT02099032 and NCT02146339; Results.

ABSTRACT Background Animal studies have highlighted critical roles of glycerophospholipid (GP) metabolism in various metabolic syndrome (MetS)-related features such as dyslipidemia, obesity, and insulin resistance. However, human prospective studies of associations between circulating GPs and risks of MetS are scarce. Objectives We aimed to investigate whether GPs are associated with incidence of MetS in a well-established cohort. Methods A total of 1243 community-dwelling Chinese aged 50–70 y without MetS at baseline and followed up for 6 y were included in current analyses. A total of 145 plasma GPs were quantified by high-throughput targeted lipidomics. MetS was defined using the updated National Cholesterol Education Program Adult Treatment Panel III criteria for Asian Americans. Results After 6 y, 429 participants developed MetS. Eleven GPs, especially those with long-chain polyunsaturated fatty acids (LCPUFAs) or very-long-chain polyunsaturated fatty acids (VLCPUFAs) at the sn-2 position, including 1 phosphatidylcholine (PC) [PC(18:0/22:6)], 9 phosphatidylethanolamines (PEs) [PE(16:0/22:6), PE(18:0/14:0), PE(18:0/18:1), PE(18:0/18:2), PE(18:0/20:3), PE(18:0/22:5), PE(18:0/22:6), PE(18:1/22:6), and PE(18:2/22:6)], and 1 phosphatidylserine (PS) [PS(18:0/18:0)], were positively associated with incident MetS (RRs: 1.16–1.30 per SD change; Bonferroni-corrected P &lt; 0.05). In network analysis, the strongest positive association for MetS incidence was evidenced in a module mainly composed of PEs containing C22:6 and PSs [RR: 1.21; 95% CI: 1.12, 1.31 per SD change; Bonferroni-corrected P &lt; 0.05]. This association was more pronounced in participants with lower erythrocyte total n–3 PUFA concentrations [Bonferroni-corrected Pinter(P value for the interaction)&lt; 0.05]. Conclusions Elevated plasma concentrations of GPs, especially PEs with LCPUFAs or VLCPUFAs at the sn-2 position, are associated with higher risk of incident MetS. Future studies are merited to confirm our findings.

Four hours after acute ingestion of 400 to 1,200 mg of propranolol by a healthy, 3-year-old boy, his plasma concentration of propranolol was 2,289 ng/ml. The only pharmacologic effect observed was a diminished heart rate response to crying and activity. In a second case, a 4-year-old boy on chronic propranolol therapy for renovascular hypertension had a hypoglycemic seizure when solid food was refused for three days because of an oral wound. The hypoglycemia was easily managed with intravenous glucose, and there were no sequelae. The first case alludes to the safety of propranolol in a healthy child even with very high plasma concentrations. The second case suggests the necessity of anticipating and avoiding hypoglycemia that can develop in children on chronic propranolol therapy when caloric intake is impaired.

Objective: The beneficial effects of exercise and ginger extract have been reported to improve obesity-related indicators. The aim of this study was to evaluate the effect of aerobic training and ginger extract on lipid profiles, body composition and selected liver enzymes in obese menopausal women. Materials and Methods: In this semi-experimental trial, 48 obese menopause women (age; 53-58 yr) were randomly divided into 4 groups including control, ginger extract, aerobic training and aerobic training -ginger extract. Aerobic training was performed 3 sessions a week, 60 minutes, with 50-70% maximum heart rate and for 24 weeks. The subjects consumed ginger extract three times a day in 500 mg capsule for 24 weeks. For analyzing the data, twoway ANOVA and Tukey's post hoc test was used with SPSS–23 and the significance level was P-value≤ 0.05. Results: The results showed that 12 and 24 weeks of training, ginger and ginger - training significantly decreased ALT and AST in obese menopausal women (P-value= 0.001). Also, 12- and 24- weeks’ ginger- training resulted to improve of body composition and lipid profile in obese menopausal women (P-value= 0.001). Conclusion: According to the findings, regular aerobic training and ginger supplementation have a beneficial effect on body composition and improvement of some liver enzymes and lipid profiles in obese menopausal women. These changes were higher in the Ginger-training group after six months of intervention.

A competitive job market is a challenge that needs to be faced by every university graduate. It is expected that every graduate has capability to keep face with expansion in job sectors. To be able to adapt to the job situation, a specific intelligence to face in facing every challenge and obstacle namely adversity quotient, is required. This study aimed to identify the correlation between adversity quotient (AQ) and career adaptability of internship nursing students. This study was descriptive correlational. Samples were recruited using a total sampling tecnique (n=142). Data were collected using a questionnaire consisting of a demographic survey, an Adversity Response Profile (ARP) Quick TakeTM and a Career Adapt-Abilities Scale (CAAS). Data were analyzed using descriptive statistics and Rank Spearman correlation test. The result of this study indicated that no students were in quitter category, only 0,7% of them were in quitter to camper transition, 76,8% of them were campers, 21,8% of the students were in camper to climber transition, 0,7% of all respondents were climbers. No students had a very low career adaptability, only 1,4% of them had a low career adaptability, while the proportions of students with a high and very high career adaptability were 75,4% and 23,2 % respectively. No significant correlation was found between AQ and career adaptability (p value >0,05; Spearman’s ρ +0,122). It can be concluded that most of internship nursing students at Faculty of Nursing Universitas Padjadjaran were campers and had a high level of career adaptability. There was also no significant correlation between AQ and career adaptability. Further research needs to explore about contributing factors related to career adaptability in internship nursing students.Keywords: Adversity quotient, career adaptability, internship nursing student. Hubungan Adversity Quotient dan Adaptabilitas Karier pada Mahasiswa Profesi NersAbstrakDalam menghadapi pasar kerja yang semakin ketat, setiap lulusan universitas diharapkan memiliki kemampuan untuk menghadapi tantangan dan beradaptasi dengan perkembangan pada dunia kerja. Untuk mengatasi hal tersebut diperlukan adanya kecerdasan dalam menghadapi setiap tantangan dan hambatan yang disebut adversity quotient (AQ). Penelitian ini bertujuan untuk mengetahui hubungan AQ dan adaptabilitas karier pada mahasiswa profesi ners. Penelitian ini menggunakan metode deskriptif korelatif. Responden penelitian berjumlah 181orang mahasiswa program profesi ners angkatan 32 dan 33 di Fakultas Keperawatan Universitas Padjadjaran yang di ambil dengan teknik total sampling dengan response rate 78,45%. Instrumen yang digunakan pada penelitian ini yaitu Adversity Response Profile (ARP) Quick TakeTM dan Career Adapt-Abilities Scale (CAAS). Teknik analisis data yang digunakan dalam penelitian ini menggunakan skoring ARP QuickTakeTM, skoring CAAS, serta uji korelasi Rank Spearman. Hasil penelitian menunjukkan bahwa tidak seorangpun responden yang berada pada kategori quitter, sebanyak 0,7% dari responden berada pada kategori transisi quitter ke camper, pada kategori camper sebanyak 76,8%, pada kategori transisi camper ke climber sebanyak 21,8%, dan pada kategori climber sebanyak 0,7%. Tidak ada mahasiswa dengan adaptabilitas karier sangat rendah, hanya 1,4% mahasiswa dengan adaptabilitas karier rendah, sedangkan mahasiswa dengan adaptabilitas karier tinggi dan sangat tinggi masing-masing proporsinya sebanyak 75,4% dan 23,2%. Tidak terdapat hubungan bermakna antara AQ dan adaptabilitas karier (p value >0,05; Spearman’s ρ +0,122). Simpulan dari penelitian ini yaitu sebagian besar mahasiswa profesi ners Fakultas Keperawatan Universitas Padjadjaran memiliki AQ dengan kategori camper dan memiliki adaptabilitas karier tinggi. Meskipun demikian, tidak terdapat hubungan bermakna antara AQ dan adaptabilitas karier. Bagi peneliti selanjutnya diharapkan untuk melakukan penelitian terkait faktor-faktor yang mempengaruhi adaptabilitas karier pada mahasiswa profesi ners.Kata kunci: Adversity quotient, adaptabilitas karier, mahasiswa profesi ners.

Background: Several studies have found that women who are overweight or obese have an increased risk of miscarriage. There is also some evidence of associations of other aspects of cardiometabolic health, including blood pressure and lipids, with miscarriage risk, although these have not been examined to the same extent as body-mass index (BMI). Methods: Our objective was to investigate the risk of miscarriage according to pre-pregnancy cardiometabolic health. We examined pre-pregnancy levels of BMI, blood pressure, fasting insulin and metabolites profile at age 18 and risk of miscarriage by age 24. The study included adult female offspring in the Avon Longitudinal Study of Parents and Children with a pregnancy between 18 and 24 years of age (n=434 for BMI and blood pressure; n=265 for metabolites). We used log-binomial regression to calculate adjusted associations between cardiometabolic health measures and miscarriage. Results: The overall risk of miscarriage was 22%.  The adjusted relative risks for miscarriage were 0.96 (95% CI: 0.92-1.00) for BMI (per unit increase), 0.98 (0.96-1.00) for systolic blood pressure, and 1.00 (0.97-1.04) for diastolic blood pressure (per 1 mmHg increase).  Total cholesterol, total lipids and phospholipids in HDL-cholesterol were associated with increased likelihood of miscarriage, but none of the p-values for the metabolites were below the corrected threshold for multiple testing (p-value ≤0.003). Conclusions: Our findings indicate no strong evidence to support a relationship between pre-pregnancy cardiometabolic health and risk of miscarriage in young, healthy women who became pregnant before age 24. Future studies are necessary that are able to evaluate this question in samples with a wider age range.

Background: Metabolic syndrome is a group of metabolic abnormalities in which the chance of developing cardiovascular disease, diabetes mellitus, chronic kidney disease are high. Aim: It aims at studying the lipid abnormalities in metabolic syndrome patients. Methods: Total of 100 metabolic syndrome patients were selected for study over a period of 1year. These patients were selected based on the criteria for metabolic syndrome as established by National Cholesterol Education Program (NCEP) adult Treatment Panel III (ATP III). Demographic data were taken and biochemical parameters were estimated by standard guideline. Results: Total cholesterol is significantly higher in very high risk (272.1 ± 8.591) compared to high risk (241.2 ± 3.901) and moderate risk (231.5 ± 4.498). TGL is significantly higher in very high risk (263.9 ± 13.70) compared to high risk (202.1 ± 6.531) and moderate risk (183.7 ± 7.650). HDL is almost same in very high risk (43.09 ± 1.533), high risk (40.44 ± 0.996) and moderate risk (42.53 ± 1.088). LDL is significantly higher in very high risk (177.9 ± 4.255) and high risk (169.4 ± 3.190) compared to moderate risk (155.7 ± 3.098). VLDL is significantly higher in very high risk (52.78 ± 2.739) compared to high risk (40.43 ± 1.306) and moderate risk (36.73 ± 1.530). CHO: HDL is significantly higher in very high risk (6.648 ± 0.366) compared to moderate risk (5.560 ± 0.207). High risk (6.060 ± 0.156) is not significantly different from very high risk and moderate risk. Thus, TC, TGL, LDL, VLDL, and CHO: HDL is significant as p value < 0.05 while HDL did not have any significance as p value > 0.05. Conclusion: In this study, high prevalence of dyslipidaemia is seen. So, timely diagnosis and treatment will help in detecting dyslipidaemia patients in future.
