Premature thelarche in an infant girl with failure to thrive related to dietary soy exposure

A 7-month-old female infant presented with failure to thrive. She was breastfed till 3 months of age, thereafter switched to soy-based milk formula. There was no history to suggest excess energy losses, recurrent infections or chronic diarrhoea. Three months after switching to exclusive soy-based milk formula, parents noticed significant enlargement of both breasts. Clinical examination was unremarkable except for enlargement of both breasts. None of the other secondary sexual characteristics were present. Initial blood investigations showed hyponatraemic hypokalaemic hypochloraemic metabolic alkalosis, which corrected after 2 days with intravenous hydration. The patient subsequently maintained normal electrolyte balance with recommended intake of cow’s milk-based standard formula milk.Further exploration of her soy-based milk revealed that it was low in sodium and calories, unsuitable for children. This was not a standard and approved infant soy-based formula milk. She achieved excellent weight gain and reduction of breast size on cessation of soy-based milk formula.

Background: Understanding the magnitude and clinical profile of pediatric HIV is essential for the clinicians and policy makers. The study was aimed to determine the rate of HIV seropositivity in pediatric patients with high risk clinical profile, study the clinical presentations and the mode of transmission of HIV in children.Methods: This prospective hospital-based study to screen 244 children aged 18 months to 12 years with high risk clinical profile for HIV seropositivity was carried out for a period of 1 year.Results: Of the 244 children screened, the commonest clinical features associated with high risk profile were failure to thrive in 200 (81.97%), persistent fever in 151 (61.89%), chronic diarrhoea in 76 (31.15%), cough >1month 112 (45.90%) patients. HIV seropositivity was reported in 11/244 (4.51%) patients; with failure to thrive in 10/11 (90.90%), chronic diarrhoea in 09/11 (81.81%), seborrheic dermatitis in 2/11 (18.18%) patients followed by persistent cough, severe malnutrition, oral thrush, generalized lymphadenopathy and recurrent bacterial skin infections in 1 patient each out of 11(9.09%). Chronic diarrhoea was a significant independent clinical risk factor for predicting HIV seropositivity (Chi2 = 13.81, p<0.001, Odds ratio=11.15). The probability of HIV seropositivity increased significantly with the number of risk factors concomitantly present, with 30% seropositivity in those with four clinical risk factors (Chi2 =32.89, D. F=1, P<0.001). The parents of all seropositive children were seropositive.Conclusions: The probability of HIV infection in a child depends upon the nature and number of clinical manifestations present. All HIV positive children h HIV positive parents in this study indicating vertical transmission.

A prospective study on steatocrit value in full-term or preterm newborn babies of~ 2 days of age in Dr. Pimgadi Hospital, Medan is reported. This study was conducted from December 7, 1992, to February 7, 1993; there were 72 newborn babies (37 males and 35 females). The body weight was> 2500 gin 60 babies and s. 2500 gin 12 babies. The median steatocrit value in babies with body weight of> 2500 g was 9%, and it was 32% in babies less than 2500 g of body weight. The proportion of babies with > 25% steatocrit value was larger in babies less than 2500 g than that in 2500 g by the diet patterns of breast milk, breast milk and milk formula, milk formula. The proportion of babies with < 25% steatocrit value was higher in babies with body weight of< 2500 g than that in babies > 2500 g for those who had either breast milk, breast milk and milk formula or milk formula. There was significant difference (p<0,05) in the steatocrit levels between babies with the body weight of > 2500 g and those who had body weight of ≤ 2500 g.

Abstract Background Early-onset chronic diarrhoea often indicates a congenital disorder. Mutation in diacylglycerol o-acyltransferase 1 (DGAT1) has recently been linked to early-onset chronic diarrhoea. To date, only a few cases of DGAT1 deficiency have been reported. Diarrhoea in those cases was severe and developed in the neonatal period or within 2 months after birth. Case presentation Here, we report a female patient with DGAT1 mutations with delayed-onset chronic diarrhoea. The patient had vomiting, hypoalbuminemia, hypertriglyceridemia, and failure to thrive at early infancy. Her intractable chronic diarrhoea occurred until she was 8 months of age. A compound heterozygous DGAT1 mutation was found in the patient, which was first found in the Chinese population. Her symptoms and nutrition status improved after nutritional therapy, including a fat restriction diet. Conclusions This case expanded our knowledge of the clinical features of patients with DGAT1 mutations. Intractable diarrhoea with delayed onset could also be a congenital disorder.

The aim of the present study was to determine the effects of age and diet (breast milk, formula milk and weaning diet) on metabolic activities in faecal samples from infants aged 1 week to 1 year, and to compare these findings with activities found in samples from adults. Such activities can provide valuable information on functional changes in the microbiota that may have significance for the health of the host. Fresh faecal samples were collected from forty-four breast-fed infants (twenty-four males, twenty females) and thirteen formula-fed infants (three males, ten females) throughout the first year of life. The samples were analysed for protein-breakdown products, including the faecal concentrations of NH3, phenol and p-cresol, and faecal bacterial enzyme activities. There was wide individual variation in all variables measured; however, the values in infants were substantially lower then those found in adults. In pre-weaned infants, faecal NH3 concentration and β-glucuronidase activity were the only endpoints that were significantly different in breast-fed and formula-fed infants (P<0·001 and P<0·05 respectively). This was not apparent after weaning. There was a significant difference between the breast-fed and formula-fed weaned groups and their pre-weaned counterparts only for NH3 (P<0·05). β-Glucuronidase activity and phenol concentration were significantly (P<0·01) greater in weaned breast-fed infants compared with pre-weaned breast-fed infants. No differences were observed between pre-weaned and weaned formula-fed infants for any of the variables except for NH3 concentration. It can be concluded from the present study that there are significant differences in two faecal characteristics between breast- and formula-fed infants and that changes occur as the infants grow older and are weaned onto solid foods.

Cakupan pemberian air susu ibu (ASI) eksklusif di Kota Pagar Alam, tahun 2011 sekitar 43% tergolong rendah. Sebaliknya, pemberian susu formula meningkat tiga kali lipat dari 10,3% menjadi 32,5%. Iklan susu formula telah menyentuh bidan swasta dan puskesmas melalui pendekatan produsen susu formula dan pemberian susu formula secara gratis kepada ibu menyusui. Penelitian yang bertujuan mengetahui determinan kegagalan praktik pemberiaan ASI eksklusif di Kota Pagar Alam Provinsi Sumatera Selatan ini menggunakan desain studi unmatching kasus kontrol. Populasi adalah seluruh ibu yang mempunyai bayi berusia 7 _ 12 bulan. Penarikan sampel dilakukan dengan metode proportional random sampling. Variabel terikat praktik adalah pemberian ASI eksklusif, variabel bebas adalah pemberian sampel susu formula. Ibu yang mendapat sampel susu formula dan yang tidak mendapat dukungan tenaga kesehatan berisiko 3,67 dan 4,2 kali lebih besar untuk tidak memberikan ASI eksklusif.The coverage of exclusive breastfeeding in the City of Pagar Alam in 2011 was by 43%. Advertising of infant formula has reached privately practicing midwives or health centers. The approach from infant formula manufacturers to midwives in health centers is by providing free milk formula to nursing mothers to be distributed under the pretext of promotion. The objective of this study is to analyze the determinants of exclusive breastfeeding practice failures in the City of Pagar Alam of South Sumatra Province. The population study with an unmatched case-control design was conducted in the City of Pagar Alam. The population was all breastfeeding mothers who had babies in the city of Pagar Alam of South Sumatra Province. The research subjects are breastfeeding mothers who had babies aged 7 - 12 months who selected with proportional random sampling method. The variables of the study included the dependent variable, i.e, the practice of exclusive breastfeeding, the independent variable, i.e, promotion of free milk formula samples. The risk of not exclusively breast feeding amang mothers who had accepted formula milk samples and who absence of support posed by health workers is 3.67 and 4.20 times higher the mothers who had not accepted the formula milk sample and who absence of support posed by heath worker.

Maternal factors relating to milk formula feeding on infants aged 0-6 months in Lampung-IndonesiaBackground: Many parents assume that breastmilk only would not be sufficient to fulfil baby’s nutrition need, so that baby need to feed with complementary feeding. The administration of complementary feeding like formula milk has been a common practice for some parents with reasons including less breastmilk production, maternal busy activities, less maternal knowledge concerning breastmilk feeding, saving time, being attracted by offered formula milk. Most parents administer formula milk with amount almost similar to breastmilk to fulfil the baby’s need.  Purpose: Knowing factors of formula feeding for infants 0-6 months.Methods: This was a quantitative analytic research by using cross sectional approach. Population was 220 babies of 0-6 months old in Waykandis public health centre working area from January to April 2019. 142 respondent samples were taken by using proportional random sampling.Results: The statistic test result showed that there were correlations of maternal occupation (p-value 0.016; OR 2.485), maternal education (p-value 0.004; OR 2.886), maternal knowledge (p-value 0.000; OR 0.089) to formula milk administration.Conclusion: There were factors of formula feeding for infants 0-6 months. The researcher expects the public health centre to create a schedule list for complementary feeding besides breast milk administration according to the children fewer than five needs, and to be always active in providing health education especially concerning the importance of formula milk knowledge.Keywords: Maternal; Formula Feeding; Infants 0-6 MonthsPendahuluan : Banyak orang tua menganggap bahwa kebutuhan nutrisi bayi tidak cukup hanya dengan ASI, sehingga bayi perlu dibantu dengan memberikan makanan pendamping ASI. Pemberian makanan pendamping ASI berupa susu formula sudah menjadi hal yang biasa, dengan berbagai alasan yang diberikan seperti ASI yang keluar sedikit, kesibukan ibu, kurangnya pengetahuan ibu tentang pemberian ASI, hemat waktu, tergiur dengan kandungan susu formula yang ditawarkan. Kebanyakan orang tua menilai pemberian susu formula hampir setara dengan ASI dan dapat mencukupi kebutuhan gizi bayinya. Tujuan  : Diketahui faktor-faktor yang berhubungan dengan pemberian susu formula pada bayi usia 0-6 bulan.Metode : Jenis penelitian kuantitatif. Rancangan menggunakan analitik, dengan pendekatan cross sectional. Populasi dalam penelitian seluruh bayi usia 0-6 bulan di wilayah kerja Puskesmas Waykandis dari bulan Januari sampai dengan bulan April 2019 yaitu sebanyak 220 orang. Sampel berjumlah 142 responden dengan teknik Proportional Random Sampling.Hasil : Berdasarkan hasil uji statistik, terdapat hubungan antara faktor pekerjaan dengan pemberian susu formula (p-value 0,016) nilai OR 2,485, terdapat hubungan antara faktor pendidikan dengan pemberian susu formula (p-value 0,004) nilai OR 2,886, terdapat hubungan antara faktor pengetahuan dengan pemberian susu formula (p-value 0,000) nilai OR 0,089.Simpulan : Ada hubungan antara faktor pekerjaan, pendidikan dan pengetahuan terhadap pemberian susu formula pada bayi 0-6 bulan. Diharapkan tenaga kesehatan dapat membuat daftar menu pemberian MP-ASI sesuai dengan kebutuhan balita, selalu aktif memberikan penyuluhan kesehatan pentingnya pengetahuan tentang susu formula.

We report the fatty acid profile of raw milk and of the corresponding digested milk from different sources (human milk, formula milk and donkey, bovine, ovine and caprine milk) to gain information on the nutritional quality of different milk sources in infant nutrition.Short chain fatty acids (SC-FA) were higher in bovine and caprine milk, intermediate in ovine and donkey and lower in human and formula milk. Medium chain fatty acids (MC-FA) showed the highest values for bovine and caprine milk and the lowest for donkey and formula milk, whereas long chain fatty acids (LC-FA) were the highest in donkey and formula milk and intermediate in human milk.The percentage distribution of fatty acids liberated after in vitro digestion did not reflect the patterns found in the corresponding milk sources. In particular, MC free fatty acids (MC-FFA) showed the highest and the lowest values in donkey and in formula milk, LC-FFA showed the highest value in human milk. The total FFA was highest in human milk, lowest in formula milk and intermediate in donkey, bovine, ovine, and caprine milk.
