Purpose. To explore the diagnostic performance of the optimized threshold b values on IVIM to detect the activity in axial spondyloarthritis (axSpA) patients. Method. 40 axSpA patients in the active group, 144 axSpA patients in the inactive group, and 20 healthy volunteers were used to evaluate the tissue diffusion coefficient ( D slow ), perfusion fraction ( f ), and pseudodiffusion coefficient ( D fast ) with b thresholds of 10, 20, and 30 s/mm2. The Kruskal-Wallis test and one way ANOVA test was used to compare the different activity among the three groups in axSpA patients, and receiver operating characteristic (ROC) curve analysis was applied to evaluate the performance for D slow , f , and D fast to detect the activity in axSpA patients, respectively. Results. D slow demonstrated a statistical difference between two groups ( P < 0.05 ) with all threshold b values. With the threshold b value of 30 s/mm2, f could discriminate the active from control groups ( P < 0.05 ). D slow had similar performance between the active and the inactive groups with threshold b values of 10, 20, and 30 s/mm2 (AUC: 0.877, 0.882, and 0.881, respectively, all P < 0.017 ). Using the optimized threshold b value of 30 s/mm2, f showed the best performance to separate the active from the inactive and the control groups with AUC of 0.613 and 0.738 (both P < 0.017 ) among all threshold b values. Conclusion. D slow and f exhibited increased diagnostic performance using the optimized threshold b value of 30 s/mm2 compared with 10 and 20 s/mm2, whereas D fast did not.

Background. The C-reactive protein (CRP) to albumin (ALB) ratio (CAR) has emerged as a novel inflammatory biomarker. This study was designed to investigate the role of CAR in the disease activity of axial spondyloarthritis (axSpA). Methods. A total of 241 patients and 61 healthy controls were retrospectively enrolled in this study. AxSpA patients were further divided into the inactive group ( n = 176 ) and active group ( n = 65 ) according to Bath Ankylosing Spondylitis Disease Activity Index (BASDAI) cutoff value of 4. Laboratory data and clinical assessment indices were recorded. Spearman’s correlation analysis, receiver operation characteristic (ROC) curve analysis, and binary logistic regression analysis were performed. Results. In axSpA patients, CAR was significantly higher than the healthy group ( P < 0.001 ). Similarly, axSpA patients in the active group had higher CAR than the inactive group ( P < 0.001 ). Besides, CAR was positively correlated with erythrocyte sedimentation rate (ESR) ( r = 0.704 , P < 0.001 ), CRP ( r = 0.996 , P < 0.001 ), BASDAI ( r = 0.329 , P < 0.001 ), and Bath Ankylosing Spondylitis Functional Index (BASFI) ( r = 0.330 , P < 0.001 ). ROC curve analysis suggested that the area under the curve (AUC) of CAR for axSpA of the active group was 0.701, which was higher than that of CRP and ESR. The optimal cutoff point of CAR for axSpA of the active group was 0.3644, with a sensitivity and specificity of 58.5% and 79.0%. Binary logistic analysis results revealed that CAR was an independent predictive factor for axSpA disease activity ( odds   ratio = 4.673 , 95% CI: 1.423-15.348, P = 0.011 ). Conclusions. CAR was increased in axSpA and axSpA of the active group. CAR may be a novel and reliable indicator for axSpA disease activity.

Abstract Background Recently, wire-based resting indices have been recognized as gold standard for evaluating physiological lesion assessment. The resting full-cycle ratio (RFR) is a unique resting index which is calculated as the point of absolutely lowest distal pressure to aortic pressure during entire cardiac cycle. It is unclear whether the diagnostic performance of RFR for detecting functional coronary artery stenosis is similar in each coronary artery. The aim of this study is to compare the diagnostic performance of RFR based on target coronary vessel. Method This study was a prospectively enrolled observational study. A total of 156 consecutive patients with 220 intermediate lesions were enrolled in this study. The RFR was measured after adequately waiting for stable condition, while FFR was measured after intravenous administration of ATP (180mcg/kg/min). Lesions with FFR ≤0.80 were considered functionally significant coronary artery stenosis. Results In all lesions, reference diameter, diameter stenosis, lesion length, RFR, and FFR were 3.0±0.7mm, 45±13%, 13.0±8.8mm, 0.90±0.09, and 0.82±0.10, respectively. Functional significance was observed in 88 lesions (40%) of all lesions. RFR showed a significant correlation with FFR in overall lesions (r=0.774, p&lt;0.001). The ROC curve analysis of RFR showed good accuracy for predicting functional significance (AUC 0.87, diagnostic accuracy 81%) in all subjects. Regarding each target vessel, there were similar and significant positive correlation between RFR and FFR (LAD; r=0.733, p&lt;0.001, LCX; r=0.771, p&lt;0.001, RCA; r=0.769, p&lt;0.001, respectively). The prevalence of discordant between RFR and FFR was significantly different among 3 vessels (LAD 26%, LCX 12%, RCA 13%, respectively, p&lt;0.05 for among 3 groups). Regarding the comparison of ROC curves according to lesion location, AUC was significantly lower in LAD than in LCX and RCA (LAD 0.780, LCX 0.947, RCA 0.926, p&lt;0.01 for LAD compared to LCX, p&lt;0.01 for LAD compared to RCA, respectively). Furthermore, the diagnostic accuracy was significantly different according to target vessel (LAD 74%, LCX 88%, RCA 87%, respectively, p&lt;0.05 for among 3 vessels). Conclusion RFR demonstrated better diagnostic accuracy for evaluating functional lesion severity. The diagnostic performance of RFR was different based on target vessel. RFR is a unique and useful resting index, and it may detect functionally significant coronary stenosis that cannot be detected with other resting indices in daily practice. Abstract. The paper examines the effect of strategic training on the performance of younger and older adults in an immediate list-recall and a working memory task. The experimental groups of younger and older adults received three sessions of memory training, teaching the use of mental images to improve the memorization of word lists. In contrast, the control groups were not instructed to use any particular strategy, but they were requested to carry out the memory exercises. The results showed that strategic training improved performance of both the younger and older experimental groups in the immediate list recall and in the working memory task. Of particular interest, the improvement in working memory performance of the older experimental group was comparable to that of the younger experimental group.

Lead found in pipes, batteries, paint, dyes ceramic glaze, gasoline, and final waste disposal. The robusta coffee grouds residues had high carbon, nitrogen etc which can adsorb heavy metal.The purpose of this study is to analyze the robusta coffee grounds residues to adsorb the Pb in the water. The method of this research is a True Experimental using completely randomized design (CRD) method. There were control groups (C) and three treatments groups (T1; T2;T3; 5gram/ litre; 8gram/ litre; 10gram/ litre) with six repetitions. The robusta coffee ground residues were contacted for 2 hours.Total samples were24 samples which analyzed each parameter of the Pb with Atomic Adsorption Spectophotometry Analysis. The results showed that the more coffee ground residues that are exposed, increasingly turbid. The KS test showed that data were a normal distribution (sig=0,324). One way ANOVA test; Turkey post Hoc showed that there was sig difference between the control and treatment (F=4,326, Sig= 0,017). There were sig difference between control and treatment 2 and 3 (Sig=0,019; Sig=0,038). Robusta coffee grounds residues can reuse to adsorb the Pb pollution in the water. It can be a solution for treating the lead pollution in the water because of it easy to the application.

Penelitian ini bertujuan untuk mengetahui pengaruh pelatihan single leg speed hop dan double leg speed hop terhadap peningkatan daya ledak otot tungkai. Jenis penelitian ini adalah penelitian eksperimen sungguhan dengan rancangan the randomized pretest posttest control groups design. Sampel penelitian ini adalah siswa putri SMP N 3 Mengwi sebanyak 45 orang ditentukan dengan teknik simple random sampling. Daya ledak otot tungkai diukur dengan vertical jump test dan data dianalisis dengan uji t independent, uji anava satu jalur dan dilanjutkan dengan uji least significant difference (LSD) pada taraf signifikansi (α) 0,05 dengan bantuan komputer program SPSS 16,0. Berdasarkan uji t independent pada pelatihan single leg speed hop diperoleh nilai sebesar 4,987 dengan nilai signifikansi sebesar 0,000 sedangkan pada pelatihan double leg speed hop diperoleh nilai sebesar 2,509 dengan nilai signifikansi sebesar 0,018. Berdasarkan uji anava satu jalur diperoleh nilai sebesar 11,695 dengan nilai signifikansi sebesar 0,000 dan uji least significant difference (LSD) menunjukkan bahwa terdapat perbedaan pengaruh antara kedua pelatihan dan pelatihan single leg speed hop mempunyai pengaruh yang lebih baik dari pelatihan double leg speed hop dengan mean difference sebesar 4,667. Dari hasil penelitian, dapat disimpulkan bahwa pelatihan single leg speed hop dan double leg speed hop berpengaruh terhadap peningkatan daya ledak otot tungkai pada siswa putri SMP N 3 Mengwi tahun pelajaran 2013/2014 dan terdapat perbedaan pengaruh antara kedua pelatihan serta pelatihan single leg speed hop mempunyai pengaruh yang lebih baik dari pelatihan double leg speed hop. This research aimed to determine the effect of speed single leg speed hop and double leg speed hop training in improving power of leg muscle. This research was a true experiment by the randomized pretest posttest control groups design. The sample was students of SMP N 3 Mengwi daughter as many as 45 people were determined by simple random sampling. Power of leg muscle was measured by vertical jump test and data were analyzed by independent t-test, one way anova and least significant difference (LSD) with significance level (α) 0,05 using computer program SPSS 16.0. Based on independent t-test on single leg speed hop training obtained 4.987 with a significance value of 0.000, while the double-leg speed hop training obtained 2.509 with a significance value of 0.018. Based on one way anova of the obtained value of F 11.695 with a significance value of 0.000 and the least significant difference test (LSD) showed there was a different effect between the two types of training and single leg speed hop training has better effect than double leg speed hop training with mean difference of 4.667. From the results, it could be concluded that single leg speed hop and double leg speed hop training were effective to improve power of leg muscle on the students of SMP N 3 Mengwi daughter of the school year 2013/2014 and there was a different effect between the two types of training and single leg speed hop training have a better effect than double leg speed hops training.keyword : single leg speed hop training, double leg speed hop training, power of leg muscle.

Objective: High b-value diffusion-weighted imaging (hDWI) with a b-value of 2000 s/mm2 provides insufficient image contrast between benign and malignant tissues and an overlap of apparent diffusion coefficient (ADC) between Gleason grades (GG) in prostate cancer (PC). We compared image quality, PC detectability, and discrimination ability for PC aggressiveness between ultra-high b-value DWI (uhDWI) of 3000 s/mm2 and hDWI. Methods: The subjects were 49 patients with PC who underwent 3T multiparametric MRI. Single-shot echo-planar DWI was acquired with b-values of 0, 2000, and 3000 s/mm2. Anatomical distortion of prostate (AD), signal intensity of benign prostate (PSI), and lesion conspicuity score (LCS) were assessed using a 4-point scale; and signal-to-noise ratio, contrast-to-noise ratio, and mean ADC (×10–3 mm2/s) of lesion (lADC) and surrounding benign region (bADC) were measured. Results: PSI was significantly lower in uhDWI than in hDWI (p < 0.001). AD, LCS, signal-to-noise ratio, and contrast-to-noise ratio were comparable between uhDWI and hDWI (all p > 0.05). In contrast, lADC was significantly lower than bADC in both uhDWI and hDWI (both p < 0.001). In comparison of lADC between tumors of ≤GG2 and those of ≥GG3, both uhDWI and hDWI showed significant difference (p = 0.007 and p = 0.021, respectively). AUC for separating tumors of ≤GG2 from those of ≥GG3 was 0.731 in hDWI and 0.699 in uhDWI (p = 0.161). Conclusion: uhDWI suppressed background signal better than hDWI, but did not contribute to increased diagnostic performance in PC. Advances in knowledge: Compared with hDWI, uhDWI could not contribute to increased diagnostic performance in PC.

Abstract Background Different anthropometric parameters have been proposed for assessing central obesity in children, but the ability of these anthropometric parameters to correctly measure central obesity in Pakistani children is questionable and needs to be assessed. The aims of this investigation were to examine the diagnostic performance of anthropometric parameters as indicators of central obesity in Pakistani children as measured by waist circumference (WC) and to determine the sex-specific best cut-off values for these parameters that would identify obese children. Methods Anthropometric measurements – height, weight, WC, waist-to-hip ratio (WHR), waist-to-height ratio (WHtR), conicity index (CI) and neck circumference (NC) – from a cross-sectional sample of 5964 Pakistani children aged 5–12 years were analyzed. Receiver operating characteristics (ROC) analysis was used to examine the diagnostic performance and to determine the optimal cut-off point of each anthropometric parameter for identifying centrally obese children. Results It was found that WC had a significantly positive correlation with all studied anthropometric parameters. The ROC curve analysis indicated that all the parameters analyzed had good performance but WHtR had the highest value of the area under the curve (AUC). Optimal cut-off points associated with central obesity for boys and girls were, respectively, 0.47 and 0.48 for WHtR, 1.20 and 1.23 for CI, 0.96 and 0.96 for WHR and 26.36 and 26.54 cm for NC. Conclusions The sex-specific cut-off points for WHtR, CI, WHR and NC can be used to detect central obesity in Pakistani children.

The ratio of soluble fms-like tyrosine kinase-1 to placental growth factor (sFlt-1/PlGF) is elevated and proved to be useful in preeclampsia (PE) diagnosis. Its value in differential diagnosis with other pregnancy complications and prediction of pregnancy duration has yet to be clarified in Chinese population. We retrospectively analyzed 118 singleton pregnancies with suspected or diagnosed PE at the Peking Union Medical College Hospital (PUMCH) in China. Among these, 62 pregnancies were diagnosed as PE (48 early onsets and 14 late onsets, with 39 and 5 severe PE, respectively), 12 gestational hypertension (GH), 15 chronic hypertension (chrHTN), 16 autoimmune diseases, and 13 pregnancies with uncomplicated proteinuria. And 76 normal pregnancies were included as control. The results showed (1) the sFlt-1/PlGF ratio in early onset PE subgroup was significantly higher than that in GH, chrHTN, and control groups; the sFlt-1/PlGF ratio in late onset PE subgroup was significantly higher than that in chrHTN and control groups, but similar as GH group; the sFlt-1/PlGF ratio was similar among GH, chrHTN, and control groups. (2) The sFlt-1/PlGF ratio was significantly increased in the PE group compared with autoimmune disease and uncomplicated proteinuria pregnancies. (3) By ROC curve analysis, the cutoff value of the sFlt-1/PlGF ratio was less than 21.5 to rule out PE and higher than 97.2 to confirm the diagnosis of PE. (4) The sFlt-1/PlGF ratio was higher in PE pregnancies delivering within 7 days than those more than 7 days, either in early onset PE or severe PE. In conclusion, we show that maternal sFlt-1/PlGF ratio is an efficient biomarker in the diagnosis and differential diagnosis of PE. This ratio can be used to predict the timing of delivery for PE pregnancies.

Background. Glioma is the most common fatal malignant tumor of the CNS. Early detection of glioma grades based on diffusion-weighted imaging (DWI) properties is considered one of the most recent noninvasive promising tools in the assessment of glioma grade and could be helpful in monitoring patient prognosis and response to therapy. Aim. This study aimed to investigate the accuracy of DWI at both standard and high b values (b = 1000 s/mm2 and b = 3000 s/mm2) to distinguish high-grade glioma (HGG) from low-grade glioma (LGG) in clinical practice based on histopathological results. Materials and Methods. Twenty-three patients with glioma had DWI at l.5 T MR using two different b values (b = 1000 s/mm2 and b = 3000 s/mm2) at Al-Shifa Medical Complex after obtaining ethical and administrative approvals, and data were collected from March 2019 to March 2020. Minimum, maximum, and mean of apparent diffusion coefficient (ADC) values were measured through drawing region of interest (ROI) on a solid part at ADC maps. Data were analyzed by using the MedCalc analysis program, version 19.0.4, receiver operating characteristic (ROC) curve analysis was done, and optimal cutoff values for grading gliomas were determined. Sensitivity and specificity were also calculated. Results. The obtained results showed the ADCmean, ADCratio, ADCmax, and ADCmin were performed to differentiate between LGG and HGG at both standard and high b values. Moreover, ADC values were inversely proportional to glioma grade, and these differences are more obvious at high b value. Minimum ADC values using standard b value were 1.13 ± 0.17 × 10−3 mm2/s, 0.89 ± 0.85 × 10−3 mm2/s, and 0.82 ± 0.17 × 10−3 mm2/s for grades II, III, and IV, respectively. Concerning high b value, ADCmin values were 0.76 ± 0.07 × 10−3 mm2/s, 0.61 ± 0.01 × 10−3 mm2/s, and 0.48 ± 0.07 × 10−3 mm2/s for grades II, III, and IV, respectively. ADC values were inversely correlated with results of glioma grades, and the correlation was stronger at ADC3000 (r = −0.722, P≤0.001). The ADC3000 achieved the highest diagnostic accuracy with an area under the curve (AUC) of 0.618, 100% sensitivity, 85.7% specificity, and 85.7% accuracy for glioma grading at a cutoff point of ≤0.618 × 10−3 mm2/s. The high b value showed stronger agreement with histopathology compared with standard b value results (k = 0.89 and 0.79), respectively. Conclusion. The ADC values decrease with an increase in tumor cellularity. Meanwhile, high b value provides better tissue contrast by reflecting more tissue diffusivity. Therefore, ADC-derived parameters at high b value are more useful in the grading of glioma than those obtained at standard b value. They might be a better surrogate imaging sequence in the preoperative evaluation of gliomas.

Abstract Objectives This study was designed to evaluate the role of anti-CD74 antibodies in diagnosis of axial spondyloarthritis (axSpA) and their relationship to disease duration and disease activity. Methods Fifty patients with axSpA, 15 patients with RA and 15 healthy subjects were included in the study. Clinical examination and laboratory tests were done. The ESR, CRP level and ASDAS were measured as markers of the disease activity. Quantitative determination of human CD74 IgG antibodies was done. Results The mean age of the patients was 38.22 (S.D.12.20) years. The level of CD74 autoantibodies was significantly higher in axSpA in comparison to control groups. Most patients with positive articular and extra-articular manifestations were positive for CD74 autoantibodies. In patients with inactive disease, 33.3% were positive for CD74 autoantibodies, as were 83% with active disease. High percentages of patients with early and late axSPA were CD74 autoantibody positive. The majority of patients with positive disease activity in early and late axSpA were CD74 autoantibody positive. CD74 autoantibodies had 80% sensitivity vs both control groups with 87% specificity vs the healthy control group and 80% vs the RA control group in the diagnosis of axSpA. Conclusions The frequency of positive anti-CD74 IgG antibodies was as high in patients with early axSpA as in those with late axSpA, with no significant differences. There was a significant difference in the frequency of positive anti-CD74 IgG antibodies between patients with positive and negative disease activity. Based on the sensitivity and specificity of anti-CD74 IgG, this is a promising diagnostic tool to support the clinical diagnosis of axSpA.
