Analysis of factors of congenital malformations in families where there were cases of infertility in anamnesis

Analyses of clinical, epidemiological, medical and statistical data of source documents over the period of 2010-2013 have been carried out prospectively. The data were selected using the method "case-control", namely, registration forms of couples with infertility cases in anamnesis. For each registered case of infertility "Registration Card of primary infertility" was filled in and, as a control, "Registration card of a couple who gave birth to alive full-term healthy baby" born in the shortest period of time (according to methodical recommendations "The organization of genetic monitoring" [13]. 3 women (10,0 %) with primary infertility who contacted with occupational hazards (masters of tailoring services who suffered from noise and vibration) and no such cases in women in the control group (p<0,05). In women, of the studied group in 66,7 % of cases were observed gynecological diseases: Hydrosalpinx and saktosalpinks – 9 patients (30,0 %), 5 – Ovarian cysts (16,7 %), 3 – Ovarian sklerokistoza (10,0 %) and recorded one case of ovarian tuberculosis (3,3 %), that is statistically different (p<0,05) from the data of the control group – in which only two patients had ovarian cysts (6,7 %). In the studied group 17 women (56,7 %) were treated for infertility, which was statistically different (p<0,05) from the data of the control group, in which only 2 women (6,7 %) received treatment for infertility. Among the relatives of women of the studied group were people with loss of hearing and speech. That was the sister of the woman with primary infertility. And the husband’s sister of the couple with infertility suffered from Down syndrome that is statistically different (p<0,05) from the data of the control group, in which there were no anomalies among the relatives.

Attitudes towards tax compliance (willingness to comply) Indonesian society is still low. Required an innovative tax dissemination to increase it. The purpose of this study was to determine whether the tax dissemination based on Fogg Behavioral Model (FBM) approach may increase wiliingness to comply. The FBM based tax dissemination asserts that for a person to perform a target behavior, which is a willingness to comply, he or she must be sufficiently motivated, have the ability to perform the behavior, and be trigerred, to perform the behavior. This research is a combination of qualitative and quantitative analysis of statistical data results of the questionnaire, interviews, and observations of the respondent and the experimental process of dissemination. The method used in this study is a quasi experimental with patterns of nonequivalent control group (pretest-post which is not equivalent). Based on the results of data analysis, interviews and observations of the respondent and the experiment, this research showed that theFBMbasedtaxdissemination hasapositiveimpactto willingnesstocomplyofthetaxpayers.

Infertility is one of the most common diseases of the reproductive system, which carries a significant medical and social burden, affecting from 10 to 15% of married couples, and one of the causes of which is polycystic ovarian syndrome (PCOS), associated with menstrual and endocrine disorders. Changes in the concentration of key enzymes of the metabolic pathway of vitamin D in PCOS may affect the regulation of local tissue homeostasis. Mechanisms of inhibition of cell proliferation under the action of vitamin D have not been fully explored. The aim of the study is to investigate the expression of VDR and hormone receptors in the ovaries of women with PCOS. Material and methods. Patients were divided into main (I) and control groups (II), that included women (n=60) aged 18 to 44 years (mean age - 29.1±3.3 years), who at diagnostic laparoscopy due to infertility of unknown cause. Immunohistochemical (IHC) study of ovarian cysts was performed according to a standard protocol using antibodies to VDR, AR, ERß, PR. The expression of VRD, 1α-OH, 24-OH in samples of ovarian cysts was determined by polymerase chain reaction in real time (PCR-RT). Results. The following IHC expression of the studied markers was observed in the samples of the 1st group cysts of women with PCOS: VDR - 67.7±2.1%; AR - 51.7±1.3%; ERß - 23.1±1.2%; PR - 89.3±2.1%. According to PCR-RT data, the relative VDR expression in group I was 4,31±0,18 arb. u., vs 2,17±0,11 arb. u. in the control group. Conclusions. The key role in PCOS pathogenesis is played by complex metabolism disorder of vitamin D and androgen-estrogen balance as well as decrease of ERß and increase of PR expression level.

ABSTRACT BACKGROUND: Some mutations of the cystic fibrosis transmembrane regulator ( CFTR ) gene may impair spermatogenesis or cause a congenital absence of the vas deferens that manifests as isolated male infertility. OBJECTIVE: Assess the frequency and analyze the spectrum of CFTR gene variations in Saudi men with primary infertility. DESIGN: Prospective, cross-sectional. SETTING: Tertiary care specialist hospital in Jeddah. PATIENTS AND METHODS: Genomic DNA was extracted from peripheral blood samples of Saudi men who presented with primary infertility to the outpatient andrology clinic with either azoospermia or oligoasthenoteratozoospermia. Polymerase chain reaction and direct sequencing were used to identify all variants of the CFTR gene. MAIN OUTCOME MEASURES: Proportion of the patients with a mutant CFTR gene and the spectrum of CFTR gene variations. SAMPLE SIZE: 50 infertile Saudi men. RESULTS: This study identified 10 CFTR gene variants in 7 (14%) subjects (100 chromosomes). The detected variants and polymorphisms were: c.1408G>A, c.4389G>A, c.2562T>G, c.869+11C>T, c.2909-92G>A, c.3469-65C>A, c.1210-6delT, c.1210-6T>A, c.2988+1G>A, and c.1210-13GT>TG. CONCLUSION: We demonstrated that 14% of the study subjects had one or more CFTR mutations and these were compounded in most of the affected patients. The spectrum of CFTR gene mutations in these subjects was similar to the mutations reported in other studies throughout the world. LIMITATIONS: Small sample size and the lack of a control group. This research is based on field findings, namely low interest in learning Indonesian, lack of teacher initiative to use varied learning models that cause students to become bored in following the learning process in Indonesian subjects. The purpose of this study is to find out the increased interest in learning Indonesian by comparing the RADEC (Read, Answer, Discuss, Explain, and Create) learning models and conventional learning. The research method used was Nonequivalent Control Group Design with a population of 173 students, and two sample classes namely class V as an experimental class totaling 28 students and class VI as a control class totaling 37 students. From the results by statistical data using SPSS version 24.0 for windows after the data are declared normally distributed and come from homogeneous samples through normality and homogeneity tests, it is known that the results of hypothesis testing using the Compare Means test with Independent Samples Test obtained sig = 0,000. Because the significance <0.05, it can be concluded that Ho is rejected, which means "There is a difference in the increase in interest in learning Indonesian by using the RADEC learning model compared to using the conventional model in SD Negeri Sukamaju, Pagerageung District, Tasikmalaya District". The score is in the interval 66.4 -70.1. Based on this, it can be concluded that the implementation of the RADEC learning model is quite successful in increasing interest in learning Indonesian compared to using conventional models. Suggestions that can be recommended based on the findings of this study include expected to be able to develop RADEC learning models that are collaborated with other learning techniques as an innovation to increase learning interest.

Aim. Study of intramyocardial stress of the left ventricle (LV) in persons exposed to occupational vibration and patients with hand-arm vibration syndrome (HAVS). Materials and methods. We examined 15 individuals exposed to vibration, 44 individuals with grade 1 HAVS, 10 individuals with grade 2. The control group consisted of 20 persons without cardiovascular pathology, who had not been exposed to hazardous occupational factors. The main occupational hazards in persons with HAVS were vibration, noise, physical exertion. Echocardiography was carried out to determine the total volume of LV (Vtotal); the volume of myocardium (Vm); myocardial mass (LVMM); myocardial mass index (LVMMI); systolic intraventricular pressure; the intensity of the functioning of the LV structures; LV circumferential and meridional diastolic and systolic stresses (σcd, σcs and σmd, σms respectively); LV tension in the circumferential and meridional direction during systole and diastole (Tcs, Tcd and Tms, Tmd respectively). Results. In persons exposed to vibration, in comparison with those who have not been exposed to occupational hazards, there is an increase in σmd by 2.5 times (p < 0.05) and σcd by 2.8 times (p < 0.05). The tension increases during systole and diastole both in the circumferential (Tcs by 17.35%, p < 0.05; Tms by 2.74 times, p < 0.05) and in the meridional (Tcd by 14.59%, p < 0.05; Tmd by 2.76 times, p < 0.05) directions. In persons with grade 1 HAVS, there is an increase during diastole σmd by 2.2 times (p < 0.05), σcd by 2.4 times (p < 0.05), and Tmd (p < 0.05) and Tms by 2.5 times (p < 0.05). In patients with grade 2 HAVS, in contrast to those with grade 1 and persons exposed to vibration, there is a change in the working conditions of LV. With the same values of Vtotal, Vm, LVMM and LVMMI, as in the described groups, in patients with grade 2 HAVS, there is a decrease by 12.3% (p < 0.05) in systolic intraventicular pressure against the background of an increase in σmd by 14.7% (p < 0.05), Tmd by 13.6% (p < 0.05), as well as σms by 2.25 times (p < 0.05) and Tcd by 1.97 times (p < 0.05). Conclusion. In the group of persons exposed to vibration and patients with grade 1 HAVS, in contrast to those who are not exposed to occupational hazards, there is an increase in stress and tension, primarily in the circumferential direction during diastole, which indicates the activation of the heterometric type of autoregulation of activity of the heart. In patients with grade 2 HAVS, stress and tension increase in the meridional direction during systole, which indicates the levelling of LV hyperfunction by isotonic type.

Abstract Background In recent years, the prevalence rate of ACS in Chinese young women has been increasing significantly, becoming the main cause of death in young female. This study aimed to investigate the characteristics and difference of risk factors in Chinese young women with ACS and to provide references for ACS prevention and treatment. Methods A 1:1 case-control study was conducted to evaluate risk factors of 415 young female patients with ACS (ACS group) who underwent PCI treatment and 415 young female cases without ACS (control group) who were hospitalized and confirmed by coronary angiography to exclude coronary heart disease from January 2010 to August 2016. The average age of the cases in the two groups was respectively (40.77±4.02) years-old and (40.57±4.01) years-old (P> 0.05). Results The risk factors in ACS group were overweight (64.10%), hypertension (49.88%), hyperlipidemia (35.66%), diabetes (23.37%), depression or anxiety disorder (16.62%), gynecological diseases (16.39%), Hyperuricemia (15.18%), family history of early onset coronary heart disease (14.94%), hyperhomocysteinemia (11.33%), hypothyroidism(14.96%), hypercholesterolemia (8.43%) and high c-reactive protein (7.47%), and were statistically significant difference (P<0.01) compared with that of control group. The average number of risk factors per case in ACS group was significantly more than that of control groups (P<0.01). There was a statistically significant difference in the number of combined risk factors of the overweight cases compared between two groups (P<0.01). Regression analysis showed that hyperlipidemia, hyperhomocysteinemia, overweight(obesity), high CRP, hypertension, hypothyroidism, gynecological diseases, depression or anxiety, cardiac insufficiency, hypercholesterolemia, diabetes, oral contraceptives, family history of early onset CHD, and autoimmune diseases were independent risk factors (P<0.01). The bivariate correlation analysis between CRP level and age was r= -0.158 (P<0.01). This result showed the younger ACS patient is the higher serum CRP. Conclusion The independent risk factors of ACS in young women are hyperlipidemia, hyperhomocysteinemia, overweight, high CRP, hypertension, hypothyroidism, gynecological diseases, depression or anxiety, cardiac insufficiency, hypercholesterolemia, diabetes, oral contraceptives, family history of early onset CHD, and autoimmune diseases. The co-existence of multiple risk factors is the main cause suffering from ACS in young women.

AbstractA prospective study was performed to investigate the effect of neurosurgery on hearing. Thirty-two patients underwent neurosurgery while 32 patients who had surgical procedures not involving puncture or drainage of the subdural space, served as a control group. In the neurosurgical group, a significant loss of hearing was observed in the immediate post-operative period, with recovery over one week. No average threshold shift was observed in the control group. It is suggested that following neurosurgery the mechanism of hearing loss results directly from a decrease in pressure and/or volume of the cerebrospinal fluid, which is reflected within the perilymphatic fluid, comparable to a transitory endolymphatic hydrops.

Background: Cystic ovarian degeneration (COD) with prevalence of 10-13% is a common and economically important condition in dairy cattle affecting fertility. Predisposing factors and aetiology of ovarian cysts are multifaceted. The most widely accepted cause is either absent, insufficient or wrong time occurrence of pre-ovulatory LH-surge. The accuracy of differential diagnosis of cyst per rectum can be increased by simultaneous use of USG and/or plasma progesterone assay and therapeutic success depends on type and duration of cystic condition. The literature on these aspects in crossbred cows is meagre, hence was planned to differentially diagnose the ovarian follicular and luteal cysts based on clinical, sonographic and endocrine findings and to study the estrus response and conception rates following different treatment modalities of COD in crossbred cattle. Methods: This study was conducted on greater than 90 days postpartum HF crossbred cows with cystic ovaries (n=58) confirmed by per rectal palpation and ultrasonography. The animals with follicular cysts were randomly treated with either conventional Ovsynch or Ovsynch + CIDR protocol (n=10 each) and those with luteal cysts with either Double PG injections 11 days apart or modified Ovsynch protocol (n=16 each) with fixed time AI. A group of six cystic cows was kept as untreated control. Blood samples were collected in heparinized vacutainers, together with trans-rectal ultrasonography, on day 0 (just before initiation of treatment), on day of last PGF2á injection, on day of induced estrus/FTAI and on day 12 post-AI for determination of plasma progesterone and estradiol-17â by RIA technique. Results: The mean diameters and wall thickness of ovarian cysts regressed significantly (p less than 0.01) among responded cows of all four protocols. The estrus induction response within 48-96 hrs of last PG injection with Ovsynch, Ovsynch + CIDR, Modified Ovsynch and Double PG protocols was 90.00, 100.00, 87.50 and 81.25 %, respectively. The conception rates at induced estrus with FTAI for these protocols were 50.00, 40.00, 50.00 and 43.75 %, respectively and the corresponding overall conception rates for 3 cycles’ post-treatment were 60.00, 70.00, 68.75 and 56.25 %, respectively, compared to zero result of control group. The mean plasma progesterone concentration varied highly significantly (p less than 0.01) between periods of treatment in all protocols, the values were lower (p less than 0.01) on day 0 and on day of FTAI than on the day of last PG injection and on day 12 post-AI. The values on day 12 post-AI were significantly higher in conceived than non-conceived cows. The trend of estradiol-17â profile on day of last PG injection was reversed than on day 0. The higher levels of estradiol-17â on day of FTAI were associated with regression of cystic structures and development of new dominant follicles with estrogenic activity concomitant to induced estrus and ovulatory LH surge in most of the animals as evident from USG monitoring, behavioural signs and conception rates.

Annotation. Preeclampsia is one of the leading causes of maternal and perinatal death, and it complicates from 5 to 8% of all pregnancies. The aim of the research was to show the differences in anamnesis, diagnostics and prognosis, in groups of pregnant with hypertensive disorders. Retrospective investigation included 138 delivery histories and perinatal record cards from Vinnitsya Municipal Clinical Maternity hospital №1, in a period from 2016 till 2018. Women were divided into two investigated groups: I – 36 women with early preeclampsia, and II group – 52 women with late preeclampsia, and a control group of 50 women. The structure of somatic and gynecological diseases of pregnant women was studied, the severity of preeclampsia and the condition of the newborn were assessed according to the Apgar scale. Statistical processing of the obtained results was performed by odds ratio, its standard error and 95% confidence interval were calculated according to D. G. Altman (1991). It is established that in women with obesity or excessive body weight (every second women in groups), and those who had cardiovascular disorders (twice more often with late preeclampsia) developed preeclampsia more often, comparing to control group. All cases of antenatal and early neonatal death of newborn were registered in a group with early preeclampsia, which points out on heavier flow of the disease and different pathogenesis. Cases of early pregnancy toxemia and edema of pregnant occurred twice more often, in group with early preeclampsia, and ran more heavy (up to the development of ascites), comparing to the second group. Debut of symptoms was earlier, that requires earlier prophylaxis (since week 12). Fetal intrauterine growth retardation, reversible and high resistant blood stream during ultrasound were marked only in group with EP. In future, this research together with pathohistological investigation of placentas and immunohistochemical research of those tissues, can underlie predictive and preventive personalized system for early and late preeclampsia.

Baby massage is a touch therapy in infants. It is the simplest and easiest way of communication, which makes contact between mother and her baby. The touch and the view of parental affection on her baby will drain the strength of love between the two. The results showed that the baby massage applied in a structured parenting program could decrease the incidence of post partum blues, the symptoms of mother depression, increasing the interaction between mothers and infants. The research method uses quasyexperimental with non rondom design pretests and posttest with control group design. The population is a pospartum mother and a healthy baby at the age of 2 weeks-1 month. Sampling techniques use acidental sampling with a large sample of 32 respondents divided into treatment and control groups. The process of data collection is done by giving a pre ters bounding atachment then given the next massage treatment done postest bounding atachment. Data analysis is used in pairs and no paired sample of the normality test. Based on the results of the analysis, there is an effect of a mother-baby massage on bounding attachment in the treatment group, and there is a difference in the result of bounding score attachment between the treatment group and the control group. Parental touches are the fundamental for the development of communication that will lead to reciprocal love. The baby massage will give more benefits to the mother and baby.
