Aims The EuroQol five-dimension (EQ-5D) questionnaire is a widely used multiattribute general health questionnaire where an EQ-5D < 0 defines a state ‘worse than death’ (WTD). The aim of this study was to determine the proportion of patients awaiting total hip arthroplasty (THA) or total knee arthroplasty (TKA) in a health state WTD and to identify associations with this state. Secondary aims were to examine the effect of WTD status on one-year outcomes. Patients and Methods A cross-sectional analysis of 2073 patients undergoing 2073 THAs (mean age 67.4 years (sd 11.6; 14 to 95); mean body mass index (BMI) 28.5 kg/m2 (sd 5.7; 15 to 72); 1253 female (60%)) and 2168 patients undergoing 2168 TKAs (mean age 69.3 years (sd 9.6; 22 to 91); BMI 30.8 kg/m2 (sd 5.8; 13 to 57); 1244 female (57%)) were recorded. Univariate analysis was used to identify variables associated with an EQ-5D score < 0: age, BMI, sex, deprivation quintile, comorbidities, and joint-specific function measured using the Oxford Hip Score (OHS) or Oxford Knee Score (OKS). Multivariate logistic regression was performed. EQ-5D and OHS/OKS were repeated one year following surgery in 1555 THAs and 1700 TKAs. Results Preoperatively, 391 THA patients (19%) and 263 TKA patients (12%) were WTD. Multivariate analysis identified preoperative OHS, deprivation, and chronic obstructive pulmonary disease in THA, and OKS, peripheral arterial disease, and inflammatory arthropathy in TKA as independently associated with WTD status (p < 0.05). One year following arthroplasty EQ-5D scores improved significantly (p < 0.001) and WTD rates reduced to 35 (2%) following THA and 53 (3%) following TKA. Patients who were WTD preoperatively achieved significantly (p < 0.001) worse joint-specific Oxford scores and satisfaction rates one year following joint arthroplasty, compared with those not WTD preoperatively. Conclusion In total, 19% of patients awaiting THA and 12% awaiting TKA for degenerative joint disease are in a health state WTD. Although specific comorbidities contribute to this, hip- or knee-specific function, mainly pain, appear key determinants and can be reliably reversed with an arthroplasty. ABSTRACT Objective: To evaluate the association of body mass index (BMI) and albumin with pulmonary function in cystic fibrosis (CF) pediatric subjects. Methods: This is a cross-sectional study with clinically stable CF’s subjects. Clinical (pulmonary function) and nutritional evaluation (body mass index and albumin) were performed. Univariate analysis was performed using simple linear correlations. Regression analysis was performed using an exit level of p<0.05. Results: Seventy-eight CF’s subjects (mean age 12.8±3.8 years) with mean albumin 4.2±0.4 mg/dL, predicted forced expiratory volume in 1 second (FEV1%) 80.8±22.6 and BMI median percentile 51.2 (1.3-97.7). In the multiple regression models, albumin, age and BMI percentile were associated with pulmonary function. Subjects with lower than 25 BMI percentile had 12.2% lower FEV1%. An albumin increase of 0.1 mg was associated with 2.7% increase in predicted FEV1%, and one year increase in age was associated with reduction in 1.2% of predicted FEV1%. Conclusions: BMI percentile, albumin and age were independently associated with predicted FEV1% in a tertiary referral hospital.

Abstract Background AECOPD is highly heterogeneous with respect to etiology and inflammation. COPD with higher blood eosinophils is associated with increased readmission rates and better corticosteroid response. However, the clinical features of eosinophilic AECOPD aren’t well explored. Then, the aim of this study was to investigate the clinical differences between eosinophilic and non-eosinophilic AECOPD. Methods A total of 643 AECOPD patients were enrolled in this multicenter cross-sectional study. Finally, 455 were included, 214 in normal eosinophils AECOPD (NEOS-AECOPD) group, 63 in mild increased eosinophils AECOPD (MEOS-AECOPD) group, and 138 in severe increased eosinophils AECOPD (SEOS-AECOPD) group. Then, demographic data, underlying diseases, symptoms, and laboratory findings were collected. Multiple logistic regression was performed to identify the independent factors associated with blood EOS. Correlations between blood EOS and its associated independent factors were evaluated. Results The significant differences in 19 factors, including underlying disease, clinical symptom, and laboratory parameters, were identified by univariate analysis. Subsequently, multiple logistic regression revealed that lymphocytes%, neutrophils% (NS%), procalcitonin (PCT), and anion gap (AG) were associated with blood EOS in AECOPD. Both blood EOS counts and EOS% significantly correlated with lymphocytes%, NS%, PCT, and AG. Conclusions The blood EOS was independently associated with lymphocytes%, NS%, PCT, and AG in AECOPD patients. Lymphocytes% was lower, and, NS%, PCT, and AG were higher in eosinophilic AECOPD. Our results indicate that viral dominant infections probably were the major etiology of eosinophilic AECOPD. Non-eosinophilic AECOPD was more likely associated with bacterial dominant infections. The systemic inflammation in non-eosinophilic AECOPD was more severe.

Background: Chronic obstructive pulmonary disease (COPD) has become a huge public health problem in the world. In Asy-Syaafi Hospital, COPD is the most commonly found disease after bcronchitis among the patients in out patient unit of lung desease. Self efficacy showed patients’ confidence in independently managing chronic desease. Wheter they want or not to start the treatment is determined by their self efficacy. Peak expiratory flow rate showed condition and problems of lung function and the narrowing or blockage of the airway. Objective: Identify conditions and problems on self efficacy and Peak expiratory flow rate of COPD patients in Asy-Syaafi Hospital Pamekasan, East Java. Method: This research was descriptive study used non analytic cross sectional design, with total sampling, involving 30 respondents. Data were collected with a questionnaire of the COPD self efficacy (CSES) and peak flow meter. Data analysis was performed with a univariate analysis. Results: Self efficacy was in low category with score less than 99 (86.7%) and peak expiratory flow rate was less than <50% of the PEF (90%). Conclusion: Most of COPD self efficacy in Asy-Syaafi Hospital Pamekasan were in not good category and peak expiratory flow rate contained in red zone or the occurrence of major constriction of the airways. (1) Background: This study aimed to explore the symptoms, functional status, and depression in patients with chronic diseases in Loja, Ecuador. (2) Methods: A cross-sectional study was carried out with patients over 60 years old having at least one chronic disease and cared for in healthcare centers of the Health Ministry of Ecuador or living in associated geriatric centers. (3) Results: The sample comprised 283 patients with a mean age of 76.56 (SD 7.76) years. The most prevalent chronic diseases were chronic obstructive pulmonary disease, followed by arterial hypertension and diabetes. Patients with a joint disease had the worst scores for the majority of the symptoms assessed with the Edmonton Scale. Cancer, dementia, and arterial hypertension contributed the most to the dependence levels assessed with the Barthel Index. Dementia contributed the most to the poor performance status evaluated with the Karnofsky Performance Status. Cancer and diabetes contributed the most to depression. Patients with a higher number of chronic diseases reported worse functional status. (4) Conclusions: Targeted interventions to address symptoms, functional status, and depression in patients with chronic diseases are needed.

Objetivo: Avaliar a qualidade de vida (QV) dos Servidores Penitenciários do Estado de MS em relação às condições de trabalho e a saúde geral. Método: Trata-se de uma pesquisa descritiva, quantitativa de corte transversal. Foram avaliados 120 servidores da AGEPEN, sendo 10 para o estudo piloto. Para a coleta de dados foram aplicados o questionário sócio-demográfico, questionário de SG de Goldberg e o questionário de QV geral WHOQOL-abreviado. Para a análise descritiva estatística foram utilizados o Teste de diferenças de médias, o Teste de análise de variância (ANOVA), o Teste de correlação Linear de Pearson e o Teste Qui Quadrado. Resultados: Os resultados do instrumento WHOQOL-abreviado demonstraram que quanto maior grau de instrução menor a qualidade de vida no domínio psicológico; os casados, oficiais, maior renda e os que dormem mais apresentam melhor QV; os que utilizam remédios para dormir ou já tiveram afastamento do serviço apresentam menor QV. No QSG-60 os resultados foram de encontro aos encontrados no instrumento WHOQOL-abreviado. Conclusão: Os participantes possuem uma percepção significativa da capacidade laboral. A presente pesquisa poderá contribuir como suporte na elaboração do plano de ações visando à promoção da saúde e à prevenção de doenças e, consequentemente, melhor QV e SM dos servidores.Descritores: Saúde mental, Saúde do trabalhador, Qualidade de Vida, Prisões                      Abstract Objective: Evaluate the quality of life among penitentiary servers in Mato Grosso do Sul – AGEPEN, related with work conditions and general health state. Method. This is a descriptive, quantitative cross-sectional study. 120 AGEPEN servers were evaluated, of which 10 were pilots. To collect the data three instruments were used: a socio-demographic questionnaire, Goldberg General Health questionnaire and WHOQOL- short questionnaire.  3 tests were applied for statistical analyzes: Average absolute deviation, Pearson’s correlation and Chi-square distribution. Results. The results of the WHOQOL-abbreviated instrument demonstrated the higher the level of education the lower the quality of life in the psychological area; married, official, higher income and who sleep more have better QoL; sleeping pills users or who have had their services removed have a lower QOL. In the QSG-60 the results were against those found in WHOQOL-abbreviated.  Conclusion. The participants have significant perception of their quality of life and general health state relationship with working capacity. This survey will contribute supporting the creation of a plan to implement a policy of actions in favor of the servants aiming to improve their health, prevent diseases and consequently improve their quality of life.Key words: Mental health, Occupational health, Quality of life, Prisons

9104 Background: We performed a cross-sectional analysis of survivors of stage I endometrial cancer (EC) to examine the relationship between receipt of adjuvant intra-vaginal radiation therapy (IVRT), health state (HS), and sexual functioning (SF). Methods: Survivors were administered anonymous questionnaires containing demographic and treatment-related items, the EQ5D to measure current HS, and the Female Sexual Function Index (FSFI) to measure current SF. All were disease-free and had undergone simple hysterectomy (SH) with or without adjuvant IVRT at least one year prior to questionnaire completion. None had received chemotherapy or hormonal therapy. The study was IRB-approved under a waiver of consent. The primary endpoint was a comparison of EQ5D-health states and FSFI scores between survivors who had received SH alone and those who had received SH and adjuvant IVRT. Fisher’s exact test was used to compare IVRT and no-IVRT groups and multivariate regression was applied to find associations between factors and the outcome measures. Results: 136 (66.3%) participants had undergone SH alone and 69 (33.7%) had undergone SH and adjuvant IVRT. The SH alone group was younger; otherwise, the two groups were balanced with respect to ethnicity, marital status, education level, medical co-morbidities (diabetes, hypertension, anxiety), type of SH (laparotomy vs minimally invasive surgery-MIS), and baseline sexual activity. In the IVRT and no-IVRT groups, the median EQ5D-health states were 88 (range 40-100) and 85 (range 10-100) respectively, and the median FSFI scores were 14.1 (range 1.2-35.4) and 16.5 (range 1.2-34.8) respectively. Controlling for age and type of SH, receipt of IVRT was associated with better HS (p=0.018) and not associated with poorer SF (p=0.1399). Receipt of laparotomy (vs MIS) was associated with poorer HS and worse SF (p=0.0156 and p=0.0115, respectively), and its detrimental effect on FSFI score was more pronounced in the IVRT compared to no-IVRT group (p=0.0486). Conclusions: Compared to SH alone, receipt of adjuvant IVRT was generally not associated with poorer SF or HS.

Objective: The Coronavirus disease 2019 (COVID-19) vaccine is currently available. This timely survey was conducted to provide insight into on the willingness of healthcare workers (HCWs)to receive the vaccine and determine the influencing factors.Methods: This was a cross-sectional online survey. An online questionnaire was provided to all participants and they were asked if they would accept a free vaccine. The questionnaire gathered general demographic information, and included the General Health Questionnaire (GHQ-12); Myers-Briggs Type Indicator questionnaire (MBTI); Depression, Anxiety, and Stress Scales (DASS-21); and the 12-item Short Form Health Survey (SF-12). The data were collected automatically and electronically. Univariate analysis was done between all the variables and our dependent variable. Multivariable logistic regression models were employed to examine and identify the associations between the acceptance of the COVID-19 vaccine with the associated variables.Results: We collected 505 complete answers. The participants included 269 nurses (53.27%), 206 clinicians (40.79%), 15 administrative staff (2.97%), and 15 other staff (2.97%). Of these, 76.63% declared they would accept the vaccine. The major barriers were concerns about safety, effectiveness, and the rapid mutation in the virus. Moreover, four factors were significantly associated with the willingness to receive the vaccine: (a) “understanding of the vaccine” (odds ratio (OR):2.322; 95% confidence interval [CI]: 1.355 to 3.979); (b) “worried about experiencing COVID-19” (OR 1.987; 95% CI: 1.197–3.298); (c) “flu vaccination in 2020” (OR 4.730; 95% CI: 2.285 to 9.794); and (d) “living with elderly individuals” (OR 1.928; 95% CI: 1.074–3.462).Conclusions: During the vaccination period, there was still hesitation in receiving the vaccine. The results will provide a rationale for the design of future vaccination campaigns and education efforts concerning the vaccine.

Abstract. Background: Medical students are at high risk of suicidal ideation. Aim: We aimed to obtain information on suicidal ideation among medical students in Dammam located in the Eastern Province of Saudi Arabia. Method: This cross-sectional study was conducted at the College of Medicine affiliated with Imam Abdulrahman Bin Faisal University in the Eastern Province of Saudi Arabia. Suicidal ideation in the past 12 months was assessed based on responses to four questions in the depression subscale of the General Health Questionnaire 28 (GHQ-28). In addition, data were collected to examine the association of suicidal ideation with various factors. Results: We found that 1 in 3 medical students in the study had suicidal ideation in the past 12 months, while around 40% had lifetime suicidal ideation. Suicidal ideation was associated with feelings of parental neglect, history of physical abuse, and dissatisfaction with academic performance. Limitations: The cross-sectional nature of this study limits its ability to determine causality regarding suicidal ideation. Conclusion: These rates are considerably high when compared with rates from studies in other countries around the world. This study provides a reference in the field of suicidology for this region of Saudi Arabia.

Objective: This study aimed to assess the knowledge and awareness regarding cervical cancer and its prevention among nurses working in different hospitals of Quetta, Pakistan. Methodology: The cross sectional, descriptive study was conducted by using structured questionnaire in different hospitals of Quetta from January to September 2016. Convenient sampling technique was applied by targeting all the nurses working in different hospitals of Quetta city. Study questionnaire was developed and tested for validity and reliability. Descriptive and inferential statistics (Mann Whitney U test and Kruskal Wallis tests, p<0.05) were used to assess the significance among study variables and were performed by using IBM SPSS v.20. Results: Out of 415 distributed questionnaires 324 were returned (response rate of 78%). The mean Age of respondents was 28.18 ±9.5 years. Majority (n=127, 43.3%) of participants were interns and had no or less than one year of experience (n=128, 43.7%) with negative family history of any cancer (n=275, 93.9%). Mean knowledge score was 18.52±4.84with majority (n=258, 88.1%) had adequate knowledge regarding cervical cancer. Respondent sage, current area of practice, qualification, Institute of degree and past family history were contributing factors (p > 0.05) in adequate knowledge in this study. The results also reviled that not only 68.3% (n=200) and 65.5% (n=192) respondents knew that cervical cancer is vaccine preventable and availability of the vaccine for it. Conclusions: Nurses working in different hospitals of Quetta city had better understanding of the disease cervical cancer and its prevention. Yet many of the respondent are not aware of it vaccine and its availability.

The rate of stroke incidence is about 200 per 100,000 people throughout the world. This study aims to determine the Relation Suffer Stroke With Independence Level In Neurology Polyclinic TK II DR Ak Gani Palembang Year Hospital 2017. The research method used is descriptive quantitative with cross sectional design that is done by interviewing techniques with questionnaires on 42 respondents with Accidental sampling technique. This research was conducted in August 2017. Data analysis used is univariate data analysis and bivariate data analysis with one way anova test result. The results of univariate analysis showed that the duration of the respondents suffering from stroke was between 2.10 years to 3.38 years. Also found that most respondents were at the level of independence f; independent, except bathing, dressing, moving, and one other function with a total of 12 respondents. The results showed that there was a significant relationship between the long suffering stroke with the level of independence with the value of p value 0.025. For that the need for rehabilitation to patients and families of patients in order to help improve the independence of stroke patients in doing their daily activities.
