AbstrakAsma merupakan gangguan inflamasi kronik saluran pernafasan yang sering dijumpai pada anak. Penyakit ini memiliki banyak faktor risiko dan faktor pencetus. Beberapa diantara faktor tersebut adalah jenis kelamin, usia, riwayat atopi, makanan, perubahan cuaca, aktivitas, berat badan lahir, status gizi, pemberian ASI dan debu. Tujuan penelitian ini adalah untuk mengetahui faktor risiko dan faktor pencetus yang mempengaruhi kejadian asma pada anak di RSUP Dr. M. Djamil Padang. Ini adalah penelitian deskriptif dengan desain cross-sectional. Penelitian dilakukan dengan melakukan wawancara pada responden yang telah ditetapkan sebagai subjek penelitian. Subjek penelitian adalah seluruh pasien anak baik rawat jalan maupun rawat inap yang telah didiagnosis asma oleh dokter di RSUP Dr. M. Djamil Padang yang memenuhi kriteria. Penelitian dilakukan dari Februari sampai Maret 2013. Hasil penelitian menunjukkan bahwa faktor risiko dan faktor pencetus yang mempengaruhi asma pada anak adalah perubahan cuaca (65,91%), debu (63,64%), jenis kelamin (52,80%), makanan (43,19%), urtikaria pada anak (38,64%), rhinitis pada anak (34,09%), dermatitis atopi pada ibu (31,82%), dermatitis atopi pada anak (29,55%), aktivitas (27,27%), rhinitis pada ibu (22,72%), asma pada ibu (22,72%), urtikaria pada ayah (20,45%), berat badan lahir <2500 gram (15,91%) dan status gizi (obesitas) 2,28%. Berdasarkan hasil penelitian, kejadian asma banyak terjadi pada laki-laki, sebagian besar dipengaruhi oleh perubahan cuaca dan debu, riwayat atopi terbanyak pada anak adalah urtikaria, riwayat atopi terbanyak pada orangtua adalah dermatitis atopi pada ibu dan status gizi serta berat badan lahir pasien sebagian besar normal.Kata kunci: asma pada anak, faktor risiko, faktor pencetus AbstractAsthma is a cronic inflammatory disorder of respiratory tract that is often found in children. It has many risk faktor and inducer. Some of these risk are gender, age, history of atopy, food, climate change, activity, weight of birth, nutritional status, breastfeeding and dust. The objective of this study was to determine the risk factors and inducer that affect the incidence of asthma in children at the hospital of Dr. M. Djamil Padang. This was a descriptive study that use cross-sectional design. The study was conducted by interview to respondents who had been designated as a research subject. The subjects were all pediatric patients both inpatient and outpatient care that have been diagnosed as asthma by a physician in the hospital of Dr. M. Djamil Padang that have the criteria. The study done from February until March 2013. The result of the research showed that the risk factors and inducer that affect asthma in children are a weather change (65.91%), dust (63.64%), gender (52.80%), food (43.19%), urticaria in children ( 38.64%), rhinitis in children (34.09%), atopic dermatitis in women (31.82%), atopic dermatitis in children (29.55%), activity (27.27%), rhinitis in the mother ( 22.72%), asthma in women (22.72%), urticaria on the father (20.45%), birth weight <2500 g (15.91%) and nutritional status (obesity) 2.28%. Based on these results, the incidence of asthma more common in men, largely influenced by changes in the weather and dust, most history of atopy in children is urticaria, parental history of atopy was highest in atopic dermatitis and maternal.Keywords: asthma in chidren, risk factor, inducer

Background Morbidity and mortality of coronary heart disease(CHD) are recently increasing. This is related to changes in lifestyle,such as lack of activity and high consumption of fatty diet. Themain cause of CHD is atherosclerosis. The development of ath-erosclerosis takes a long time, is asymptomatic, and might beginin childhood. The important risk factors that have roles in increas-ing the likelihood of atherosclerosis are family history of prematureCHD, hypertension, hyperlipidemia, obesity, smoking and irregu-lar activity.Objective The aim of this study was to find out the prevalence ofCHD risk factors in children and young adults who had parentalhistory of premature CHD.Methods This was a descriptive cross sectional study conductedon offspring of premature CHD patients who were admitted in theintensive cardiology care unit (ICCU) of Cipto MangunkusumoHospital between January 1999 to December 2001 and of prema-ture CHD patients who visited the Cardiology Clinic of the Depart-ment of Internal Medicine, Cipto Mangunkusumo Hospital duringMarch and April 2002. Subjects were aged 12 to 25 year-old.Results Among the subjects, 40% had hyperlipidemia, 8% hadhypertension, 11% were obese, 21% were active smokers, 41%were passive smokers, and 73% had irregular activity. Ninety-sevenpercents subjects had more than 1 risk factors.Conclusions The prevalence of hyperlipidemia, hypertension,obesity, passive smoker, active smoker and irregular activity inchildren and young adults with parental history of premature CHDin this study were higher than those in the normal population.Most had more than 1 risk factor, increasing the likelihood of CHD.A screening test should be performed on children with parentalhistory of premature CHD so that early preventive measures mightbe done to minimize the risk factors

Objective: To create an allergic disease risk factors scale score that would screen for the risk assessment of asthma, allergic rhinitis and atopic dermatitis (AD) in children from 3 to 17 years. Methods: This case-control study, conducted between December 2015 and April 2016, enrolled 1,274 children. The allergic disease risk factors scale was created by combining environmental, exposure to toxics during pregnancy and breastfeeding and parental history of allergic diseases. Results: Playing on carpets, male gender, child’s respiratory problems or history of eczema before the  age of 2 years, and humidity significantly increased the odds of allergies in the child. Maternal waterpipe smoking, maternal history of rhinitis, history of asthma in the mother or the father, along with the maternal drug intake or alcohol consumption during pregnancy significantly increased the odds of allergies in the child. There was a significant increase in allergy diseases per category of the allergic disease risk factors scale (p < 0.001 for trend). Scores ≤2.60 best represented control individuals, while scores > 5.31 best represented children with allergic diseases. Conclusion: Allergic diseases seem to be linked to several risk factors in our population of school children. Many environmental factors might be incriminated in these allergic diseases.

ABSTRACTBackgroundPatients with COVID-19 can develop myocardial injury and arrhythmia during the course of their illness. However, the underlying risk factors for the development of cardiovascular related manifestations are unclear.MethodsUsing a register-based multi-center cross-sectional design, we analyzed 80 patients with myocardial injury and 401 controls, as well as 71 patients with arrhythmia and 409 controls, all admitted with COVID-19. Putative risk factors for myocardial injury and arrhythmia were evaluated with logistic regression with adjustment for potential confounders.ResultsCOVID-19 patients with myocardial injury had fatigue (66.2%) and dyspnea (63.7%), while those with arrhythmia had dyspnea (71.8%). Patients with myocardial injury and arrhythmia had a significant mortality of 92.5% and 94.4%, respectively. A history of chronic obstructive pulmonary disease (COPD) or heart diseases was associated with an increased risk of myocardial injury (odds ratio [OR] = 1.94, 95% confidence interval [CI]: 1.01-3.71; OR = 7.43, 95% CI: 3.99-13.83) and arrhythmia (OR = 1.94, 95% CI: 1.00-3.75; OR = 13.16, 95% CI: 6.75-25.68). In addition, we found that gamma glutamyltranspeptidase (GGT) >50U/L (OR = 2.14, 95% CI: 1.37-3.32; OR = 1.85, 95% CI: 1.19-2.85), serum creatinine >111μmol/L (OR = 8.96, 95% CI: 4.4-18.23; OR = 3.71, 95% CI: 2.01-6.85), serum sodium <136 mmol/L (OR = 4.68, 95% CI: 2.46-8.91; OR = 2.06; 95% CI: 1.06-4.00) were all associated with increased risk of myocardial injury and arrhythmia, respectively.ConclusionOur reported clinical characteristics and identified risk factors are important for clinical study of COVID-19 patients developing myocardial injury and arrhythmia.

To determine the prevalence and risk factors of IBS in children 8–13 years of age in Suzhou city, a cross-sectional study was conducted on children in grades 1 through 6 in public elementary schools in three districts of Suzhou. A multistage stratified random-sampling survey was conducted in a primary investigation using standardized questionnaires. Rome II criteria were used to confirm IBS and their risk factors were analyzed. Of 8,000 questionnaires 7,472 responded satisfactorily for a response rate of 93.4%. IBS was diagnosed in 10.81%. A decrease in the prevalence of IBS was significantly associated with advancing age and grade in school (trend test,P<0.05). The prevalence of IBS in females was higher but not significantly different than males. The significant risk factors for IBS included young age (OR = 0.94), food allergy (OR = 1.53), gastroenteritis during childhood (OR = 1.29), eating fried food (OR = 1.62), anxiety (OR = 1.49), psychological insults in early childhood (OR = 1.47), and parental history of constipation (OR = 1.81; allP<0.05). IBS prevalence of 10.81% in study population warrants preventive measures such as encouraging dietary changes, preventing gastroenteritis and childhood psychological insults.

Helminthiasis is a disease caused by intestinal parasite helminth with a high prevalence and spread throughout Indonesia, not deadly but secretly and slowly affect the health and productivity of patients through the decline in nutritional status. Slow and less symptom-prone effects cause the disease to be neglected among other diseases. This research is descriptive observational with cross sectional design, to know the description of nutritional status and risk factors of helminthiasis at elementary school children suffering from helminth in Meratus Dayak community, conducted in April 2017 at SDN Loksado and SDN Tanuhi, Loksado Subdistrict, Hulu Sungai Selatan Regency, South Kalimantan Province. The results showed 6.5% of helminthiasis occurred in the Dayak community in the Loksado Subdistrict. Helminthiasis consists caused by hookworm (58.33%), Enterobius (16.66), Hymenolepis (8.33%), Trichuris (8.33%), and mix infection of hookworm and enterobius (8.33%). However, this helminthiasis has not had an impact on the patient's nutritional status and progressed without symptoms, indicated by the nutritional status of patients who are mostly in the normal category until the fat. Although helminthiasis is not a deadly disease and is emergency, but the risk factor of helminthiasis in the patient can cause chronic infestation and transmission to other children. Abstract: Proteinuria is an excessive excretion of protein in urine. It is defined as 300 mg or more potein within 24 hours or 30 mg/Dl (+1 dipstick ) permanently in random urine samples. The main cause of proteinuria in pregnant woman is any desease associated with damage of the urinary tract organs. Furthermore, other causes of proteinuria in pregnant woman are pre-eclampsia and eclampsia, influenced by parental history of hypertension. This study was aimed to obtain the protein level in first trimester of primigravidas with parental history of hypertension in Manado. This was an observational descriptive study with a cross sectional design. Subjects were taken by using non-probality sampling consecutive sampling type to obtain urine samples from all subjects that matched the criteria and the specified time. The results showed that of 15 urine samples, there were 9 samples of positive protein (60%). Conclusion: Protein was found in urine samples of some primigravidas with parental history of hypertension in Manado.Keywords: proteinuria, pre-eclampsia, parental history of hypertension Abstrak: Proteinuria adalah ekskresi protein berlebihan dalam urin. Proteinuria didefinisikan sebagai terdapatnya 300 mg atau lebih protein dalam 24 jam atau 30mg/dL (1+ dipstik) secara menetap pada sampel acak urin. Penyebab utama terjadinya proteinuria pada ibu hamil ialah penyakit yang berhubungan dengan kerusakan pada organ sistem traktus urinarius. Penyebab lain dari munculnya proteinuria pada wanita hamil ialah pre-eklampsia dan eklampsia, dan dipengaruhi oleh faktor riwayat hipertensi pada orang tua. Penelitian ini bertujuan untuk mengetahui kadar protein urin pada primigravida trimester I dengan riwayat orang tua hipertensi di Kota Manado. Jenis penelitian ialah deskriptif observasional dengan desain potong lintang. Subyek penelitian diambil dengan cara non-probability sampling jenis consecutive sampling untuk mendapatkan urin dari semua subyek yang sesuai dengan kriteria dan waktu yang ditentukan. Hasil penelitian memperlihatkan bahwa dari 15 sampel urin, terdapat 9 sampel dengan positif protein (60%). Simpulan: Protein urin didapatkan pada sebagian primigravida dengan riwayat orang tua hipertensi di Kota Manado.Kata kunci: proteinuria, pre-eklampsia, riwayat hupertensi pada orang tua

Skin bacterial infections are often observed in eczema patients, but the risk factors are not fully understood. The current study evaluated the prevalence of clinically suspected bacterial infection and its associated risk factors. Moreover, we investigated the practice of skin infection diagnosis in China. A hospital-based, multicenter, cross-sectional epidemiologic survey of bacterial infection was performed in a total of 6208 outpatients diagnosed with dermatitis and eczema from 39 tertiary hospitals of 15 provinces and municipalities in China. All patients completed a specific questionnaire regarding their demographic characteristics, disease duration, distribution of lesions, severity of itching, and medical history. Univariate analysis and multivariate analysis were used to evaluate risk factors associated with bacterial infection in patients with different types of eczema. The prevalence of clinically suspected bacterial infection was 47.0% in patients with eczema. Compared to atopic dermatitis, widespread eczema ( OR = 1.50 , P < 0.001 ) and other eczema ( OR = 1.42 , P < 0.001 ) were more likely to suffer bacterial infection. The itching grade was positively associated with the infection ( OR = 2.11 , 7.04, and 12.3 in patients with mild, moderate, and severe itching, respectively; P < 0.001 ). Lesion distribution in the cubital fossa, popliteal fossa, ear, shoulder and back, axillary, foot, and pudendum was positively associated with bacterial infection (all OR > 1.0 , P < 0.05 ). In contrast, the face and abdomen were reversely associated with bacterial infection ( OR < 1.0 , P < 0.005 ). History of asthma, allergic rhinitis, allergic conjunctivitis, atopic dermatitis, infantile eczema, and flexion dermatitis was positively associated with bacterial infection (all OR > 1.0 , P < 0.005 ), while the history of dry skin was reversely associated with bacterial infection ( OR = 0.76 , 95% CI: 0.64-0.90; P = 0.002 ). Patients with eczema were easily infected with bacteria. Widespread eczema and other eczema were more likely to have bacterial infection than atopic dermatitis. The high rate of infection may attribute to the lack of corresponding bacterial detection, suggesting the need of guideline development in China to prevent overuse of topical antibiotics.

Background : Hypertension is the main risk factors of cardiovascular diseases that cause mortality. One of the risk factors of hypertension is obesity. Purpose: This study aimed to determine the relationship between nutritional status and waist circumference with the incidence of hypertension in the elderly. Methods: This study was observational analytic with cross-sectional design. The sample in this study was a partial of the elderly who participated in the elderly posyandu as many as 271 respondents. The sampling techniques is simple random sampling. Secondary data in the form of the results of the elderly health checkup at the elderly posyandu in the Puskesmas Sidotopo Wetan in 2018. Results: This study showed that of 271 respondents, there were 107 respondents found with hypertension (39.50%). The majority of hypertension was suffered by respondents with overweight nutritional status (46.70%), and waist circumference with the central obesity category (74.80%). Correlation found between nutritional status and the incidence of hypertension in the elderly (p=0.02<α=0.05). Prevalence Ratio of 1.32 means that elderly people with overweight nutritional status have the risk of 1.32 times greater to experience hypertension than elderly with underweight and normal nutritional status (p=0.02; PR=1.32). No correlation found between waist circumference and the incidence of hypertension (p=0.21). Conclusion: Nutritional status significantly associated with the incidence of hypertension in the elderly. However, waist circumference is not significantly associated with the incidence of hypertension.

Risk factors for bronchial asthma in children of Chita Region were studied. According to the literature data, 12 clinical-anamnestic characteristics were suggested to be risk factors for bronchial asthma. Prevalence of risk factors was assessed in 72 children with severe asthma, 60 children with moderate, and 58 children with mild bronchial asthma. Odds ratios (OR) with the 95% confident interval for each factor were calculated. Factors with predominance in the asthma group included: parental history of asthma (OR = 16.2), congenital abnormalities and complications of pregnancy and labor (OR = 1.9), prenatal lesion of central nervous system (OR = 2.8), recurrent respiratory tract infections (OR = 12.4), coexisting atopic dermatitis (OR = 1.8), passive smoking (OR = 2.8), and food sensitization (OR = 7.7). The findings can be used to calculate individual risks in a population for prediction of development of this pathology and for its timely prevention.
