Background: Tuberculosis still remain as a health problem in the world. No specific biomarkers can be used to monitor the rapid response to TB therapy. This study aims to assess the change of IL-17 levels in pulmonary tuberculosis patients with AFB smear-positive before and after 2 months of anti tuberculosis drugs (ATD) treatment. Method: Design of study is analytic observational with prospective cohort. The subject include 17 new cases of pulmonary tuberculosis patients with AFB smear-positive in Soetomo Hospital Surabaya from July - October 2016. Plasma levels of IL-17 were measured using ELISA technique before treatment and 2 months after the ATD therapy. Results: The mean levels of IL-17 before treatment is 3.510 pg / ml (2.087- 5.051 pg/ml). There were significantly decreament (p = 0.001

      Cytokines and chemokines are small-secreted proteins involved in many aspects of cell development, differentiation, and activation functions. A prominent characteristic of these molecules is their effect on the immune system in relation to the development of cell trafficking and immune tissues and organs. Furthermore, they play an important role in initiating and coordinating the organized and sequential recruitment and activation of cells into Mycobacterium tuberculosis-infected lungs. We aimed to evaluate the levels of interleukin -17 (IL-17) and the chemotactic chemokine (C-C motif) ligand 5 (CCL5) in the sera of pulmonary tuberculosis (PTB) patients. About 90 subjects were included, involving 50 patients with pulmonary TB and 40 apparently healthy individuals who were selected as a control group. Sera were obtained for measuring IL-17 and CCL-5 levels by enzyme linked immunosorbent assay (ELISA). The results revealed that serum levels of IL-17 showed no significant differences between each patient's group and control. In contrast, the serum level of CCL-5 was significantly increased in pulmonary tuberculosis patients compared to control (P ≤0.01). The mean ±SE values of IL-17 level in PTB patients and controls were 43.06 ±3.64 and 41.009 ± 0.009 pg/ml, respectively. While, the mean ±SE values of CCL-5 level in PTB patients and controls were 455.40 ±25.35 and 80.86 ± 5.96 ng/L, respectively. The results of the current study suggest that high levels of CCL-5 in the sera of PTB patients may indicate an important role in the immunopathogenesis of the disease. Therefore, this chemokine could be considered as a useful biomarker for the severity of PTB infections.

Pulmonary Tuberculosis (TB) is still a public health problem in the world. Not only in the world, in Indonesia is also a public health problem both in terms of mortality, the incidence of the disease and its diagnosis and treatment. A lot of factors affect whether a person is obedient and submissive in treatment of pulmonary TB disease, which are due to the role of health workers, the role of the PMO (supervisors of taking medicine), distance and knowledge of the patient. Pulmonary tuberculosis patients must be able to do the treatment on an ongoing basis, where the treatment is done for 6 months without stopping. Noncompliance treatment of pulmonary tuberculosis patients can increase the number of smear-positive pulmonary tuberculosis patients. Keywords: Compliance treatment, age, distance, PMO's role and the role of health workers

Hepatitis has become a health problem in the world. The hepatitis virus infected many people. According to the teacher of MTsN 02 Bondowoso more than 20 students have hepatitis A viral infection. The purpose of this research was to know the differences of students' knowledge about hepatitis A before and after counseling in MTsN 02 Bondowoso 2015. This study used pre-experimental (pre-post test design). This study used stratified random sampling technique, 127 students from 270 sample involved this research,and 143 students was excluded. We used questionnaires to collect data. The results showed that the mean value of the students 'knowledge about hepatitis A before counseling in MTsN 02 Bondowoso 2015 was 83.96 with the lowest value of 37.5 and the highest value was 100. The mean value of the students' knowledge about hepatitis A after counseling in MTsN 02 Bondowoso 2015 was 93.21 with the lowest value waf 62.5 and the highest value was 100. Paired t test showed that t (-9.07) > t table (1.98), the null hypothesis (H0) was rejected. There was a difference between students' knowledge about hepatitis A before and after counseling in MTsN 02 Bondowoso 2015. This study showed that routine counseling by healthcare provider was important to prevent hepatitis A infection.; Keywords: counseling, knowledge of students, hepatitis

This study was carried out to study of some immunological aspects among the pulmonary Tuberculosis patients infected with causative agent, Mycobacterium tuberculosis. A Total of 200 sputum samples were collected from patients attending the consultant Clinic for Chest and Respiratory disease center, Diwaniya. Control group (No=15) also included. According to acid fast stain of sputum, the patients were classified as positive (No=91,45.5%) and negative (No=109,54.5, Lowenstein Jensen medium used for the cultivation of samples, on which 70% of sputum samples where positive culture for this microorganism. The grown microorganism were identified as M. tuberculosis, based on positive A.F.B, Niacin producers ,negative for catlase at 68c. The mean IgG level was l184.053±76.684 mg/100 ml in tuberculosis group compared with 1016.533 ± 44.882 mg/100ml in control group, rendering the statistical difference significant. For IgA and IgM levels, they were at mean of 315.880±38.552 mg/100 ml and 119.527±8.464 mg/100 ml in control group compared with 396.358±38.776 mg/100 ml and 134.207±11.696 mg/100 ml in patients group respectively with significant difference

ABSTRACT Plasma levels of testosterone (T) and 5α-dihydrotestosterone (DHT) were determined by radioimmunoassay in 29 patients with benign prostatic hypertrophy (BPH) and in 56 control men of various ages. No significant difference was found in T, DHT nor DHT/T ratio between BPH and control subjects of similar age. Plasma DHT was higher in the prostatic than in the peripheral veins in 8/9 patients with BPH during laparotomy, indicating a prostatic secretion of DHT. No difference in the mean T nor the mean DHT was found in peripheral plasma before and after adenomectomy.

This research aimed at (1) developing teaching materials based on local content for Civic Education in University; and (2) describing the potential impact of local content on student consciousness on local culture. It was a developmental research. The subject were student registered in Civic Education Course (Mata Kuliah PKn) at History Department. Pilot project was validated through one to one, small group, and field evaluation. The analyzed result indicates that the developed instrument was valid and has potential impact. Supported data shows that the mean score prior to implementation was 6.86 and after implementation was 7.73. In other words, there was an increasing student achievement before and after its implementation. Accordingly, we suggest that this developed material could be implemented widely.

AbstractPulmonary tuberculosis is a chronic bacterial infection by the bacillus Myobacterium tuberculosis. This disease causes the patient to cough with phlegm or coghing up blood, shortness of breath, chest pain, night sweats and decreased appetite. The purpose of this scientific paper is ti describe nursing care for pulmonary tuberculosis patients with ineffective airway clearance problems. This research uses a descriptive case study method with the subject of two pulmonary tuberculosis patients who experience ineffective airway clearance problems in Pelutan Pemalang Village. The chest physiotherapy and effetive coughing actions were carried out for three days, twice a day, morning and evening. The results showed that the effective chest physiotherapy and coughing were able to overcome the ineffectiveness of airway clearance. Therefore, it can be concluded that the effective chest and cough physiotherapy is considered effective against ineffective airway clearance in pulmonary tuberculosis patients. For this reason, nurses are expected to be able to provide effective chest and cough physiotherapy therapy to help overcome the ineffectiveness of airway clearance in pulmonary tuberculosis patients.Keywords: Effective Cough and Chest Physiotherapy, Ineffective Airway Clearing AbstrakTuberculosis paru merupakan penyakit infeksi bakteri menahun yang disebabkan oleh basil myobacterium tuberculosis. Tujuan dari Karya Tulis Ilmiah ini untuk menggambarkan asuhan keperawatan pada pasien tuberculosis paru dengan masalah ketidakefektifan bersihan jalan nafas. Rancangan Karya Tulis Ilmiah ini menggunakan metode studi kasus deskriptif dengan subjek dua pasien tuberculosis paru yang mengalami masalah ketidakefektifan bersihan jalan nafas di Kelurahan Pelutan Pemalang, dilakukan tindakan fisioterapi dada dan batuk efektif dilakukan selama tiga hari dikerjakan dua kali sehari pagi dan sore. Hasil pemberian fisioterapi dada dan batuk efektif berhasil mampu mengatasi ketidakefektifan bersihan jalan nafas. Kesimpulan : fisioterapi dada dan batuk efektif dinilai efektif terhadap ketidakefektifan bersihan jalan nafas pada pasien tuberculosis paru. Untuk itu diharapkan perawat dapat memberikan terapi fisioterapi dada dan batuk efektif untuk membantu mengatasi ketidakefektifan bersihan jalan nafas pada pasien tuberculosis paru.Kata kunci: Fisioterapi Dada dan Batuk Efektif, Ketidakefektifan Bersihan Jalan Nafas

Abstract Background Opioids addiction and misuse are among the major problems in the world today. There have been several preliminary studies examining the effect of methadone on depression among addicts, however, these studies have reported inconsistent and even contradictory results. Therefore, the aim of the present study was to determine the effect of methadone on depression in addicts in Iran and around the world, using a meta-analysis approach. Methods This study was a systematic review and meta-analysis including articles published in the SID, MagIran, IranMedex, IranDoc, Cochrane, Embase, ScienceDirect, Scopus, PubMed and Web of Science databases were searched systematically to find articles published from 2006 to March 2019. Heterogeneity index was determined using the Cochran's test (Qc) and I2. Considering heterogeneity of studies, the random effects model was used to estimate the standardized difference of mean score for depression. Subsequently, the level of depression reduction in Iran and worldwide in the intervention group before and after the testwas measured. Results A total of 19 articles met the inclusion criteria, and were therefore selected for this systematic review and meta-analysis. The sample size of the intervention group in the selected studies was 1948. According to the meta-analysis results, the mean depression score in the intervention group was 26.4 ± 5.6 and 18.4 ± 2.6 before and after intervention respectively, indicating the reducing effect of methadone on depression, and this difference was statistically significant (P < 0.01). Conclusion The results of the present study show that methadone significantly reduces depression in addicts. Therefore, regular methadone use can be part of a drug treatment plan.

1. We measured alveolar carbon monoxide (CO) after a 20 s breath-holding period and carboxyhaemoglobin both before and after smoking a cigarette on 500 occasions (101 individuals). The two measurements were closely correlated but there was a marked difference in the change or ‘boost’ after smoking one cigarette. The mean relative boosts ([post value—pre value]/[pre + post]/2) for alveolar CO and carboxyhaemoglobin were 7.7% and 20.3%, while negative boosts (fall rather than the expected rise) were seen in 103 of 500 and three of 500 occasions respectively. In 140 studies a third alveolar CO reading taken 5 min later was slightly larger, but the difference was insignificant. 2. In seven subjects where the carboxyhaemoglobin level was raised by breathing a 2% CO gas mixture, the alveolar CO and carboxyhaemglobin boosts were similar (71.7% and 75.2% respectively), and they fell sharply subsequently rather than increasing further as occurred after smoking. 3. We conclude that alveolar CO measurements give a useful estimate of carboxyhaemoglobin level if the subject has not smoked for at least half an hour but that measurements of alveolar CO boost are useless since the act of smoking interferes with alveolar sampling. We postulate that cigarette smoking induces a transient change in pulmonary gas exchange.

ABSTRACT Plasma levels of testosterone, luteinizing hormone (LH) and follicle-stimulating hormone (FSH) as well as the response of LH and FSH to the intravenous administration of 100 μg of luteinizing hormone releasing hormone (LRH) were measured in 16 well-trained athletes (mean age 30 years) before and after 2 months of daily oral intake of 15 mg of metandienon, an anabolic steroid (Anabolin®, 17α-methyl-17β-hydroxy-1,4-androstadien-3-one, Medica, Finland). All athletes continued to train regularly, just as they had done for several years. During administration of metandienon the mean plasma testosterone level fell 69%, from 29.4 ± 11.6 nmol/l to 9.1 ± 7.5 nmol/l. The mean plasma levels of LH and FSH also fell significantly (P < 0.001 and P < 0.01, respectively), both about 50%. Because LH and FSH levels were low after administration of the steroid the maximum stimulation values after LRH administration were also lower than pre-treatment values although the mean increments did not differ significantly before and after administration of the anabolic steroid. However, after treatment, the FSH response curve had a biphasic pattern in most subjects, with peaks at 10 to 20 and 50 to 60 min after the iv injection of LRH. Administration of LRH after the treatment period had no effect on FSH secretion in two subjects and no effect on LH secretion in one. Our results show that administration of an anabolic steroid causes a pronounced lowering of plasma levels of testosterone, LH and FSH but causes no gross alteration in the response of LH secretion to stimulation by LRH. The reason for the biphasic response pattern of FSH to LRH administration in most subjects is not known.
