Maternal aging and different biological factors play an important role in the birth of Down syndrome baby. Hormones play a crucial role for the maintenance of female sex cycle and oocyte maturation. Disparity in the level of these hormones during menstrual cycle has profound effect on female reproductive system. Hormonal imbalance also affects meiotic process and integrity of spindle structure and leads to nondisjunction of chromosome. Follicle-stimulating hormone (FSH), anti-Müllerian hormone (AMH) and luteinizing hormone (LH) play a crucial role in ovarian aging and nondisjunction of chromosomes. FSH stands as a hormonal indicator for ovarian aging, and its high level is responsible for aneuploid birth. Advanced chronological age of mother, ovarian aging, environmental factors and accelerated telomere shortening at older reproductive age are found to be risk factors for the birth of trisomy 21 Down syndrome.

Introduction:The high level of morbidity and mortality from cardiovascular disease is largely due toinsufficient influence on the main risk factors that contribute to the development of myocardial infarction.Therefore, a detailed study and assessment of risk factors is among the most important problems of medical and social importance. The aim: To study and evaluate the impact of biological, social and hygienic, social and economic, psychological, natural and climatic risk factors on the development of myocardial infarction. Materials and methods: A sociological survey was conducted in 500 people aged 34 to 85. They were divided into two groups. The main group consisted of 310 patients with myocardial infarction. The control group consisted of 190 practically healthy people, identical by age, gender and other parameters, without diseases of the cardiovascular system. Results: It was defined that 30 factors have a significant impact on the development of myocardial infarction.Data analysis revealed that the leading risk factors for myocardial infarction were biological and socio-hygienic. The main biological factors were: hypertension and hypercholesterolemia. The man socio-hygienic factor was smoking. Conclusions: Identification of risk factors provides new opportunities for the development of more effective approaches for the prevention and treatment of myocardial infarction.

Polycystic ovary syndrome (PCOS) is the most prevalent endocrine and metabolic disorder, affecting 5–10% of women of reproductive age. It results from complex environmental factors, genetic predisposition, hyperinsulinemia, hormonal imbalance, neuroendocrine abnormalities, chronic inflammation, and autoimmune disorders. PCOS impacts menstrual regularities, fertility, and dermatological complications, and may induce metabolic disturbances, diabetes, and coronary heart disease. Comprehensive metabolic profiling of patients with PCOS may be a big step in understanding and treating the disease. The study aimed to search for potential differences in metabolites concentrations among women with PCOS according to different body mass index (BMI) in comparison to healthy controls. We used broad-spectrum targeted metabolomics to evaluate metabolites’ serum concentrations in PCOS patients and compared them with healthy controls. The measurements were performed using high-performance liquid chromatography coupled with the triple quadrupole tandem mass spectrometry technique, which has highly selective multiple reaction monitoring modes. The main differences were found in glycerophospholipid concentrations, with no specific tendency to up-or down-regulation. Insulin resistance and elevated body weight influence acylcarnitine C2 levels more than PCOS itself. Sphingomyelin (SM) C18:1 should be more intensively observed and examined in future studies and maybe serve as one of the PCOS biomarkers. No significant correlations were observed between anthropometric and hormonal parameters and metabolome results.

Abstract Background Chemokine (C–C motif) ligand 19 (CCL19) is a leukocyte chemoattractant that plays a crucial role in cell trafficking and leukocyte activation. Dysfunctional CD8+ T cells play a crucial role in persistent HBV infection. However, whether HBV can be cleared by CCL19-activated immunity remains unclear. Methods We assessed the effects of CCL19 on the activation of PBMCs in patients with HBV infection. We also examined how CCL19 influences HBV clearance and modulates HBV-responsive T cells in a mouse model of chronic hepatitis B (CHB). In addition, C–C chemokine-receptor type 7 (CCR7) knockdown mice were used to elucidate the underlying mechanism of CCL19/CCR7 axis-induced immune activation. Results From in vitro experiments, we found that CCL19 enhanced the frequencies of Ag-responsive IFN-γ+ CD8+ T cells from patients by approximately twofold, while CCR7 knockdown (LV-shCCR7) and LY294002 partially suppressed IFN-γ secretion. In mice, CCL19 overexpression led to rapid clearance of intrahepatic HBV likely through increased intrahepatic CD8+ T-cell proportion, decreased frequency of PD-1+ CD8+ T cells in blood and compromised suppression of hepatic APCs, with lymphocytes producing a significantly high level of Ag-responsive TNF-α and IFN-γ from CD8+ T cells. In both CCL19 over expressing and CCR7 knockdown (AAV-shCCR7) CHB mice, the frequency of CD8+ T-cell activation-induced cell death (AICD) increased, and a high level of Ag-responsive TNF-α and low levels of CD8+ regulatory T (Treg) cells were observed. Conclusions Findings in this study provide insights into how CCL19/CCR7 axis modulates the host immune system, which may promote the development of immunotherapeutic strategies for HBV treatment by overcoming T-cell tolerance.

AbstractWild-type Arabidopsis L. leaves exposed to low ultraviolet-B (U V B ) conditions contained predominantly kaempferol glycosides, with low levels of quercetin glycosides. The flavonoid level doubled on treatment with UVB and an increase in the ratio of quercetin: kaempferol was observed. These results suggest that flavonols protect Arabidopsis plants from UVB damage, and indicate that the flavonoid 3’-hydroxylase (F3’H) enzyme, which converts dihydrokaempferol to dihydroquercetin, may play a crucial role. The tt7 mutant lacks this gene and, after treatment with sub-ambient UVB, contained kaempferol glycosides exclusively, to a level of total flavonols similar to that in wild-type Arabidopsis. Total flavonols after enhanced UVB treatment were higher in tt7 than in similarly treated wild-type plants, and only kaempferol glycosides were detected. Despite this high level, tt7 plants were less tolerant of UVB radiation than wild-type plants. These observations suggests that kaempferol is a less effective photoprotectant than quercetin. The chalcone isomerase (CHI) mutant (tt5) surprisingly did not accumulate naringenin chalcone, and this suggests that the mutation may not be restricted to the CHI gene alone. The concentration of hydroxycinnamic acid derivatives did not change with UVB treatment in most varieties indicating that their role in UV photoprotection may be subordinate to that of the flavonoids.

This article examines the demographic situation in Dnipropetrovsk region and identifies the main demographic problems of the region , among which the most important are low fertility, reduced family size, high divorce rates and unregistered marriages, contimuing aging of the population, increased gender disproportion among the population, intra-regional and especially modern external migrations of the population, etc. The Dnipropetrovsk settlement system includes four industrial agglomerations: Dniprovsky-Kamianske, Kryvyi Rih, Nikopol and Pavlohrad, where more than 83.5% of the region’s population lives , indicating a high level of urbanization of the territory. It was found that the dynamics of the population of the urban agglomerations of the Dnipropetrovsk region over the past 20 years has a tendency to progressive reduction: if in 1997 the size of the existing population amounted to 3,888.8 thousand people, in 2017it amounted to 3,227.5 thousand people. It is stated that the natural replenishment of the population provides a continuous change of generations, creating the quantitative boundaries of labour resources of urban agglomerations of Dnipropetrovsk region. The current state of migration processes in the agglomerations of Dnipropetrovsk region is caused by socio-economic and political problems. This has led to an increase in labour out-migration, which now has already become catastrophic, not only in the Dnipropetrovsk region, but throughout Ukraine. Negative consequences of this are the depopulation of cities and areas of agglomerations, the outflow of high quality labour potential (and in particular qualified labour), the collapse of families, rise in crime and corruption, etc. In general, it is established that the demographic aspect, due to natural and mechanical movements, has become a determining factor inthe formation of the labour resource situation and affects not only the process of replenishmnent itself, but also the structure of the economically active population. We present the general demographic features of the population's reproduction, in the urban agglomerations of Dnipropetrovsk region from 1997 to 2017; a decrease in the total population, a negative natural population growth (mortality rates will prevail over birth rates), aging of the population, which is reflected in an increase in the number of people aged 70, disproportion in the sexual structure of the population (prevalence of women), negative balance of migration and continuous outflow of inhabitants of reproductive age, especially to the countries of the European Union.

Polycystic ovarian syndrome (PCOS) is a probably fastest growing endocrinological disorder in females of reproductive age. In PCOS, follicular growth is influenced by hormonal imbalance during the ovarian cycle, leads to affected follicles remain in the ovary. The prevalence of PCOS is estimated 6-10% globally and 3.7% - 22.5% in Indian female population, which is relatively high. It is characterised by hyperandrogenism, anovulation and polycystic ovaries which clinically manifests in the terms of amenorrhea or oligomenorrhea, hirsutism, acne, infertility etc. In present scenario, incidence of this disease increasing exponentially due to sedentary lifestyle and faulty dietary habits. This particular disease is not described word to word in Ayurveda. Associated features of PCOS are closely resembling with Bandhya Yonivyapada, Artavavahasrotasa-Vidhha Lakshana, Nastartava and Ksheenaartava described by Acharya Sushruta and Pushpghani Jatharini and Vikuta Jatharini mentioned by Acharya Kashyap. Maximum congruence of PCOS can be established with Bandhya Yonivyapada. The purpose of this study is to understand Nastartava which is the cardinal feature of Bandhya. Word Artava has been used exclusively in Samhita in context of menstrual blood, ovum and ovarian hormones. Therefore Amenorrhea, anovulation, hormonal dysfunction is considered exposed manifestations of Nastartava. Possible line of treatment is stipulated with Nidanaparivarjan, herbal drugs and Panchakarma procedures. Keywords- Ayurveda, Artava, Nastartava, PCOS, Herbal drugs, Panchakarma

Background: Abnormal Uterine Bleeding (AUB) is one of the commonest gynecological complain in reproductive age group. Menstrual abnormalities are commonly seen when there is any alteration in thyroid function. Objective of present study was to investigate the prevalence of AUB and to determine the menstrual pattern in cases with thyroid dysfunction.Methods: The present cross sectional observational study was conducted Teerthankar Mahaveer Medical College and Research Center Moradabad. Total 400 cases presenting with AUB were included in the study. Routine blood test, ultrasonography and thyroid function tests were done in these cases.Results: Among all the cases presenting with menstrual abnormalities 26% had hypothyroidism and 9% have hyperthyroidism and rest had euthyroid status. Menorrhagia (45.2%) and polymenorrhoea (37.5%) were commonest menstrual abnormality seen in cases with hypothyroidism. Most cases with hyperthyroidism presented with hypomenorrhoea (27.8%).Conclusions: Thyroid function abnormality is common in cases presenting with AUB and it gets relieved in correcting hormonal imbalance.

Intracerebral haemorrhage is one of the uncommon initial CNS manifestations of choriocarcinoma in reproductive age group women. Disease is usually diagnosed by history, clinical examinations and investigations like CT scan of brain and chest, MRI of brain, USG, X-ray chest P/A view and high level of â-hCG in serum and CSF. Overall prognosis of this disease is generally good (80-90% long term survival with chemotherapy and radiotherapy). But intracranial metastasis has poor prognosis which comprises 3-28% of choriocarcinoma. Most intracranial metastasis occurs late in the course of disease but 20% experiences as first sign. Our patient is a teenager girl presented with convulsion and loss of consciousness. CT scan of brain revealed hemorrhage in right fronto-parietal region with ventricular extension, X-ray chest P/A view shows metastasis in mid zone of right lung, USG shows invasion in posterior myometrium and high concentration of serum â-hCG (273400 mUI/ ml) confirmed our diagnosis. This case report describes that the conservative treatment with radiotherapy and chemotherapy gives good outcome of with metastasic intracerebral haemorrhage with choriocarcinoma is rewarding with farther radio and chemotherapy. Bangladesh is a country having high density of population in the world. Its fertility rate ranges from 4.1 to 5.49 and life expectancy is 66years while the total fertility rate of Asia is 2.2 and life expectancy 70 years. This descriptive type of cross sectional survey was carried out among 240 married women of reproductive age from July to December, 2012 in some villages of Keranigonj, Upazila, Dhaka. 73(31%) were practicing some methods of contraception, while 167 (69%) were not using it. OCP (Oral Contraceptive Pill) was the commonest method of contraception followed by Condoms 12(5%), Injectable 12(5%), Implant 12(5%) & Tubectomy 6(3%). None was found using IUCD and Traditional method ( withdrawal, rhythm method ) and emergency contraceptive method. The use of contraceptive was more common in grand multipara (p<0.01), >35 years old ladies (p<0.05). Non users of contraceptives in this study were 167 (69%) and the major reason for the non use was intention to have more children 53(31.46%) followed by pressure from the husband 21(12.35%), prohibition by the religion 18 (10.9%) and desire for son 17 (10.11%). Among the 73 contraceptive users 38 (52%) experienced side effects with the use of contraceptives. The commonest side effects were menstrual irregularities 17(23.8%) followed by change in body weight 8(11.19%). Frequency of contraceptive use was found comparatively low among rural married women despite high level of awareness. Desire for larger family, religious concerns and fear of side effects were the main factors responsible for non users. Religious scholars must play their role in clarifying many aspects regarding contraceptives. Hyperandrogenism syndrome, the main manifestations of which are pathological hirsutism, acne, oily seborrhea, androgenic alopecia, accompanies a wide range of diseases of the female reproductive system, manifested by various disorders of menstrual function and infertility. Isolated hirsutism is also a serious psychological and cosmetic problem, especially since almost one in ten women of reproductive age have excessive hair growth. To get rid of "excess" hair, women use various methods. However, in severe manifestations of hyperandrogenism or in the case of severe and moderate hirsutism, it is advisable to use systemic antiandrogenic therapy.
